Antithrombins Wibble and Wobble (T85M/K): archetypal conformational diseases with in vivo latent-transition, thrombosis, and heparin activation.

The inherent variability of conformational diseases is demonstrated by two families with different mutations of the same conserved aminoacid in antithrombin. Threonine 85 underlies the opening of the main beta-sheet of the molecule and its replacement, by the polar lysine, in antithrombin Wobble, resulted in a plasma deficiency of antithrombin with an uncharacteristically severe onset of thrombosis at 10 years of age, whereas the replacement of the same residue by a nonpolar methionine, antithrombin Wibble, gave near-normal levels of plasma antithrombin and more typical adult thromboembolic disease. Isolated antithrombin Wibble had a decreased thermal stability (Tm 56.2, normal 57.6 degreesC) but was fully stabilized by the heparin pentasaccharide (Tm 71.8, normal 71.0 degreesC), indicating that the prime abnormality is a laxity in the transition of the main sheet of the molecule from the 5- to 6-stranded form, as was confirmed by the ready conversion of antithrombin Wibble to the 6-stranded latent form on incubation. That this transition can occur in vivo was shown by the finding of nearly 10% of the proband's plasma antithrombin in the latent form and also, surprisingly, of small but definitive amounts of latent antithrombin in normal plasma. The latent transition will be predictably accelerated not only by gross mutations, as with antithrombin Wobble, to give severe episodic thrombosis, but also by milder mutations, as with antithrombin Wibble, to trigger thrombosis in the presence of other predisposing factors, including the conformational stress imposed by the raised body temperatures of fevers. Both antithrombin variants had an exceptional (25-fold) increase in heparin affinity and this, together with an increased inhibitory activity against factor Xa, provides evidence of the direct linkage of A-sheet opening to the conformational basis of heparin binding and activation.

A model for the structure of the P domains in the subtilisin-like prohormone convertases.

The proprotein convertases are a family of at least seven calcium-dependent endoproteases that process a wide variety of precursor proteins in the secretory pathway. All members of this family possess an N-terminal proregion, a subtilisin-like catalytic module, and an additional downstream well-conserved region of approximately 150 amino acid residues, the P domain, which is not found in any other subtilase. The pro and catalytic domains cannot be expressed in the absence of the P domains; their thermodynamic instability may be attributable to the presence of large numbers of negatively charged Glu and Asp side chains in the substrate binding region for recognition of multibasic residue cleavage sites. Based on secondary structure predictions, we here propose that the P domains consist of 8-stranded beta-barrels with well-organized inner hydrophobic cores, and therefore are independently folded components of the proprotein convertases. We hypothesize further that the P domains are integrated through strong hydrophobic interactions with the catalytic domains, conferring structural stability and regulating the properties and activity of the convertases. A molecular model of these interdomain interactions is proposed in this report.

Regulatory roles of the P domain of the subtilisin-like prohormone convertases.

A unique feature of the eukaryotic subtilisin-like proprotein convertases (SPCs) is the presence of an additional highly conserved sequence of approximately 150 residues (P domain) located immediately downstream of the catalytic domain. To study the function of this region, which is required for the production of enzymatically active convertases, we have expressed and characterized various P domain-related mutants and chimeras in HEK293 cells and alpha-TC1-6 cells. In a series of C-terminal truncations of PC3 (also known as PC1 or SPC3), PC3-Thr594 was identified as the shortest active form, thereby defining the functional C-terminal boundary of the P domain. Substitutions at Thr594 and nearby sites indicated that residues 592-594 are crucial for activity. Chimeric SPC proteins with interchanged P domains demonstrated dramatic changes in several properties. Compared with truncated wild-type PC3 (PC3-Asp616), both PC3/PC2Pd and PC3/FurPd had elevated activity on several synthetic substrates as well as reduced calcium ion dependence, whereas Fur/PC2Pd was only slightly decreased in activity as compared with truncated furin (Fur-Glu583). Of the three active SPC chimeras tested, all had more alkaline pH optima. When PC3/PC2Pd was expressed in alpha-TC1-6 cells, it accelerated the processing of proglucagon into glicentin and major proglucagon fragment and cleaved major proglucagon fragment to release GLP-1 and tGLP-1, similar to wild-type PC3. Thus, P domain exchanges generated fully active chimeric proteases in several instances but not in all (e.g. PC2/PC3Pd was inactive). The observed property changes indicate a role for the P domain in regulating the stability, calcium dependence, and pH dependence of the convertases.

Internal division of capillaries in rat skeletal muscle in response to chronic vasodilator treatment with alpha1-antagonist prazosin.

