Priapism following ingestion of papaverine tablets.

A case of priapism following oral intake of papaverine hydrochloride (300 mg) is reported. The priapism persisted after coitus. It resisted conservative treatment, but responded to aspiration of the corpora cavernosa and irrigation with phenylephrine solution. We found no earlier report of priapism induced by oral papaverine. The possible therapeutic role of oral papaverine in impotence merits study.

Fracture of polyurethane double pigtail stents: an in vivo retrospective and prospective fluoroscopic study.

OBJECTIVE: To study the behaviour of pigtail polyurethane ureteric stents during breathing and change in body habitus and thereby explain some side-effects of stenting, particularly spontaneous stent fracture. PATIENTS AND METHODS: The study comprised retrospective evaluation of records and radiographs of seven patients (five men and two women, mean age 41; SD 10.9 years) who were admitted with fractured polyurethane pigtail stents, and a prospective fluoroscopic study of the behaviour of 14 stents in 12 patients (11 men and one woman; mean age 40; SD 9.3 years) during breathing, spinal flexion, extension and rotation. RESULTS: Five of the seven pigtail stents fractured where the stent crossed the lateral edge of psoas muscle, 7-10 cm below the renal coil of the stent. The sixth stent was fractured at its lower third and the seventh fractured just proximal to the stent's bladder coil. The prospective study revealed a 20-80 degrees bend in the upper third of 11 of the 14 stents and a 35 degrees bend in the lower third of two stents. A 1-5 cm craniocaudal movement and a 0-2 cm vertical movement of the stents occurred during breathing and spinal flexion and extension. There were no changes in stent configuration during spinal lateral rotation or at the sacral promontary. The bend in the upper third of the ureter was profound in two stents whose vertical movement was restricted by other conditions. CONCLUSION: As the kidney-to-bladder distance shortens during inspiration and spinal flexion, the stent adapts by bending and/or moving. Fracture of the stent may be caused by repeated mechanical stress, particularly across the side-holes, and by diminution of its tensile strength by depolymerization during long indwelling times. Large polyurethane stents tended to bow, displacing the upper ureter anteriorly, while small, soft stents conformed to the anatomical course of the ureter, suggesting that they should be used after open or endoscopic surgery to the upper two-thirds of the ureter. Stress on the stent can be minimized by choosing the correct length, particularly where vertical stent movement is limited by other conditions.

Effect of freezing and mincing technique on microbial load of minced meat.

This experiment was carried out to explain the variation between the use of either fresh or frozen meat cuts in manufacturing of minced meat; in this connection the role of the mincer and the hygienic condition of handling as well as of the temperature under which the raw material and product was handled should be investigated. The achieved results indicated that the minced meat manufactured from fresh meat has low microbial load as compared with that produced from frozen one. However, the mincing at butcher's shops may expose the meat to more sources of contamination than that minced at home (self-made mincing). The handling of raw meat either fresh or frozen in butcher's shops exposed the produced minced meat to high numbers of either spoilage or food poisoning microorganisms. The sources of contamination of produced minced meat were discussed.

Psychrotrophic bacteria in sausage.

A total of 50 samples of frozen fresh beef sausage were collected from grocery stores at Beni Suef City. These samples were examined for enumeration, isolation and identification of psychrotrophic bacteria. All samples contained psychrotrophic bacteria in variable numbers. The mean counts of psychrotrophic bacteria, psychrotrophic Enterobacteriaceae, Pseudomonas, Aeromonas, Brochothrix thermosphacta and Lactic acid bacteria were 2 x 10(5) +/- 10(3), 6 x 10(3) +/- 4 x 10(2), 10(3) +/- 2 x 10(2), 4 x 10(2) +/- 8 x 10, 8 x 10(2) +/- 10(2) and 8 x 10(2) +/- 2 x 10(2) organisms per gram, respectively. Psychrotrophic Enterobacteriaceae isolates were identified biochemically. The hygienic significance of the isolates, their public health importance as well as supposed measures to improve the product are discussed.

Impact of donor age on living related donor kidney transplantation.

Two comparable groups of kidney transplant recipients were identified according to the age of their kidney donors. The first group (A) comprised 42 recipients of donors aged < 40 years, and the second group (B) comprised 48 recipients of donors aged > 50 years. The patients were followed for a mean period of 26 months (range 13-50 months). Post-transplant renal function and graft survival were assessed together with the frequency of post-transplant proteinuria and hypertension. Moreover, the functional reserve of the grafts was determined by comparing the clearance values, obtained by both isotope and chemical means, before and after a combined infusion of dopamine and an amino acids preparation. The graft function was significantly better in group A according to the serum creatinine levels (micromol/l) at 1 month (107+/-4.5 vs. 134+/-10.7, P < 0.01), 12 months (119+/-5.3 vs. 181+/-88, P < 0.05) and at last follow-up visit (118+/-6.2 vs. 223+/-63, P < 0.03) for groups A and B, respectively. The graft survival in group A was significantly higher than that in group B (100% vs. 87% at 1 year, P < 0.05). The graft functional reserve was significantly better in group A than in group B. Post-transplant proteinuria was significantly more frequent in group B recipients (70% vs. 40%, P < 0.03). The age of the donors had no impact on the incidence of post-transplant hypertension. These observations suggest that the transplantation of a kidney from an older live kidney donor is associated with an inferior post-transplant outcome.
