Fatal acute tubular necrosis occurred in 1 patient in whom intravesical formalin was used to control massive persistent hemorrhage from radiation cystitis. A suggestion is made to monitor blood formic acid levels and institute prompt dialysis whenever these exceed 80 mg. per 100 ml. to avert such a catastrophe. Intravenous sodium bicarbonate appears to be indicated prophylactically in combating the associated metabolic acidosis due to absorbed formic acid.
Of 51 patients with detrusor-sphincter dyssynergia undergoing external urethral spincterotomy by transurethral resection, histological examination revealed no skeletal muscle in the resected tissue in six patients. The functional improvement which resulted in these cases despite this finding with an explanation for the same forms the topic of this paper.
We present a patient with electrical burns which resulted in loss of the right upper arm and a urinary bladder fistula. A jump flap was used to cover the repaired bladder, and also to cover the stump of the amputated right arm.
Primary benign mesodermal tumours of the ureter mostly occur in the proximal part of the ureter. A case of a large fibrous polyp of the lower end of the ureter is described. The clinical presentation, radiological findings and treatment are discussed.
Acute obstructive uropathy, following an abdominal hysterectomy in a 45-year-old woman, had been temporarily circumvented by bilateral nephrostomy. Reconstruction of the upper urinary tracts, in the face of a small, defunctionalized bladder, was successfully accomplished by a Boari flap ureteroplasty and transureteroureterostomy.
Infusion pyelography was performed in 103 anuric patients with no attendant mortality. It was useful in 83 cases in distinguishing obstructive uropathy from renal parenchymal disease. However, in the remaining 20 cases when either renal parenchymal disease co-existed with obstructive uropathy or obstructive uropathy existed alone under certain circumstances, opacification of the collecting system did not occur. The clinicopathological setting of some of the latter group of cases is illustrated by examples.
The possibility of massive venous hemorrhage after bilateral internal iliac artery ligation for postprostatectomy bleeding is illustrated by this case report. Emergency measures to differentiate the arterial from the venous sources of such a hemorrhage and their influence on the subsequent management are discussed.
Reports of Melon et al. [7] and Hodgkinson [4] regarding the efficacy of succinic acid or its derivative (salt) in reducing the endogenous synthesis of oxalate are conflicting. The possible application of succinic acid as a potential inhibitor of oxalate synthesis, and the biochemical alterations resulting therein, has been studied in 10 idiopathic stone-forming patients (6 primary stone-formers and 4 recurrent stone-formers) and in 21 normal adult rats. In rats no significant changes in oxalate excretion were observed during the 4 weeks of succinic acid administration (500 mg/100 g BW/day) and 2 weeks therafter. In the clinical trial of 3 months, where stone-formers were given 20 g/day of succinic acid, the post-therapy values of urinary oxalate excretion showed a singificant drop ( less than 0.001) as compared with those of pretherapy--recurrent stone-formers showing a more marked decrease (63%) than primary stone-formers (49%).
A rare case of multiple urethral metastases from adenocarcinoma of prostate causing urinary retention is reported. The unique findings of the urethrogram are described. Transurethral resection provided symptomatic relief. No local recurrence of tumor was observed during the follow-up period of sixteen months.
A case of blowout of the colonic segment of a colocystoplasty at its free end owing to late development of bladder neck obstruction is described. Prostatitis and secondary bladder neck contracture provided further hindrance to the already inefficient voiding mechanism and predisposed to the rupture of the decompensated bowel loop. This is an extreme example of proof that the colonic loop, except as an improved substitute for an ileal segment, cannot withstand high intraluminal pressures generated in the presence of bladder outlet obstruction.

Considerable controversy exists regarding the ideal anaesthetic coverage for external urethral sphincterotomy in spinal injury patients with detrusor-sphincter dyssynergia. From this preliminary experience on 34 patients in whom this procedure was indicated and then attempted under topical (urethral instillation of 20 ml of 2 per cent Lignocaine jelly) or no anaesthesia, we are encouraged into believing that these patients may not require general or regional anaesthesia for this operation.

The spectrum for the use of retrograde pyelography in renal failure has been narrowed further by extending the facility of infusion urography to anuric patients. The latter study has helped greatly in sifting apart pre-renal and renal anuria patients from those with obstructive uropathy. In obstructive anuria infusion urography has proved comprehensive in influencing judgment about the exact site and nature of obstruction, in affixing laterality towards the better functioning renal unit and in deciding a suitable definitive operation or planned staged procedures.
