The transition in practice: policy makers and purchasers.

The transition to CFC-free pressurised metered dose inhalers (pMDIs) is not optional and cannot be ignored. The UK Government intends that CFCs will be removed from all medicinal products by the end of 1999. An overall strategy is needed to guide health professionals and patients through the transition process. Collaborative working between the pharmaceutical industry, health authorities and relevant health professionals is essential to ensure a smooth transition with minimal disruption to patients. The factors that need to be taken into consideration for a smooth transition and the role of national and local drivers in this process are outlined. The main focus around change must be on patients' confidence in their medication.

Mechanisms involved in iontophoretic transport of angiotensin.

PURPOSE: The feasibility of using iontophoresis to enhance the permeation rate of a model peptide was investigated in vitro using hairless mouse skin. METHODS: Angiotensin 2 (AT 2) was employed as a permeant probe, using optimum iontophoresis conditions. A number of physicochemical parameters (donor ionic strength; valence of competitive ions; pH of donor solution) were studied with the aim of exploring the mechanisms involved in the iontophoretic transport through the skin: electrokinetic transport or convective transport. For this purpose, the magnitude of the convective solvent flow was also evaluated by the permeation of (3H) H2O. The interest of pulsed currents for peptide delivery was also investigated and the effect of current density and frequency was studied. RESULTS: AT 2 transport was found to be enhanced 20-fold in comparison to passive permeation and was found to be proportional to the current density with direct currents as with pulsed currents. CONCLUSIONS: Although the flux enhancement of ions during iontophoresis is due principally to the electrical potential gradient, secondary effects such as convective solvent flow contribute also to flux enhancement of peptide delivery. This effect is dependent of physicochemical conditions of formulation.

Reference services for a wellness program.

The concept of wellness, although an old one, has recently become an important part of health education. The theory behind wellness is that the person tries to achieve a state of self-directed wellness with the guidance of facilitators and educational materials, rather than being directed by an instructor. In 1981, York Hospital, York Hospital Library and the York County Public Library System, through an LSCA Title III grant, joined forces to promote wellness in the community through a series of Wellness Institutes and a combined, computerized health education bibliography, of which an updated edition is attached.

Comparison of qucik and slow thaw methods of producing cryoprecipitate antihaemophilic factor from fresh and 24-hour-old blood.

Cryoprecipitate antihaemophilic factor concentrate was prepared from fresh and 24-hour-old blood by quick and slow thaw methods. Recovery of factor VIII was greater by the slow thaw method and there was less loss into the supernatant plasma. Cryoprecipitate produced from fresh blood contained more factor VIII than that produced from 24-hour-old blood so that the most potent concentrate was produced from fresh blood by the slow thaw process. An adequate therapeutic preparation was, however, produced by the slow thaw method using 24-hour-old blood and it is suggested that this procedure could be adopted in order to supplement supplies.
