Echinococcosis involving the liver, retrovesical and seminal vesicle presented with syncope.

Hydatid disease of the urogenital system, especially of the retroperitoneum and seminal vesicles, is a very rare condition. We report a case of hydatid disease located in the liver, retrovesical region and seminal vesicle that was diagnosed incidentally while investigating the etiology of syncope. Transabdominal and transrectal ultrasonography revealed hypoecoic multicystic masses which had thin septations and walls in the liver, retrovesical region and seminal vesicle. Abdominal computed tomography examination showed multicystic low attenuation masses in the same region. Pelvic magnetic resonance image findings revealed multiple cystic masses in the retrovesical region and the right seminal vesicle. In conclusion, the diagnosis of hydatid disease should be kept in mind with patients who have cystic lesions in seminal vesicle and retrovesical region.

A thoracic-epidural granulocytic sarcoma case that was diagnosed preceding the onset of and that recurred co-incidental to acute promyelocytic leukemia, which developed after surgical treatment.

Granulocytic sarcoma or chloroma is a tumor seen in myelocytic leukemia. Spinal epidural onset is rare and is generally seen before or together with the onset of myelocytic leukemia. An epidural mass located at the 2nd-5th thoracic levels in an 18-year-old male patient was pathologically diagnosed as granulocytic sarcoma. Radiotherapy was performed after surgical intervention. Ten months later, he was re-admitted with abdominal pain. At this time, an epidural mass at the 6th-9th thoracic levels was detected on magnetic resonance imaging, and acute promyelocytic leukemia was diagnosed. After systemic chemotherapy, partial remission was achieved. We aimed to present this rare case with its remarkable follow-up findings.

[Double inversion recovery sequence in temporal lobe epilepsy: preliminary results].

PURPOSE: DIR (double inversion recovery) is a sequence with the hybrid contrast of both FLAIR and STIR sequences, produced by the application of double inversion recovery pulse. It has been suggested that DIR provides high sensitivity to lesions with low T2 contrast. The purpose of this study was to test the hypothesis that DIR sequence is superior to conventional sequences in the identification of mesial temporal sclerosis and other temporal lobe lesions in patients with temporal lobe epilepsy. MATERIALS AND METHODS: Thirty-three subjects with a prediagnosis of temporal lobe epilepsy and ten healthy control subjects with no abnormality on magnetic resonance imaging studies have been studied with DIR, FLAIR and T2W sequences. Coronal images through temporal lobes and hippocampus were acquired. Qualitatively, overall sensitivity to the presence of lesions, hippocampal lesion detectability, temporal horn dilatation and artifacts were evaluated in temporal lobe epilepsy cases and quantitatively, hippocampal and white matter signal-to-noise ratio as well as hippocampus-white matter contrast-to-noise ratio were calculated in the healthy subjects. Subjective scores were graded on a scale of 3 or 4 points. RESULTS: Signal-to-noise ratio scores were higher on T2W sequence, however contrast-to-noise ratio scores were higher on DIR sequence compared to the other two sequences. Imaging findings were compatible with mesial temporal sclerosis in seven patients, tumoral mass in two, and chronic infarct in four. DIR sequence was less sensitive to hippocampal atrophy than the other two sequences because of cerebrospinal fluid artifacts. Overall sensitivity to the presence of lesions and hippocampal lesion detectability scores, although similar among three sequences, were highest with DIR sequence. However, lesion sensitivity scores were highest for lesions compatible with mesial temporal sclerosis and for solid masses and lowest for cystic lesions on DIR sequence. CONCLUSION: Despite the presence of artifacts, DIR is a sequence providing high sensitivity to mesial temporal sclerosis like lesions at hippocampus. DIR might be useful as an additional sequence when the other conventional sequences reveal a suspicious lesion with no accompanying hippocampal atrophy.

[3D-CT investigation of craniofacial and cervical spine anomalies in congenital muscular torticollis].

