Positional candidate approach for the gene responsible for benign adult familial myoclonic epilepsy.

Benign adult familial myoclonic epilepsy (BAFME) is an autosomal dominant idiopathic epileptic syndrome characterized by adult-onset tremulous finger movement, myoclonus, epileptic seizures, and nonprogressive course, recognized in Japanese families. We recently assigned the gene locus to chromosome 8q23.3-q24.11 by linkage analysis. In this study, we identified the sequence of human cDNA encoding Kv8.1, a neuronal modulatory alpha-subunit of the voltage-gated potassium channel, mapped to human chromosome 8q22.3-8q24.1. Human Kv8.1 cDNA had a 1,500-nucleotide open reading frame encoding a polypeptide of 500 amino acids, in which six hydrophobic transmembrane segments were well conserved, and its overall amino acids homology as compared with those of rat and golden hamster was 95.0% and 95.0%. Tissue-distribution analysis by reverse transcription-polymerase chain reaction (RT-PCR) showed the presence of the Kv 8.1 transcript exclusively in the brain. The analysis of the genomic organization of the human gene revealed consistency of three exons interrupted by two introns each of 1.2 and 3.5 kb. We analyzed the genomic sequence of the patients with BAFME and found no change in the pathogenesis of the disease.

Evaluation of tapered-end toothbrush bristles regarding efficacy of access to occlusal fissures.

The purpose of this in vitro study was to determine how two toothbrushes, with a combination of a tapered-end and round-end bristle design (Group A), compared to a conventional toothbrush with only rounded-end bristles (Group B) for cleaning in simulated occlusal fissures on a brushing machine. The fissure width at the surface was 200 microns, and then tapered down to the fissure base depths of 230 and 400 microns. The toothbrushes were positioned 90 or 180 degrees to the fissure groove surface on the model, and a 200 g force was applied to the brush head. The fissure model received 190 strokes over one minute by the brushing machine. The removal of an artificial plaque substance in the fissure model was measured by a two-dimensional analyzing system. For each combination of the fissure depth versus bristle angle versus brush head configuration variables, five specimens were measured and means were determined. The results were statistically analyzed using a three-way ANOVA followed by Scheffe's test (alpha = 0.05). The results demonstrated that the combination of tapered and round-end bristles (Group A brush) was statistically significantly (p < 0.01) more effective in removing the artificial plaque material on the fissure area than the Group B brush for specific types and brush stroke directions.

Delayed vertical rectus abdominis myocutaneous flap for anterior chest wall reconstruction.

BACKGROUND: Not only is a radiation ulcer nonviable itself, but the surrounding irradiated tissue also shows poor healing. Therefore, healing in an irradiated field cannot be expected if a flap used for reconstruction fails even partially. For repair of radiation ulcers, a flap with a stable blood supply is required. A superiorly based vertical rectus abdominis myocutaneous (VRAM) flap is commonly used for chest wall reconstruction. Because the VRAM flap is nourished only by the superior epigastric vessels, the blood supply to the distal part of the flap often is precarious. CASE REPORT: A case is reported in which a delayed VRAM flap was used successfully to treat a radiation ulcer on the anterior chest wall. RESULTS: Consecutive angiograms showed that the delay procedure augmented the blood supply to the VRAM flap. The flap showed complete take without any postoperative complications. CONCLUSIONS: A delay procedure may make the VRAM flap more reliable for anterior chest wall reconstruction. This flap may be a valuable option for reconstruction of intractable ulcers such as radiation ulcers, and may be applicable for breast reconstruction after radiation therapy.
