Treatment of congenital glaucoma.

We reviewed 37 consecutive patients who had undergone at least one goniotomy, filtering surgery, and patching for amblyopia because of congenital glaucoma between 1969 and 1979. Of 32 eyes treated for uncomplicated congenital glaucoma, 25 (78%) achieved satisfactory control of intraocular pressure. Seven of the 12 eyes (58%) for which Snellen visual acuities could be obtained had visual acuities of 6/15 (20/50) or better. Of eight eyes treated for complicated congenital glaucoma, four (50%) achieved satisfactory control of intraocular pressure, but the visual acuities of these patients could not be tested.

Halogeton poisoning in range cattle.

Acute Halogeton glomeratus poisoning occurred in 16 of 680 range cattle during and following a trail drive. Signs of toxicosis included posterior ataxia, recumbency, coma, and death. Histopathologically, abundant, refractile calcium oxalate crystals were seen in renal tubules. Inasmuch as the plant is generally unpalatable for cattle, poisoning in this case was enhanced by a preceding period of food deprivation.

The effect of magnesium ions on the binding of calcium ions to glycerinated rabbit psoas muscle fibers.

The effect of Mg2+ on Ca2+ bound to glycerinated rabbit psoas muscle was studied by means of a double-isotope technique. The troponin-C content of the fibers was analyzed by quantitative sodium dodecyl sulfate polyacrylamide gel electrophoresis and found to be 0.5 mumol/g fiber protein. In the absence of Mg2+ the fibers bound a maximum of 3.5 mumol Ca2+/g protein. This value could be readily accounted for in terms of the four Ca2+-binding sites of troponin-C and the two divalent cation-binding sites of myosin. In the presence of 1 mM Mg2+ the entire Ca2+ titration curve was shifted downward with a maximum bound Ca2+ of slightly more than 2 mumol/g, or 4 mol Ca2+/mol troponin-C. Further increase in Mg2+ concentration to 10 mM had little effect on Ca2+ binding when the free Ca2+ concentration was in the upper part of the physiological range (5 . 10(-7)-5 . 10(-6) M) but caused a marked reduction when the Ca2+ concentration was low. The results are consistent with biochemical data showing two groups of (Mg2+/Ca2+)-binding sites. One group, located in myosin, has a high affinity for Mg2+ while the other group, located on troponin-C, has a low affinity for Mg2+. If the free Mg2+ concentration in muscle is in the range of 2--5 mM, as suggested by recent data, it can be inferred that the binding sites on myosin will never be occupied by Ca2+ under physiological conditions.

Myeloma proteins from NZB and BALB/c mice: structural and functional differences.

Structural and functional analyses of myeloma immunoglobulins from inbred BALB/c mice and humans have provided important insights into the structure of the antibody molecule and the expression and evolution of antibody genes. One important question concerning these analyses is whether the myeloma process selects, in a nonrandom manner, the lymphocytes to be transformed. The availability of myeloma tumors in a second inbred strain of mouse, NZB, permits us to approach this question. In this respect. the NH2-terminal amino acid sequences of 27 kappa light chains as well as data relating to the antigen-binding properties and immunoglobulin class distribution of NZB myeloma proteins are presented and compared with similar data from the BALB/c mouse. These studies suggest that the myeloma proteins from the BALB/c and NZB mice constitute two populations of immunoglobulins with distinct functional and structural properties. The implication of this observation are discussed.

BIPP induced artefacts on computerized tomography.

A case of BIPP induced artefacts on computerized tomography (CT) is presented. Because of the value of CT as a non-invasive intra-cranial investigation, the use of radio-opaque materials in the site to be investigated is to be avoided. In cases likely to require the use of CT, a substitute for BIPP packs in ear surgery is now required.

Branchiogenic carcinoma.