PURPOSE: To investigate the accompanied craniofacial and cervical spine anomalies in congenital muscular torticollis (CMT) with three-dimensional computerized tomography (3D-CT). MATERIALS AND METHODS: We examined six cases of CMT. Cranial and cervical 3D CT was performed in all cases. Facial midline deviation angle (FDA), upper hemifacial width, lower hemifacial width (LHFW), length of hemimandible and zygomatic arcus, posterior hemicranial width (PHCW), anterior hemicranial width and cranial base midline deviation angle (CBDA) were measured on cranial 3D-CT images. Value of orbital index was calculated. Data achieved both ipsilaterally and contralaterally were compared with paired t-test. Atlanto-axial rotation angle (AARA) and atlantodental interval (ADI) values were measured on cervical 3D-CT images. RESULTS: Ages ranged between 2-26 years with a M/F ratio of 1/5. Facial asymmetry was diagnosed in all cases and LHFW was significantly undersized when compared to contralateral side (p < 0.05). FDA was measured 5.4+/-2.0 degrees on the average. Zygomatic arcus and hemimandible were significantly undersized on the side of torticollis (p < 0.05). Occipital plagiocephaly existed in all cases and PHCW was significantly extensive on torticollis side (p < 0.05). CBDA was measured 4.5+/-1.7 degrees on the average. Rotational movement of atlas over axis was observed in all cases where the AARA was 15.0+/-7.4 degrees. ADI was lesser than 3 mm in all cases. CONCLUSION: Even occipital plagiocephaly, maxillary and orbital deformity were commonly found in cases of CMT, most dominant asymmetry was observed in lower hemifacial region, zygomatic arcus and mandible by 3D-CT imaging. Rotational movement of first cervical vertebra over the second one was determined in all cases.

Calciphylaxis involving both the upper and lower extremities.

Calciphylaxis is an uncommon complication of end stage renal disease (ESRD) and secondary hyperparathyroidism. It characterized by cutaneous necrosis with mural calcifications and thrombosis in the small vessels of dermis. It is important to diagnose and treat, because of mortality rate from calciphylaxis is very high. We present the case of a patient with ESRD and type II diabetes mellitus developing calciphylaxis of the both upper and lower extremities had normal corrected calcium-phosphate product level. After amputation, necrosis was showed rapid progression resulting in death in one month.

Wavelet-packet-based texture analysis for differentiation between benign and malignant liver tumours in ultrasound images.

The purpose of this study was to apply a novel method of multiscale echo texture analysis for distinguishing benign (hemangiomas) from malignant (hepatocellular carcinomas (HCCs) and metastases) focal liver lesions in B-mode ultrasound images. In this method, regions of interest (ROIs) extracted from within the lesions were decomposed into subimages by wavelet packets. Multiscale texture features that quantify homogeneity of the echogenicity were calculated from these subimages and were combined by an artificial neural network (ANN). A subset of the multiscale features was selected that yielded the highest performance in the classification of lesions measured by the area under the receiver operating characteristic curve (Az). In an analysis of 193 ROIs consisting of 50 hemangiomas, 87 hepatocellular carcinomas and 56 metastases, the multiscale features yielded a high A: value of 0.92 in distinguishing benign from malignant lesions, 0.93 in distinguishing hemangiomas from HCCs and 0.94 in distinguishing hemangiomas from metastases. Our new multiscale texture analysis method can effectively differentiate malignant from benign lesions, and thus has the potential to increase the accuracy of diagnosis of focal liver lesions in ultrasound images.

Pseudoaneurysm of the dorsalis pedis artery: color Doppler sonographic and angiographic findings.

Pseudoaneurysm of the dorsalis pedis artery is an uncommon condition that is usually caused by a traumatic injury or an iatrogenic intervention. The patient usually complains of an enlarging painless, pulsatile mass. A tentative diagnosis may be made by palpation of the pulsatile mass and detection of an associated systolic bruit. Color Doppler sonographic and arteriographic examinations can be used to confirm the diagnosis. We report the case of a 17-year-old patient with a posttraumatic pseudoaneurysm of the dorsalis pedis artery. Sonographic examinations revealed pulsatile flow into and out of a cystic structure surrounded by a thick hypoechoic wall and a "to-and-fro" pattern in the neck of the vascular mass; these findings were consistent with the diagnosis of a pseudoaneurysm. Angiography confirmed the diagnosis. The patient was treated with ligation of the artery and resection of the pseudoaneurysm. He recovered well after surgery and remained free of symptoms 3 months postoperatively. We believe that color Doppler sonography should be the procedure of choice for use in diagnosing pseudoaneurysms; arteriography can then be used to evaluate the alternative blood supply before surgery is undertaken.

[Comparison of three-dimensional gradient echo, turbo spin echo and steady-state gradient echo sequences in axial MRI examination of the cervical spine].

