Missense mutation in the gene encoding the alpha subunit of rod transducin in the Nougaret form of congenital stationary night blindness.

Patients with congenital stationary night blindness enjoy normal daytime vision, which is mediated by cone photoreceptors, but are blind when ambient light is so dim that a normal individual would utilize only rod photoreceptors to see without colour discrimination. The disease is genetically heterogeneous. One form of dominantly inherited congenital night blindness is eponymously named "Nougaret' because pedigree analysis reveals that the disease originated in Jean Nougaret (1637-1719), a butcher who lived in Vendémian in southern France. Here we report that his affected descendants carry a missense mutation in the gene encoding the alpha subunit of rod transducin the G-protein that couples rhodopsin to cGMP-phosphodiesterase in the phototransduction cascade. Based on these results, rod transducin joins rhodopsin and the beta subunit of rod cGMP-phosphodiesterase to become the third component of the rod phototransduction cascade where a defect is implicated as a cause of stationary night blindness. Interestingly, the amino acid residue in transducin affected by the Nougaret mutation is in the position homologous to that affected by the oncogenic mutation originally reported in p21ras, a distant relative in the G-protein superfamily.

[Bilateral ocular blast injury after refractive surgery by 2 current techniques. Comparative anatomo-clinical analysis of a case].

Radial keratotomy reduces the strength of the cornea front of a trauma by blast. The authors present and analyze the case of a 25-year-old patient having been submitted to an ocular bilateral trauma by explosion. Twenty-two months earlier he had undergone radial keratotomy (RK) in the left eye followed 8 days later by photorefractive keratectomy (PKR) in the right eye for a -6,00 d. myopia RLE. Though the trauma by blast essentially reached the right eye, the corneal anatomical damages were more important in the left eye. This is according to us the first case report of trauma on eye subjected to PRK. We appreciate the clinical differences presented by each eye.

[Trans-scleral cyclo-photocoagulation. Results over 6 months].

Transscleral contact cyclophotocoagulation with ND YAG Laser continuous mode permits the induction of a focused ciliary process with a secretory surface decrease. This treatment has been used for the treatment of refractory glaucoma since 1987. We report the treatment by transscleral cyclophotocoagulation of 49 patients with terminal glaucoma. All 49 patients had a measurable visual acuity and an elevated intraocular pressure under maximal medical treatment. None could be operated by trabeculectomy. The origin of the glaucoma was neovascular (26 patients), open angle glaucoma (9 patients) secondary glaucoma (14 patients). 62.2% of the patients had an intraocular pressure < or = 25 mm after 6 months; 8% showed decrease visual acuity. We describe the complications of the treatment and compare the efficacy of treatment depending on the origin of the glaucoma: neovascular or not.

The trochlear nerve: anatomy by microdissection.

This work is based on the microscopic study of 30 trochlear nerve trunks (15 heads). In 17 cases, the trunk arose from two nerve bundles, in 8 cases from one bundle, and for the other 5 nerves, three or four bundles. The mean total length of the trochlear nerve was 86 mm. The nerve may be separated into the 3 following parts: infratentorial, intracavernous, intraorbital. In all 30 cases studied, the first part of the nerve was infratentorial, thus leading us to suggest the term "infratentorial part" for this segment of the nerve. In 27 cases, contact was found with the superior cerebellar artery, in the infratentorial part. In the intracavernous part of ten nerves we found two rami tentorii and in eight cases fibers were exchanged with the ophthalmic nerve. In the orbit, 18 trochlear nerves crossed the posterior ethmoidal artery. 23 trochlear nerves ended on the medial face of the superior oblique muscle. The remaining 7 ended at the superior border of the muscle.

Precision of conjunctival provocation tests in right and left eyes.

BACKGROUND: Conjunctival provocation tests (CPTs) are used for assessing the efficacy of antiallergic treatments, but their reproducibility is not well characterized. A study was carried out to assess the reproducibility of CPTs and the release of mediators during CPTs. METHODS: Both eyes of 30 grass-pollen-allergic patients were challenged with threefold increasing concentrations of a standardized orchard grass pollen extract. The positivity of the CPT was assessed by a cumulative symptom score. The release of mediators was examined by means of histamine (radioimmunoassay), prostaglandin D2 and leukotrienes C4 and D4 (enzyme immunoassay). RESULTS: There was a significant correlation between the concentrations of allergen inducing a positive CPT in both eyes (p < 0.0001, Spearman). All but one patient had a significant release of at least one mediator. After allergen CPT there was a significant release in both eyes in 13 of 20 patients for prostaglandin D2, 11 of 19 for leukotrienes C4 and D4 and 15 of 18 for histamine. The correlations between the levels of mediators released during diluent and allergen challenges in both eyes were significant for prostaglandin D2 (diluent and allergen challenges) and leukotrienes C4 and D4 (allergen challenge). CONCLUSION: Considering the whole group of patients, CPT is reproducible in both eyes, but the results are less satisfactory when patients are examined individually.

