[Endoscopic and surgical therapy of malignant colorectal polyps].

87 malignant colorectal polyps were removed in 81 patients by endoscopic polypectomy from 1972 until 1987. Thereafter 34 of these patients had a surgical resection of the colon. Surgical resection was performed for incomplete excision of the polyp (20 x), for tumor invasion of the lymphics in the polyp stalk and for moderately/poor differentiated carcinoma. In 6 patients a residual carcinoma was detected in the removed colon specimen, but lymph-node metastasis only in one with moderate-differentiated carcinoma. Our results demonstrate that colorectal polyps with invasive well-differentiated carcinoma and tumor free base do not need a surgical resection. Patients of the high-risk-group (moderately/poor-differentiated carcinoma, invasion of blood vessels and lymphatics, incomplete excision), should be referred to further surgical resection. With regard to the low rate of metastatic spread even in patients with high-risk-polyps accurate differentiation seems to be necessary. In 19 out of 52 (36.5%) patients endoscopic control examinations after surgical or endoscopic resection revealed recurrent pathological findings. On the base of the presented data we conclude that there is a need for short-term endoscopic controls over a period of five years.

[Therapy of stomach ulcer with sucralfate and ranitidine. A multicenter double-blind study].

134 outpatients with acute benign gastric ulcer confirmed by endoscopic biopsy received either 1 g sucralfate suspension four times daily or one 150 mg ranitidine tablet twice daily for six to 12 weeks in a multicentre therapy study (double-blind study according to the double-dummy technique). After six to 12 weeks, respectively, 56% and 82% of the ulcers had healed in the sucralfate group. The rates of healing in the ranitidine group were 72% and 88%, respectively. The differences in the rates of healing between sucralfate and ranitidine were not statistically significant. Ranitidine was superior to the sucralfate suspension in corpus ulcer. In the distally located ulcers, the two kinds of treatment were equivalent. There were no appreciable differences between the medications with regard to antacid consumption and compliance. Gastric pain was influenced better by ranitidine than by sucralfate.

[New aspects of early gastric carcinoma].

Between 1st January 1970 and 31st December 1984, 910 total or subtotal resections for gastric cancer were carried out in the Department of Surgery of the University Clinic Mannheim. A work-up of operative specimens showed that 146 patients (16%) had early gastric carcinoma. The patients comprised 97 men and 49 women; the youngest was 31, the oldest 86 years. The following trends were apparent when the 58 patients operated on between 1970 and 1976 were compared with the 68 patients operated on between 1977 and 1982: An increase in early gastric carcinoma is also detectable in younger patients. The previously seen tendency for localisation in the antrum appears to have changed in favour of the upper segments of the stomach. The proportion of the diffuse type rose from 26% in the first group to 44% in the second. The 5-year survival of all patients with early gastric carcinoma was 78.8%. No significant difference could be found between the diffuse and intestinal type. The prognosis of early gastric carcinoma appears to be dependent on the depth of invasion and involvement of the lymph nodes.

[Implantation of ultra thin naso-duodenal and naso-jejunal tubes for feeding].

Enteral nutrition through filiform naso-duodenal or naso-jejunal tubes with homogeneous low molecular nutritional solutions has a rather important place in therapy of different conditions; endoscopy can help to introduce these tubes into the intestinal tract in the following situations: A leak in the upper gastrointestinal tract, which may occur after surgical resections of the esophagus or stomach, or which may occur in rare cases after bouginage or after diagnostic procedures can be passed by with an ultrathin tube until it is healed of. Patients with stenotic lesions of the esophagus or the cardia, which can not be treated by surgery or conservative procedures like pertubation and dilation can be fed enterally if it is possible to move the tube beyond the stenosis. Patients with neurogenic swallowing dysfunction of different origine can be fed during long periods of time through such filiform naso-enteral tubes successfully and reach a perfect nutritional state. In acute treatment of Crohn's disease and other inflammatory intestinal diseases enteral nutrition through tubes is an essential therapeutic procedure.

