Designing interdisciplinary documentation for the continuum of care.

An increased emphasis on integrated care delivery and the need to access information across the care continuum led to an assessment and modification of the current documentation system at Summa Health System in Akron, Ohio. The goal was to achieve more complete and concise interdisciplinary charting. This article describes the process the two-hospital system developed to achieve integrated documentation reflecting the patient's progress toward team-defined outcomes. Steps in the evaluation and modification of the old system, lessons learned, and results/implications for quality improvement are shared.

Enhancing documentation for the orthopaedic patient.

A need for integrated, interdisciplinary documentation led to an assessment and evaluation of our current documentation system. This article describes the experiences of a two-hospital health care system in piloting and implementing documentation changes on an orthopaedic unit. Outcomes, conclusions, and a copy of the total joint plan of care are also included.

Continuing education evaluation of behavior change.

Using the Cervero model of behavior change and questionnaires developed by Brigham, Ryan, and Elkins (1996), this study assessed the impact of a workshop on behavior change. Data on the individual professional (receptiveness), proposed change (feasibility), social system (climate), and program (quality), as well as self-report data about outcomes (expertise and actions) were included. Information on perceived expertise was obtained before the workshop, immediately after the workshop, and 3 months after the workshop. Information on actions initiated by the participants was obtained 3 months after the workshop. There were 70 participants in this study, with a variety of educational backgrounds and nursing experience. A matched test comparing expertise between time 1 and time 2, as well as time 1 and time 3 was significant at p = 0.00. There were significant correlations among the four variables influencing behavior change and actions and expertise. All except one participant in the study reported taking specific actions after the workshop. The mean number of actions taken was 4.0, with a range from 1 to 9. These findings indicate that a continuing education program can lead to behavior change.

Preparing for an international consultation.

This article outlines the experiences of a staff development educator in preparing for an international staff development consultation. Sections on preparing for travel and for the actual consultation visit provide suggestions in each of these areas. The consultation is described in terms of work done before, during, and after the visit. This information will be helpful to staff development educators considering an international consultation.

Role of continuing educators in encouraging use of clinical practice guidelines.

With the current emphasis on outcomes and quality improvement, continuing educators need to encourage the use of clinical practice guidelines. Specific strategies to assist educators with this task are outlined. Use of clinical practice guidelines can facilitate research linking interventions and outcomes and lead to practice that is both more scientific and cost-effective.

Lessons learned: developing education for a system-wide documentation project.

BACKGROUND: As Summa Health System implemented an integrated interdisciplinary documentation system, educational sessions for staff had to be developed and presented. METHOD: Structure of classes, scheduling of sessions, ongoing revisions and refinements, and key elements of success are shared. RESULTS: Staff members incorporated the documentation changes into practice. CONCLUSION: Other organizations may benefit and learn from the experiences of our two-hospital system in developing and scheduling education for a system-wide change project.

The documentation maze. Finding the right path.

This article outlines the steps taken by a 670-bed hospital to develop a coordinated documentation system. The various tasks are described in terms of preimplementation and implementation activities. Staff development educators can benefit from the reported experiences by capitalizing on the successes and learning from the mistakes.