Chronic vasodilatation represents a stimulus for capillary growth associated with increased luminal shear stress. We have examined the ultrastructure of more than 2000 capillaries to establish whether the sequence of angiogenesis in response to this stimulus is similar to that described during development and under pathological circumstances. Administration of the alpha1-blocker prazosin to rats for 2 weeks led to a greater capillary length density in extensor hallucis proprius muscles without any change in capillary tortuosity: Jv(c,f)=262+/-54 compared with 350+/-17 mm-2, control compared with prazosin (P<0.002). There were obvious signs of endothelial cell (EC) activation after prazosin treatment, including an increased proportion of capillaries with rough endoplasmic reticulum, large cytoplasmic vacuoles, thickened endothelium and an irregular luminal surface. Capillaries from control muscles had a maximum of three ECs in cross section, whereas four ECs were noted in 0.8+0.5% of capillaries after 1 week (n.s.) and 2.5+/-0.9% after 2 weeks (P<0.01) of treatment. This could be due to elongation and/or migration of ECs, as cell proliferation has not been described at these time points. There was also an increase in the proportion of capillaries having a narrow, slit-like lumen (1.7+/-0. 8% of controls; 7.1+/-1.9% at 1 week; 8.8+/-2.5% at 2 weeks; P<0.02), some of which were smaller in size (less than 2 microm diameter) than in controls (3-5 microm) and/or "seamless", i.e. lacking EC junctions. These may represent newly formed vessels. Focal discontinuity of the basement membrane and abluminal EC processes were rarely seen, and capillary growth by abluminal sprouting appeared to be very infrequent (less than 0.001% of profiles). Of more importance was growth starting from the luminal side. Significantly more thin cytoplasmic processes were observed protruding into the lumen of capillaries after 1 week (47.5+/-6.2%, P<0.001) and 2 weeks of prazosin (34.2+/-5.5%, P<0.05) than in control vessels (16.7+/-3.9%). Some of these traversed the entire lumen and connected with endothelium of the opposite side, probably involving membrane fusion, resulting in the appearance of a double lumen. Individual capillaries with a complete double lumen were observed after 2 weeks' prazosin but comparatively rarely, in only four out of six muscles. These findings indicate a pattern of luminal growth which is completely different from intussusceptive growth previously described during development, and from the abluminal capillary sprouting seen under pathological circumstances.

Impact of RNase L overexpression on viral and cellular growth and death.

The biologic actions of interferons (IFNs) are complex and involve multiple biochemical mechanisms, including the 2-5A system, a regulated RNA decay pathway. The 2-5A system is implicated in the antipicornavirus activity of IFN and in the control of apoptosis. To further investigate involvement of the 2-5A system in the control of viral and cellular growth and death, human RNase L cDNA was stably expressed in murine 3T3 cells from a constitutive cytomegalovirus (CMV) promoter. A clonal cell line, 3T3/pLZ, was isolated that overexpressed RNase L by >100-fold compared with levels of the endogenous murine RNase L. Interestingly, human RNase L levels in 3T3/pLZ cells decreased 3-fold as cells entered a confluent, growth arrest state, suggesting autoregulation. Overexpression of human RNase L greatly enhanced both the cell growth inhibitory activity of IFN and the proapoptotic activity of staurosporine. Furthermore, high levels of RNase L suppressed the replication of diverse viruses: encephalomyocarditis virus, vesicular stomatitis virus, human parainfluenza virus-3, and vaccinia virus. Additional reductions in viral growth were obtained by treating 3T3/pLZ cells with IFN (a + beta) before infections. These results directly demonstrate the anticellular and antiviral potential of the 2-5A system.

Electrophysiological and biochemical responses of mouse vomeronasal receptor cells to urine-derived compounds: possible mechanism of action.

Receptor cells of the vomeronasal organ (VNO) are thought to detect pheromone-like molecules important for reproductive physiology. Several compounds derived from male mouse urine have been demonstrated to affect endocrine events in female mice. In the present study, the ability of these compounds to affect VNO activity was tested. In dissociated VNO cells held under voltage clamp conditions, application of dehydro-exo-brevicomin (DHB) evoked an outward current at negative holding potentials and an inward current at positive holding potentials. Under current clamp, DHB reduced action potential firing. Since DHB application caused a decrease in membrane conductance, this compound appeared to act by reducing inward current through closing an ion channel. Biochemical experiments tested the effects of DHB and 2-(sec-butyl)-4,5-dihydrothiazole (SBT) on cAMP levels in the VNO. A mixture of DHB and SBT decreased cAMP levels in VNO sensory tissue and had no effect on VNO non-sensory tissue. The results suggest that pheromones have an inhibitory influence on action potential generation and on cAMP levels in receptor cells of the VNO.

Staged sequential reconstruction of a total lower lip, chin, and anterior mandibular defect.

The combination of a total lower lip, chin, and anterior mandibular defect following cancer resection is an extremely complex problem that requires a sequence of operations to optimize functional and aesthetic results. One patient is presented in whom the defect was reconstructed with a free fibular flap followed by a series of ancillary procedures using both modern and traditional techniques. At the time of tumor ablation, the through-and-through oromandibular defect was reconstructed with a fibular osteocutaneous flap. The lower lip and gingivolabial sulcus was reconstructed later with a tongue flap. Tissue expansion was subsequently used to replace the fibular skin with expanded submental hair-bearing skin. A polyethylene implant was added later to the fibular bone for chin augmentation. Subsequently the lower lip was supported with a tendinous graft suspended to the anterior masseter bilaterally. Lastly, the vermilion border was elevated by removing a rim of the tongue flap and covering the secondary wound with a full-thickness skin graft. At the end of the reconstructive procedures, lip seal and oral aperture were good with no drooling and excellent speech.