[Fibrin gluing of congenital esophageal malformations].

Gluing of oesophageal anastomoses in atresia effected by means of three or four sutures will definitely produce better results than suturing alone. The results of endoscopic closure of tracheo-oesophageal fistulas by gluing with fibrin are excellent. Furthermore, it is possible with fibrin gluing alone to treat iatrogenic perforations of the oesophagus in the newborn, following a false passage of nasogastric tubes from the hypopharynx into the mediastinum or pleural cavity.

[Recognition and management of perforation of the gastro-intestinal tract during endoscopy (author's transl)].

In the course of 71/2 years there occurred 37 cases of perforation of the gastro-intestinal tract after endoscopic investigation or operation in the abdominal region. Analysis of the therapeutic results indicates that immediate laparotomy in case of confirmed perforation provides the best survival chance. In individual cases with pneumoperitoneum and no symptoms careful supervision in a surgical intensive care unit before surgical intervention can be justified.

[Obstructive jaundice of benign and malignant origin: endoscopy in diagnosis and therapy].

Laparoscopy and laparoscopic cholecysto-cholangiography for diagnosis of obstructive jaundice have been superseded by ERCP. ERCP is indicated in all patients with bilirubinaemia over 2 mg%. Surgery for obstructive jaundice should be abandoned in favour of surgery for common bile duct concretion, benign or malignant stenosis or periampullary neoplasia, as diagnosed by ERCP. Endoscopic sphincterotomy (EST) for recurrent common bile duct concretion is preferred in patients more than 60 years old. Biliary drainage techniques for litholysis or palliative tumour therapy have widely enlarged the spectrum of endoscopic surgery.

Early postoperative endoscopy in the operated stomach.

Early endoscopy in the upper alimentary tract after surgical operations on the stomach reveals multifarious diagnostic and therapeutic possibilities to give decisive help to patients when applied critically and rationally. The possibilities of endoscopy can differentiate between active and arrested bleeding episodes, can induce hemostasis, can diagnose and overcome sutureline insufficiencies, can differentiate the different kinds of stenosis in the lower esophageal junction. It can treat postoperative atony, diagnose perforation at an earlier stage, localize obstructive jaundice, remove intestinal foreign bodies and characterize unexpected postoperative histological findings.

[Surgical therapy of early gastric carcinoma (author's transl].

From January 1970 to September 1980 101 patients with early gastric carcinoma were operated. The youngest patient was 33 years, the oldest 83 years old. The incidence was highest in the 7th decade. Two thirds of all early gastric carcinomas were localized in the prepyloric region. 4 of these tumours were found in the gastric stump after previous B II resection. 3 multifocal carcinomas were found in our series. According to Laurën's classification 70 early gastric carcinomas were of the intestinal type, 28 of the diffuse type, in three cases a correct classification could not be performed. The predominant operative procedure was the B II resection in 71 patients. Further methods were total gastrectomy 15, B I resection 4, proximal resection 4, endoscopic polypectomy 3, Maki-resection 2, local excision 2. In the postoperative period complications occurred in 9 patients; four of them died. The 5-years survival rate was 72%. In 4 patients a recurrence of the malignancy was detected, in 3 of them another early gastric carcinoma and in 1 an advanced gastric carcinoma.

[Congenital dilatations and stenoses of the bile ducts in children (author's transl)].

The congenital choledochus cyst is an extremely rare disease pattern. The author presents two own cases of boys aged 4 1/2 years in whom the disease was manifested as a fusiform dilatation of the cholodochus and of the other bile ducts on the one hand, and by a cyst in the second case. Both cases were characterised by an additional annular stenosis of the cholodochus immediately before the junction of the pancreatic duct. The article discusses the most essential criteria in respect of clinic, diagnosis and therapy, as well as the etiological aspects.
