We presented our results with silicone oil in vitreoretinal surgery in pediatric patients. The retinas reattached in 58% but corneal opacification occurred in 75% and glaucoma in 25%. Our results indicate that silicone oil should be considered in otherwise hopeless cases of retinal detachment surgery in children.
Because of the conflicting opinions in the ophthalmic literature concerning the efficacy of Ketamine anesthesia in strabismus surgery, we decided to report out experience in 44 children operated upon between the ages of six months and six years. Satisfactory anesthesia was achieved in all cases except two who required intubation and inhalation technique in order to complete the surgery. No serious side effects were observed during or after the surgery. Its ease of administration, freedom from side effects, and satisfactory anesthesia, make Ketamine our primary anesthetic choice for strabismus surgery in children.
The rare association of two syndromes of paradoxical innervation, bilateral Duane's retraction syndrome and congenital crocodile tears is presented. This association assumes a central localization to explain both anomalies.
A case of ptosis due to giant papillary conjunctivitis in a 24-year-old female is described. The contact lens was removed and the ptosis disappeared. A possible etiology is discussed.
A family is described in which four of six siblings have congenital hereditary corneal dystrophy associated with esotropia. All cases had been erroneously diagnosed as and operated on for congenital glaucoma. The hereditary aspect of this association is discussed.
A 6-year-old boy had the combination of Mobius Syndrome and the Poland anomaly. Although rare, this association is repeatedly encountered. The absence of the pectoral muscle in the Poland anomaly supports the theory that in both conditions the muscular and skeletal abnormalities are due to a common fetal mesodermal defect.
Congenital anomalies of the eye occurring in Potter's syndrome have been described, including leucoma cornea, cataract and prolapse of the lens, and expulsive hemorrhage in one eye. The etiological aspects of this association are discussed.
There were 3,375 children between the ages of 6 months and 3 years screened for refractive errors and strabismus. In 363 children (10.7%) some kind of pathology was detected. We suggest that continued efforts should be made to lower the age of detection of defects in vision.
A 12-year-old hemophilic boy suffered from secondary glaucoma in his right eye due to total hyphema following trauma. An emergency operation was performed to evacuate the hyphema under a tight anti-hemophilic treatment. A careful postoperative treatment with factor VIII concentrate, antiglaucomatous drugs and aminocaproic acid was administered. Intraocular pressure returned to normal and remained so during the four months follow-up period. Visual acuity returned to 6/9 and there was no rebleeding into the anterior chamber. To the best of our knowledge, this is the first reported case of traumatic hyphema and of an operation to wash this hyphema in a hemophiliac. The dilemma of management of such a case is discussed.
A 32-year-old man had a history of acquired Brown's syndrome associated with diplopia, proptosis, downward displacement of the globe, and lid edema. A CT scan of the orbit revealed an osteoma arising from the left frontal sinus and extending into the left orbit. After surgical extirpation the proptosis and diplopia disappeared and the globe returned to normal. To our knowledge, such a case has not been reported previously.
Conventional surgical management of surgically undercorrected esotropia includes bilateral lateral rectus resection, marginal myotomy, and Faden operation. During a 3-year period, 10 patients with surgically undercorrected esotropia had medial rectus re-recession 13.5 mm from the limbus. Successful binocular realignment (to 10 prism diopters or less residual binocular misalignment) was achieved in 9 of the 10 cases for both near and distance measurements.