[Diagnosis by Doppler color echography of dural carotid-cavernous fistula of ophthalmological manifestation].

The case reported here, concerns a spontaneous low-flow fistula between the external carotid arterial network and the cavernous sinus, with ophthalmological symptoms (exophthalmos, red eye) in an old woman with cardiac failure. The shunt was diagnosed by color-Doppler-imaging, which showed a flow reversal with a systolic component in the superior and inferior enlarged ophthalmic veins. This finding led the authors to extend the arterial filling sequence since the shunt was not detectable on standard arterial views. Embolization was performed during angiography which remains necessary to localize the shunt and to treat the fistula. The clinical symptoms progressively returned to normal and the correction of the hemodynamic disturbances could be followed by color-Doppler imaging, a non-invasive technique which can be easily repeated.

[Arterial occlusion associated with toxoplasmic chorioretinitis].

The authors report a case of unusual toxoplasma chorioretinitis presenting a branch retinal occlusion of an artery passing through the acute necrotic focus. This complications has been very rarely described previously and may have been caused by the acute retinal necrosis. Arterial inflammatory phenomena constitute another etiologic hypothesis. As this branch vascular obstruction may change the appearance of the fundus and obscure the initial inflammatory focus, ocular toxoplasmosis should be included in the differential diagnosis of branch retinal vascular occlusion associated with posterior uveitis, particularly in young patients.

[A case of post-traumatic occlusion of the central retinal artery].

We report a case of central retinal artery occlusion following a minor ocular injury. The occlusions are an uncommon but well following some surgical operations on the eyeball, the eyelids or the nose. They probably result from an arterial spasm which trigger mechanisms are not very well known.

[Mucocele of ophthalmologic expression and treatment apropos of 46 cases].

Mucoceles of the facial sinuses are benign tumors. They consist of a pouch made up from altered sinusal mucosa containing mucus ant inflammatory exudate. This tumors have a tendency to distension and to erode the bony walls. Their common site is in the anterior frontoethmoidal, rarely in the posterior ethmoidal or sphenoidal and exceptionally in the maxillary sinuses. They grow progressively at the expense of the eye and its annexes. Standard radiology is the main diagnostic key, but the scanner is essential to determine the extension and to evaluate the integrity of the posterior wall of the frontal sinus. Surgical intervention is the only way to treat a mucocele. The way of access to the mucocele can be either via the inferior way of Jacques or via the superior way of neurosurgical approach.

[Role of acute normovolemic hemodilution in treating retinal venous occlusions].

The supposed state of retinal venous occlusions by a thrombus has led to the use of diverse antithrombotic treatments (heparin, fibrinolytic agents). This theory being considered at the moment as controversial, haemorheological theories being more particularly favoured, other treatments are proposed in an attempt to better retinal microcirculation, and this by changing the blood viscosity parameters. The study reported here compared the effects on recovery of visual acuity of retinal venous occlusion patients of anticoagulant treatment alone, haemodilution alone, or both together. After initial ophthalmic assessment, twenty-five patients were randomly assigned to one of three groups: group I (n = 5; mean age 62 +/- 14 yr) received heparin for 21 days, followed by antivitamin K drugs for a further 30 days; group II (n = 10; mean age 54 +/- 16 yr) were acutely hemodiluted with 40,000 daltons molecular weight dextran, bringing the haematocrit to between 0.25 and 0.30; on day 2, the same anticoagulant treatment as in group I was associated; group III (n = 10; mean age 58 +/- 18 yr) were only hemodiluted. The hemodiluted state was maintained for 21 days. A biological assessment was carried out for the three groups on days 1, 2, 7, 14, 21 and 30 (haematocrit, fibrinogen level, platelet count, kaolin-cephalin time, heparin level). The evolution of visual acuity was assessed on days 7, 14, 21, 30, 60 and 90. Mean visual acuity was assessed on days 7, 14, 21, 30, 60 and 90. Mean visual acuity was virtually the same for the three groups on day 0.(ABSTRACT TRUNCATED AT 250 WORDS)
