A method for the differential determination of plasma antithrombins.

A method for the differential determination of plasma antithrombins, antithrombin III and alpha2 macroglobulin, is described. The method is based on the selective inactivation of plasma alpha2 macroglobulin by treatment with 0-1 M methylamine for 10 minutes at 37 degrees C and on the observation that antithrombin III and alpha2 macroglobulin inhibited in defibrinated plasma low concentrations of thrombin without mutual interference and according to pseudo-first order reaction. In healthy subjects antithrombin III was shown to account for about 70% of the total antithrombin activity. But in patients with liver cirrhosis, where low levels of total antithrombin activity were observed, the relative contribution of antithrombin III was found to be noticeably lower.

Pretreatment with heparin in thrombolytic therapy with Urokinase. Heparin-urokinase therapy.

Initially, we administered urokinase to five patients according to the following schedule: 500,000 CTA U during the first 10 min, then 250,000 CTA U/h for 12 h. Using this modality, we noted the appearance, during the first hours of treatment, of hypercoagulability. We then choose to modify the schedule by pretreatment with 7,500 U. i.v. of heparin, followed promptly by 250,000 CTA U/h of urokinase (without a loading dose). This obviate the appearance of hypercoagulability without reducing the fibrinolytic effect of treatment and without producing hemorrhagic complications.

Discrepancies between immunological and caseinolytic plasma plasminogen assays in health and hyperfibrinolytic states.

A sensitive immunological assay for plasminogen and/or plasmin was developed by using a haemagglutination inhibition technique. Plasma and the corresponding euglobulin fraction of healthy subjects and of patients with hyperfibrinolysis (liver cirrhosis, thrombolytic treatment) were assayed for plasminogen using immunological and caseinolytic techniques. In hyperfibrinolytic states discrepancies were found between immunoreactive and caseinolytic plasma plasminogen whereas a good correlation was observed between immunoreactive and caseinolytic euglobulin plasminogen. Experiments in vitro suggested that these discrepancies might depend on the presence in hyperfibrinolytic plasma of variable amounts of plasmin(ogen)-related antigen which has no caseinolytic activity and is not precipitated with euglobulin.

Factor VIII complex in myelomatosis and related disorders.

The behaviour of the factor VIII/von Willebrand factor complex (VIII:C, VIIIR:Ag and VIIIR:RCof) was investigated in 23 patients with secretory myeloma, in 2 patients with non-secretory myeloma and in 5 patients with macroglobulinemia. In most patients (21 of 25 patients with plasma cell myeloma and 2 of 5 patients with macroglobulinemia) VIIIR:Ag was increased usually without corresponding increases in VIII:C and VIIIR:RCof. There was no correlation between the VIIIR:Ag levels and paraprotein Ig type or level nor with the presence or the absence of Bence Jones proteins in serum and urine. Furthermore, increased levels of VIIIR:Ag were found in patients with non-secretory myeloma. In general, VIIIR:Ag was higher in patients with extensive bone lesions and there was a significant correlation between cell mass and the VIIIR:Ag level. The crossed-immunoelectrophoresis of plasmas with discrepant VIIIR:Ag and VIIIR:RCof showed variation from the normal pattern.
