A method for assessing the height of the patella.

In 150 normal subjects the distance (P) between the upper and lower limits of the articular surface of the patella, and the shortest distance (T) between this same lower limit and the tibial plateau, were measured in standard lateral views of the knee in about 30 degrees of flexion. The limits of confidence of the T/P ratio are shown in the Table. The 5% upper limit of normality of this ratio is 1.11 (one-sided). This method of determining the position of the patella is accurate and more simple than those previously described.

Monarticular involvement of the manubriosternal joint.

Four females and two males with seronegative monoarthritis of the manubriosternal joint are reported. Two patients had pustulosis palmoplantaris, one psoriasis vulgaris, and one HLA-B27. The arthritis is considered to be a monoarticular manifestation of seronegative spondylarthritis. Two patients had no other diseases or signs. Four patients were followed for a mean of 8.5 years (range 2-16 years) during which time affection of other joints or the spine was not seen.

Sclerotic changes of the manubrium sterni.

Six females with nearly identical sclerotic and hyperostotic changes of the manubrium sterni are reported. Malignancies, bacterial inflammatory processes, and Paget disease, which were first suspected, could be excluded. The youngest patients also had sclerotic changes of other bones, including the lumbar spine, the pubic bone, and the clavicle, and may be classified as having "chronic recurrent multifocal osteomyelitis" (CRMO). The two oldest patients had ossification of the costoclavicular ligament(s) and may be classified as having "inter-sterno-costo-clavicular ossification" (ISCCO). One had only hyperostotic and sclerotic changes as seen in "sterno-costo-clavicular hyperostosis" (SCCH). The pathogenesis of these uncommon diseases is unknown, but they are all frequently associated with pustulosis palmo-plantaris and have similar clinical courses and laboratory abnormalities. None of the present patients had HLA-B27. The similarity of the radiological abnormalities of the manubrium sterni suggests that the diseases themselves may be similar, but with different courses depending on age, CRMO being present in children and young adults and ISCCO or SCCH in older adults.

An unusual lesion in the knee of a breaststroke swimmer.

In a 21-year-old male breaststroke swimmer with characteristic symptoms in the medial aspect of the knee joint, synovial leakage suggesting medial capsular rupture caused by the whip kick was demonstrated arthrographically.

[A system for improving the quality of the image in knee arthrography].

A simple and inexpensive technique for arthrography is described. A standard fluoroscopic unit with overhead tube is used. A special cassette holder allows the use of cassettes with a screen-film combination normally used for mammography. An exceptionally good combination of factors important for a good image quality is obtained.

Calcifications simulating peroneus longus tendinitis.

In two patients with sprains of the ankle joint calcification adjacent to the posterior tibial margin was evident in the lateral projection of a standard radiographic examination. Calcifying peroneus longus tendinitis was suggested. Further tangential views and computed tomography (CT) scan disclosed, however, that the calcifications in both patients were located in the tibial insertion of the posterior and inferior tibio-fibular ligament. In such cases, a correct diagnosis will avoid unnecessary treatment for a non-existent tendinitis.

Omnipaque and urografin in arthrography of the knee.

The first experience with Omnipaque in arthrography of the knee in patients is reported. Forty arthrographies were performed using either Omnipaque 350 mg I/ml or Urografin 370 mg I/ml at random. The low-osmolar non-ionic Omnipaque showed a tendency for pain of shorter duration and lower incidence of hydrops. A significantly lower incidence of complaints of tension and swelling and assessed reactive synovitis was demonstrated. Both contrast media produced excellent synovial coating on standard series, but Omnipaque showed a slower decrease in attenuation.

Computed tomography of the knee with reference to meniscal tears. A preliminary report.

CT scanning of the knee was performed in one patient with a Baker's cyst and normal menisci and 8 patients with meniscal tears diagnosed at arthrography and confirmed at arthrotomy. The normal menisci were clearly demonstrated. The torn parts of the menisci invariably had ill-defined free edges and contained areas of low CT attenuation. In 2 patients swelling peripheral to the torn meniscus was only demonstrated at CT.

Radiologic aspects of the Galeazzi lesion.

In lesions of the forearm that included a fracture of the distal two thirds of the radial shaft, a concomitant disruption of the distal radio-ulnar joint was found in 20 out of 38 cases. In 15 cases a typical Galeazzi lesion was present. Dislocation of the distal radio-ulnar joint frequently goes unrecognized. The clinical significance of a lesion in the distal radio-ulnar joint is related to its prognostic value.

The course of atlanto-axial involvement and disc narrowing of the cervical spine in rheumatoid arthritis.

In 188 patients with rheumatoid arthritis 585 radiological examinations of the cervical spine were performed. Subluxation of C1-2 was found more often than expected; it was often the only radiological change in that segment, and its degree was not related to the duration of the disease. Lateral radiographs taken in full flexion were important for the diagnosis. Age over 40 years at the onset of RA, increased ESR at least on one occasion and seropositivity were related to more severe degrees of involvement. The presence of subcutaneous nodules, high titres of the Rose-Waaler reaction and the presence of antinuclear antibodies had no definite relation to the degree of involvement.

Prevention of crackle in double contrast examinations of the colon.

Crackle of the barium contrast during double contrast examinations of the colon probably results from a drying effect due to resorption of water. The duration of the examination should therefore be kept as short as possible and the patient should be well hydrated. The tendency to crackle can be decreased by increasing the molality of the barium enema by suspending barium contrast in 0.4% saline.

Discriminative power of Larsen's grading system for assessing the course of rheumatoid arthritis.

In the classification of 18628 joints in 186 patients with rheumatoid arthritis during a follow-up of 3 to 12 years, the discriminative power of Larsen's scoring system was assessed. Obvious radiologic progression did not give rise to a change of grading in 9.5 per cent of the examinations. For investigations with a relatively short observation period, more detailed scoring systems should therefore be used.

Radiologic evaluation of synovectomy in rheumatoid arthritis.

The course of radiologic rheumatoid arthritis changes in 14 proximal interphalangeal joints of the fingers, 11 metacarpophalangeal joints and 16 knees that were subjected to synovectomy was compared with non-synovectomized joints that were suitable for matching, according to clinical, laboratory and radiologic criteria. The progression of radiologic involvement was probably not changed by synovectomy.
