Abnormal lactic dehydrogenase isoenzyme patterns in ulcerative colitis with precancerous change.

The isoenzyme pattern of lactic dehydrogenase was measured in rectal biopsies from six patients with ulcerative colitis in whom precancerous histological changes had been independently recognized. There was a highly significant increase in the ratio of isoenzymes IV + V to I + II. This suggests that lactic dehydrogenase isoenzyme measurement may prove a valuable addition to histology in the recognition of precancer in ulcerative colitis.

Solitary ulcer of the rectum.

Solitary ulcer of the rectum is usually a disease of young adults of either sex which has a characteristic appearance on sigmoidoscopy. Distinctive changes may also be seen in biopsies taken from mucosa adjacent to the ulcer. The name ;solitary ulcer' is misleading because more than one ulcer may be present. Moreover, there is a preulcerative phase which is clinically and histologically recognizable. The condition is essentially benign and may persist for many years unchanged. It has not responded satisfactorily to medical or surgical methods of treatment. The cause of solitary ulcer is unknown. Different views on the pathogenesis are discussed.

Chagas disease of the colon and rectum.

Chagas disease is a rare condition in patients anywhere but in Central and Latin America. This case is therefore reported because the patient was born in Britain and spent only a short time in an area where Chagas disease is endemic. It is believed that this patient is the first case of Chagas disease to be treated in Britain. Chagas disease causes a generalized degeneration of autonomic ganglia and usually presents as megaoesophagus or megacolon. The disease may also cause the heart to enlarge followed by cardiac failure.