PURPOSE: The aim of this study was to assess the detectability of the structures that are affected by cervical spondylosis by different 3D MRI sequences and compare the image quality of those sequences. MATERIALS AND METHODS: Twenty healthy volunteers were examined using a 1.5 T MR unit. T2* weighted 3D-gradient echo sequence with magnetization transfer saturation pulse (3D-FFE-MTC), T2 weighted 3D turbo spin echo (TSE) sequence with and without spectral presaturation inversion recovery pulse (T2-3D-TSE/SPIR+ and T2-3D-TSE/SPIR-) and balanced-FFE (b-FFE) sequences were compared. Spinal cord signal-to-noise ratio (SNR), cerebrospinal fluid (CSF) SNR, and CSF-spinal cord contrast-to-noise ratio (CNR) were calculated quantitatively. The detectability of neural foramina, spinal nerve roots, uncinate process and ligamentum flava were graded on a 5-point scale (0: minimum, 4: maximum) qualitatively. The presence of the artifacts and overall image quality were graded on a 4-point scale (0: minimum, 3: maximum). RESULTS: Balanced-FFE sequence yielded the best results for each of three quantitative evaluation. In terms of qualitative evaluation, for the uncinate process T2-3D-TSE/SPIR- sequence was superior to the other three sequences (p < 0.01). For the ligamentum flava T2-3D-TSE/SPIR- and b-FFE sequences were superior to the other two (p < 0.01). For the neural foramina b-FFE yielded the lowest score (3), however the other three sequences were not significantly different (3.2-3.5) (p > 0.05). Artifacts were most commonly seen using 3D-FFE-MTC. In terms of overall image quality T2-3D-TSE/SPIR- yielded the highest score, followed by b-FFE. CONCLUSION: 3D-FFE-MTC sequence is frequently preferred for cervical spinal MRI studies. However our study yielded best scores for T2-3D-TSE/SPIR, followed by b-FFE in the quantitative and qualitative evaluation of the structures affected by cervical spondylosis.

[Venous reflux: Measurement variability due to positional differences].

PURPOSE: The purpose of this study was to evaluate the role of the subject's position and two different maneuvers in the reflux measurements in femoral veins. MATERIALS AND METHODS: Venous reflux times in the common femoral vein (CFV) and superficial femoral vein (SFV) were studied in 50 healthy subjects (25 women, 25 male) aged between 22-57 years-old while the subjects were in supine and standing positions. Reflux was provocated only by Valsalva maneuver (VM) when the subject was lying down, whereas it was provocated by (1) Valsalva maneuver and (2) compression and release maneuver, respectively, when the subject was standing. Reflux times were recorded with each of the three methods. RESULTS: Regarding CFV, a reflux time of 1 second or more was recorded in 29 limbs (29%) only in the supine position. Of these 29 measurements, 19 were between 1 and 1.9 sec and 8 between 2 and 2.9 sec. When the subjects were examined in the standing position, the prevalence of reflux lasting more than 1 sec substantially decreased (5%). The prevalence of reflux lasting less than 0.5 sec was 97% with standing compression maneuver, 84% with standing VM and 27% with supine VM. Similarly, prevalence of reflux of longer than 0.5 sec or 1 sec was lower in the standing position than in the supine position in SFV. In CFV, mean reflux time was 1.3 sec in the supine position, whereas it was 0.7 sec and 0.4 sec with standing compression and standing Valsalva maneuvers, respectively. Corresponding values in SFV were 0.5 sec, 0.3 sec and 0.2 sec, respectively. CONCLUSION: The probability of making a false positive diagnosis of pathological reflux is high with supine examinations. When an examination in the supine position reveals a reflux time of longer than 1 second, making the patient stand up is expected to yield more reliable results. Compression and release maneuver, with its lowest false positive reflux prevalence, is the most reliable method in the evaluation of venous insufficiency.

[Pelvic digit: a case report with reference to the differential diagnosis of pelvis abnormalities].

Pelvic digit was incidentally detected on plain pelvis radiography of a 44-year-old male patient who presented with right-sided hip pain. Three-dimensional reconstruction of computed tomography scans provided a more clear illustration of the anatomy and localization of this anomaly. To avoid unnecessary investigation methods and treatment, this entity should be kept in mind when an atypical bone structure is noted around the pelvis

Enlargement of the bone component in different parts of compensatorily hypertrophied inferior turbinate.

BACKGROUND: To evaluate the unilateral compensatorily hypertrophied inferior turbinate (CHIT) by computed tomography (CT) and determine the enlargement of the bone component in different parts of the CHIT. METHODS: Patients were studied in three groups: those with a straight or nearly straight septum (n = 143), with mild deviation (n = 42), and with moderate to severe deviation (n = 99). The cross sectional area (CSA) of the inferior turbinate (IT) bone and the whole turbinate were measured at anterior, middle, and posterior thirds of the IT in coronal sections. The ratio of CSA of the IT bone on two sides of the septum (interturbinate ratio) and the ratio of the CSA of the overall turbinate to the IT bone (intraturbinate ratio) were calculated. RESULTS: The interturbinate ratio of the bony turbinate CSA for the severe deviation group was significantly higher compared with other groups in anterior and middle segments (p < 0.0001). The intraturbinate ratio was highest in the posterior segment and lowest in the middle segments in compensatorily hypertrophied sides for all groups (p < 0.001). CONCLUSION: Skeletal enlargement is prominent in anterior and middle thirds of CHIT in patients with pronounced septal deviation.