An unusual case of upper cervical carcinoma probably of true branchial origin is presented and the literature relevant to this condition reviewed. Branchiogenic carcinoma is a rare condition and should not be diagnosed in the absence of the 4 criteria laid down by Dr Hayes Martin, viz. that the carcinoma should arise in the wall of a branchial cyst, that it should occur along a line from the tragus to the clavicle along the anterior border of sternomastoid, that the histology should be compatible with the branchial apparatus and that no other primary tumour should come to light in a 5-year follow-up. A fifth criterion, ie. the demonstration of premalignant changes in the epithelium of the cyst, is suggested to aid a conclusive diagnosis.

A study of five year old children born after elective induction of labour.

A controlled retrospective study of some short-term and long-term effects of elective induction of labour was conducted. There did not appear to be any increase in the incidence of maternal complications during labour, or of neonatal problems in the induced-labour group compared with the remainder of the mothers in the hospital. In the follow-up phase of the study, two treatment groups of children who were delivered after amniotomy and amniotomy plus the administration of oxytocin were compared with each other, and with a control group of children born after spontaneous labour. The children were assessed at the age of five years on verbal and non-verbal subtests of a standardized intelligence scale, tests of gross motor and fine motor coordination, and auditory and visual tests. A full physical examination was also performed. No statistically significant differences nor trends of clinical interest were found between the groups on any measure.

Experiences in homograft membrane tympanoplasty.

Experience of a short series of 37 cases of tympanoplasty involving the use of a homograft tympanic membrane is presented, with a discussion of the problems involved. The initial results correspond to the earlier series' anatomical success rates and indicate that these grafts will become widely used as an effective method of tympanoplasty.

Antibody differentiation: apparent sequence identity between variable regions shared by IgA and IgG immunoglobulins.

We have analyzed a pair of human myeloma immunoglobulins (biclonal proteins) of the IgG and IgA classes from a single patient, GR. The light chains are identical in amino-acid sequence over 40 residues at their NH2-terminus, hwereas the heavy chains are identical throughout 45 residues of their NH2-terminus. Additional chemical and serological studies suggest the light chains and variable regions of the heavy chains (VH) are very similar, if not identical. The implications of these and of other published studies are discussed with regard to (i) the association of one VH region with multiple constant regions of the heavy chain (CH regions), (ii) two alternative types of V-C joining mechanisms, (iii) the differentiation of antibody-producing cells, and (iv) three categories of biclonal immunoglobulins.

Lack of efficacy of the adenosine reuptake inhibitor dipyridamole in the treatment of anxiety disorders.

We administered dipyridamole, an adenosine reuptake inhibitor, to 12 outpatients with DSM-III-R anxiety disorders (2 patients with generalized anxiety disorder, 10 patients with panic disorder). Dipyridamole was administered at a flexible dose in a single-blinded fashion following a placebo washout phase and elimination of placebo responders. The mean duration of active treatment with dipyridamole was 46 days (range 21-88 days); the mean peak dose of dipyridamole was 202 +/- 55 mg/day (range 100-300 mg/day). Symptom ratings were completed at regular intervals by the patient and by a research nurse unaware of the treatment condition. Clinically significant improvement in anxiety symptoms was not demonstrated. The implications of these findings for an adenosinergic dysfunction model of panic disorder are discussed.

Neurotoxic radiosensitizers and head and neck cancer patients--how many will benefit?

The causes for the exclusion of patients from a Phase I dose tolerance study of metronidazole as a hypoxic cell sensitizing agent in patients receiving radiation therapy for head and neck cancer have been reviewed. One hundred and fifty nine consecutive patients were assessed between October 1979 and December 1980 according to eligibility criteria decided upon prior to the study. Only 26 (23%) of 111 patients treated with radical radiation therapy entered the Phase I study. The major reasons for exclusion were a history of prior nervous system abnormality, age over 70, and refusal by some patients to participate in the study. The criteria used for patient selection for studies of hypoxic cell sensitizers (and other investigational agents) must be known so that data from each study can be assessed appropriately before being extrapolated to the general patient population. Clinical trials should be designed to take into account factors that might influence patient entry.
