Lipid deposition on silicone hydrogel lenses, part I: quantification of oleic Acid, oleic Acid methyl ester, and cholesterol.

PURPOSE: To investigate the sorption of oleic acid, oleic acid methyl ester, and cholesterol on currently marketed silicone hydrogel contact lenses. METHODS: Two liquid chromatography methods were developed and used to analyze lens extracts from continuous-wear and daily-wear modalities from asymptomatic silicone hydrogel contact lens wearers. RESULTS: Of the three probed compounds, cholesterol is the most prevalently sorbed, at levels ranging from below the limit of quantitation (<1.50 microg per lens) to approximately 37.0 microg per lens. Oleic acid and oleic acid methyl ester were found to exist at levels below the limit of quantitation (<1.50 microg per lens). In general, there appears to be no significant difference between the amounts of cholesterol sorbed on the continuous-wear PureVision and daily-wear PureVision lenses evaluated. CONCLUSIONS: The quantities of lipid sorbed to continuous-wear PureVision lenses are significantly different from those previously reported by other authors in a similarly conducted experiment. This difference suggests that any hypothesis of silicone hydrogel lenses based on these previous lipid data should be reconsidered.

Human EDEM2, a novel homolog of family 47 glycosidases, is involved in ER-associated degradation of glycoproteins.

In the endoplasmic reticulum (ER), misfolded proteins are retrotranslocated to the cytosol and degraded by the proteasome in a process known as ER-associated degradation (ERAD). Early in this pathway, a proposed lumenal ER lectin, EDEM, recognizes misfolded glycoproteins in the ER, disengages the nascent molecules from the folding pathway, and facilitates their targeting for disposal. In humans there are a total of three EDEM homologs. The amino acid sequences of these proteins are different from other lectins but are closely related to the Class I mannosidases (family 47 glycosidases). In this study, we characterize one of the EDEM homologs from Homo sapiens, which we have termed EDEM2 (C20orf31). Using recombinantly generated EDEM2, no alpha-1,2 mannosidase activity was observed. In HEK293 cells, recombinant EDEM2 is localized to the ER where it can associate with misfolded alpha1-antitrypsin. Overexpression of EDEM2 accelerates the degradation of misfolded alpha1-antitrypsin, indicating that the protein is involved in ERAD.

Putting state legislative issues in context.

Dramatic changes in the past decade have occurred in the areas of delegated prescriptive authority for physician assistants (PAs), determination of scope of practice, relations with organized medicine, and participation by PAs in regulation of the profession. As PAs continue in their role of extending care to underserved populations, state legislation and regulations may have to be modified to optimize PA utilization in mental and behavioral health, chronic disease management, and occupational health.

Physician assistants: scope of practice, regulation and reimbursement.

This article details the regulatory statutes that define the scope of practice of physician assistants and the realities of their function in clinical practice. It describes the certification and credentialing processes and the level of reimbursement as a function of the practice setting and the type of payor.
