Effective nasal influenza vaccine delivery using chitosan.

Nasal influenza vaccination may prove to be a good alternative to parenteral injection because of the enhancement of the mucosal immune response and the ease of vaccine administration. This study investigated the use of chitosan, a bioadhesive polymer, as a nasal delivery system with inactivated, subunit influenza vaccine. Subjects received nasally 15 or 7.5 microg of the standard inactivated trivalent influenza vaccine with chitosan or 15 microg of the same vaccine intramuscularly. Serum haemagglutination inhibition (HI) titres for all three vaccine components were measured prior to, and at time points up to 14 weeks after dosing. Serum HI titres following intranasal vaccination with the nasal chitosan-influenza vaccine met the criteria set by the Committee for Proprietary Medicinal Products in terms of seroprotection rate, seroconversion rate and mean fold increase of HI titre for at least one of the three antigens in the vaccination schedules used. These data show that nasal immunisation with chitosan plus trivalent inactivated influenza is a potentially effective, easily-administered form of vaccination.

Effect of primer precuring on the shear bond strength of orthodontic brackets.

Two adhesive systems, Transbond APC II (3M Unitek, Monrovia, Calif) and Quick Cure (Reliance Orthodontic Products, Itasca, Ill), were used to determine the efficacy of precuring the liquid resin primer phase of the systems to increase shear bond strength. One hundred sixty bovine incisors were divided into groups of 20 specimens. In 1 group, the primer was cured before placement of the bracket with the filled adhesive material, and, in the other group, the primer was not precured. The shear-peel bond strength was tested with a testing machine at 30 minutes and 24 hours. No statistically significant difference in bond strength was found between the groups that had or had not been precured. More adhesive remained on the teeth with the Transbond when the primer was not precured. There is no advantage or disadvantage in bracket bond strength by precuring the primer before placing the bracket and filled component of the adhesive.

Pilot study of nasal morphine-chitosan for the relief of breakthrough pain in patients with cancer.

Breakthrough pain in patients with cancer is common, often unpredictable, and can rapidly become severe. Treatment using the oral administration of opioids is not optimal due to the slow onset of pain relief. Nasal administration of analgesics potentially offers more rapid pain relief. This study investigates the tolerability and efficacy of a novel morphine-chitosan formulation. Twenty episodes of breakthrough pain were observed in 14 patients with cancer who received 5-80 mg of nasal morphine-chitosan. Nasal symptoms, sedation, giddiness, nausea, and other volunteered symptoms, along with pain scores (pain intensity and pain relief), were recorded at baseline and at regular intervals up to 4 hours after administration, together with an overall satisfaction rating. The formulation was acceptable to patients, generally well tolerated, and had an onset of pain relief 5 minutes after dosing. This formulation warrants further study.
