Forty-eight patients who had undergone surgical reduction of a fractured neck of femur or in whom deep vein thrombosis was suspected clinically were studied by ascending phlebography and imaging after injection of autologous indium-111-labelled platelets to assess the accuracy and value of the radioisotopic technique in diagnosing deep vein thrombosis. Imaging was performed with a wide-field gammacamera linked with data display facilities. Phlebography showed thrombi in 26 out of 54 limbs examined and a thrombus in the inferior vena cava of one patient; imaging the labelled platelets showed the thrombi in 24 of the 26 limbs and the thrombus in the inferior vena cava. The accumulation of indium-111 at sites corresponding to those at which venous thrombi have been shown phlebographically indicates that this radioisotopic technique is a useful addition to methods already available for the detection of deep vein thrombosis.
No significant excess of deep vein thrombosis (DVT) as measured by the 125I-labelled fibrinogen method was observed in patients having staging laparotomy and splenectomy for Hodgkin's disease (HD) compared with patients having elective cholecystectomy under highly standardized surgical conditions. Patients who did have DVT all had splenic involvement with HD. There was no correlation between the post-splenectomy thrombocytosis and the occurrence of DVT. Patients with non-Hodgkin's lymphoma (NHL) and splenomegaly had a high incidence of DVT after splenectomy.

Agents activating human plasminogen have been prepared from a variety of human sources, namely plasma, blood vessel endothelium and tissue. Their physical and functional properties have been found to differ from that of the urinary activator, urokinase. Antiserum to urokinase, while it quenched the activity of urokinase itself, had no effect on the activity of the plasma, endothelial or tissue activators.
Two patients with familial ATIII deficiency received infusions of a concentrate of ATIII prepared from human plasma. Serial plasma samples were assayed for their functional and immunological content of ATIII. The rates of disappearance of ATIII activity found in this study suggest that, initially, frequent infusions are necessary to maintain normal levels of ATIII activity in patients with a severe deficiency. In one patient platelet survival did not appear to be influenced by ATIII infusions.

Platelets from eight patients thought clinically to have deep venous thrombosis were labbelled with indium-111 and reinjected. Subsequent scanning of the patients with a wholebody scanner and imaging with a gammacamera showed focal accumulation of the label at five sites in four legs, which correlated precisely with the sites of venous thrombi identified by ascending venography. This technique is a useful addition to methods for diagnosis venous thrombosis.

Prostatic crystalloids previously reported only in association with carcinoma were identified in 3.6% of a series of 456 cases of prostatic hyperplasia. Seventy percent of cases with crystalloids contained multiple areas of typical hyperplasia. The incidence of crystalloids in cases of atypical hyperplasia was 9.6%. One patient subsequently developed adenocarcinoma of the prostate. The follow-up period of the majority of these patients is too short to allow evaluation of the prognostic significance of prostatic crystalloids with regard to the development of prostate carcinoma.
The respiratory health of workers exposed to polyvinylchloride (PVC) dust has been investigated in 818 men sampled from the work force of a factory manufacturing PVC. In a cross-sectional survey, the lung function and prevalences of respiratory symptoms and chest radiographic abnormalities were compared with estimates of individual PVC dust exposures based on detailed occupational histories and current measurements of respirable PVC dust. Complaints of slight exertional dyspnoea were associated with PVC dust exposure, though age and smoking effects were much stronger. The forced expired volume in one second (FEV1) and forced vital capacity (FVC) were inversely related to dust exposure after age, height, and smoking effects had been taken into account. This effect was seen principally in cigarette smokers, and there was suggestive evidence that PVC dust exposure and cigarette smoking interacted in the reduction of FEV1 and FVC. Gas transfer factor was not related to dust exposure. The chest radiographs were read according to the ILO U/C classification by three experienced readers. One reader recorded a low prevalence of small rounded opacities, and these were not related to age or dust exposure. Another reader recorded a higher prevalence of small rounded opacities category 0/1 or more, and these were related to age but not to dust exposure. The third reader recorded the highest prevalence of small rounded opacities (though none greater than category 1/1), and these were independently related both to age and to PVC dust exposure, indicating an effect of PVC dust on the appearance of the chest radiography. These appearances were so slight that only the higher sensitivity of this reader in the interpretation of profusion of small rounded opacities on the ILO U/C scale enabled detection of this effect of PVC dust. In conclusion, exposure to PVC dust is associated with some deterioration of lung function, slight abnormalities of the chest radiograph, and complaints of slight dyspnoea. The mean decline in FEV1 associated with the average dust exposure experienced in the study was small, though some of the men with higher dust exposures may have suffered clinically important loss of lung function as a result of their occupation.

A procedure is described for monitoring the biosynthesis of hypothalamic catecholamines in vitro, using the incorporation of the labeled precursor, 3H-tyrosine, into norepinephrine (NE), L-DOPAS and dopamine (DA). The technique involves separation of amines by high performance liquid chromatography, quantiation by electrochemical detection (LCEC), and the collection and measurement of labeled eluant. Rat hypothalami incubated in vitro were found to rapidly synthesize labeled DA while L-DOPA and NE synthesis were less apparent. The monoamine oxidase inhibitor, iproniazid, reduced incorporation. The tyrosine hydroxylase inhibitor, alpha methyl para-tyrosine (alpha mpt), virtually abolished DA synthesis while not appreciably affecting tissue content. The advantages of this technique over others is that it allows for the concurrent quantitation of tissue catecholamine levels, as well as their biosynthetic rate.

We present a case with the unusual association of Graves' disease and functioning pulmonary metastases of follicular thyroid carcinoma. Unlike other reports of thyroid cancer and thyrotoxicosis, this patient's hyperthyroidism was related to a hyperfunctioning primary thyroid neoplasm and not merely to large amounts of functioning metastatic tumor. Thyroidectomy cured her hyperthyroidism. A lung biopsy confirmed the impression of metastatic thyroid cancer. Following therapeutic radioactive I131 pulmonary metastases completely disappeared.
