[Sarcoidosis of the large intestine].

A 68-years old female patient suffering over a period of several weeks from fatigue, weight loss, recurrent diarrheas and mild fever was admitted in our clinic. A few months ago, sarcoidosis of a cervical lymph node had been diagnosed and had been treated with steroids. During the present admission, the extensive study of the gastrointestinal system revealed a tumor of the ascending colon, obstructing the lumen. The patient was subjected to laparotomy due to persistent symptomatology of intestinal obstruction. A right hemicolectomy was performed. The pathological findings were persistent with sarcoidosis of the large bowel. Post-operatively, the patient has been symptom-free over a period of more than one year.

The role of interferons in the treatment of colorectal cancer.

There is a substantial body of evidence suggesting in vivo synergy between fluorouracil (FU) and interferons (IFNs). The mechanism(s) which is implicated in the expression of synergy with the combination of FU and IFNs is unknown. Although phase I studies have demonstrated a pharmacokinetic advantage for this combination, subsequent phase II studies in patients with advanced colorectal cancer showed that only one-third of the patients responded partially to this treatment. Double modulation of FU with leucovorin and IFN does not appear to add any survival benefit to these patients compared to single modulation of FU by IFN. In conclusion, the role of IFNs in the treatment of colorectal cancer has not been defined yet. It is hoped that phase III trials which currently are in progress will clarify this issue.

Desmoid tumor in Gardner's syndrome.

Desmoid tumors are often associated with familial polyposis of the colon with or without Gardner's syndrome. Recurrence rate and the invasive, aggressive character of these tumors is emphasized. The authors present three cases and discuss surgical and conservative treatment.

Rigid or flexible choledochoscopy?

This is a comparative study of two types of choledochoscopes: The Wolf rigid scope and the flexible CHF B3 Olympus scope. These were used for the exploration of 120 consecutive cases of common bile duct disease covering a 4-year period. The rigid and the flexible choledochoscopes were used in 30 and 88 cases, respectively, while in another 2 cases both scopes were used to complete the examination. We established that the Wolf scope does not provide ease of handling in inexperienced hands. Its low price and durability under rough handling should not be an excuse for recommending it for beginners. On the other hand, the Olympus scope is an expensive and delicate instrument, which is much easier to handle. Both choledochoscopes can, however, equally provide good results in experienced hands.
