Intrabiliary rupture of a hydatid cyst of the liver with concomitant intraperitoneal rupture into the lesser sac.

A case is reported of a young man of Moroccan origin, admitted with fever, aerobilia and an epigastric mass. Laparotomy revealed a suppurated hydatid cyst extending from the left lobe of the liver, which had ruptured into the left hepatic duct and intraperitoneally, into the lesser sac. A left hepatic lobectomy was performed. Gas producing micro-organisms were cultured from the cyst contents whereas no viable scolices were demonstrated. Medical therapy with mebendazole was instituted postoperatively. Six months later, the patient underwent subtotal pancreaticoduodenectomy because of an obstructive, lymphoblastic non-Hodgkin lymphoma in the ampullary region. One year after the initial operation, there were no signs of recurrence of the hydatid disease.

Chloroquine-resistant falciparum malaria imported into the Netherlands.

A prospective study of imported chloroquine-resistant falciparum malaria in the Netherlands is described. From 1979 to 1983, 77 non-immune patients were investigated; in 41 (53%) decreased sensitivity of Plasmodium falciparum to chloroquine could be confirmed. Signs and symptoms in these patients differ from the classical picture. Resistance to sulfadoxine-pyrimethamine (Fansidar) was established in 6 patients. Parasitaemia was found twice during dapsone-pyrimethamine (Maloprim) prophylaxis. The implications for advice on treatment and prophylaxis are discussed.

Leptospirosis acquired in Surinam.

A case of leptospirosis seen in the Netherlands but acquired in Surinam is described. Marked jaundice, disturbance of renal function, a first degree atrioventricular block and pancreatic irritation were observed.

A new leptospiral serovar in The pyrogenes serogroup.

A new leptospiral serovar in the Pyrogenes serogroup is described. Parallel investigations by the 'classical' cross-agglutinin absorption test method and the 'factor analysis' method determined the serological properties of the new serovar. The serovar name kwale with reference strain Julu is proposed.

Two new leptospira serovars belonging to the Hebdomadis serogroup.

Two new serovars in the leptospira Hebdomadis serogroup are described following parallel investigations by the "classical" comparative cross-agglutinin absorption test method and by a method derived from factor analysis. The serovar names nyanza with reference strain Kibos and dikkeni with reference strain Mannuthi are proposed.