The vaccinia virus A18R DNA helicase is a postreplicative negative transcription elongation factor.

Loss of vaccinia virus A18R gene function results in an aberrant transcription profile termed promiscuous transcription, defined as transcription within regions of the genome which are normally transcriptionally silent late during infection. Promiscuous transcription results in an increase in the intracellular concentration of double-stranded RNA, which in turn results in activation of the cellular 2-5A pathway and subsequent RNase L-catalyzed degradation of viral and cellular RNAs. One of three hypotheses could account for promiscuous transcription: (i) reactivation of early promoters late during infection, (ii) random transcription initiation, (iii) readthrough transcription from upstream promoters. Transcriptional analysis of several viral genes, presented here, argues strongly against the first two hypotheses. We have tested the readthrough hypothesis by conducting a detailed transcriptional analysis of a region of the vaccinia virus genome which contains three early genes (M1L, M2L, and K1L) positioned directly downstream of the intermediate gene, K2L. The results show that mutation of the A18R gene results in increased readthrough transcription of the M1L gene originating from the K2L intermediate promoter. A18R mutant infection of RNase L knockout mouse fibroblast (KO3) cells does not result in 2-5A pathway activation, yet the virus mutant is defective in late viral gene expression and remains temperature sensitive. These results demonstrate that the A18R gene product is a negative transcription elongation factor for postreplicative viral genes.

Evaluation of a safety educational programme for the prevention of pesticide poisoning.

The causal factors of pesticide poisoning in spraymen usually include sloppy handling, leakage of sprayers and a lack of personal protection, as well as of a knowledge about pesticide toxicity. This paper outlines the results of a safety educational programme to prevent pesticide poisoning among Chinese village spraymen in 1991 and 1992. The programme consisted of two parts: an interview of 3,286 trained subjects who were surveyed before and after the education courses, and a survey on the prevalence of pesticide poisoning among spraymen in 10 villages in 1991 and 1992, respectively. The results showed a general improvement in the knowledge about pesticide toxicity and safe use among the surveyed subjects. The prevalence of pesticide poisoning among the surveyed spraymen decreased from 1.05% in 1991 to 0.25% in 1992. Moreover, a 68.2% reduction of pesticide poisoning cases was observed in 10 villages in 1992. The success of the safety educational programme proved that education is an effective measure for preventing pesticide poisoning.

Acute effects of carbofuran in workers of two pesticide plants.

This paper investigates the acute effects of carbofuran in workers of two pesticide-formulating plants. Mean airborne carbofuran concentrations ranged from 0.025 to 1.115 mg/m3 in plant A and from 0.018 to 0.067 mg/m3 in plant B, respectively. In workers of plant A the post-shift blood cholinesterase activity was significantly reduced, compared to pre-shift values. No difference in blood cholinesterase activity was found between pre- and post-shift values in workers of plant B. During the investigation, 25 cases of acute carbofuran poisoning were diagnosed by their clinical picture and depressed cholinesterase activity in blood. Usual symptoms included dizziness, weakness, blurred vision, nausea and sweating. Pallor, epigastric pain, vomiting and chest tightness occurred only in a few cases. Myosis was recorded in 24 cases. Fasciculation of muscle gastrocnemius induced by percussion was found in 6 cases, and four of them had also fasciculation of muscle orbicularis oculi. Inhibition of cholinesterase activity in the blood was related with the clinical features; however, the inhibition was rapidly reversible. In most cases, recovery was complete within 2-3 hours, with or without atropine treatment, after the subjects were removed from exposure. Rapid onset, mild illness and quick recovery are typical characteristics of occupational acute carbofuran poisoning.

[Clinical evaluation of cardiac troponin I in ischemic heart diseases].

The detection of cardiac troponin I(cTnI), including serum or plasma, was evaluated in 114 patients with ischemic heart diseases or other heart diseases. The results were that the sensitivity of cTnI detection(qualitative analysis, cutoff is 0.2 ng.ml-1) was higher than that of creatine kinase(CK), creatine kinase-MB(CK-MB), alpha-hydroxybutyrate dehydrogenase(alpha-HBD), lactic dehydrogenase(LDH), and aspartic transaminase(AST) for diagnosing acute myocardial infarction (AMI) (93.2% vs 68.2%, 68.2%, 65.9%, 65.9%, and 75.0%; P < 0.05; respectively). The specificity of cTnI detection was higher than that of alpha-HBD, LDH, and AST(95.2% vs 81.0%, 73.8%, and 54.0%; P < 0.05; respectively) and similar to CK and CK-MB(95.2% vs 83.3% and 83.3%; P > 0.05; respectively). On the other hand, the sensitivity of cTnI in patients with unstable angina pectoris was 45.5%. It was higher than stable angina pectoris and lower than AMI. The results suggest that the cardiac troponin I is a better cardiac injury marker than other cardiac markers for diagnosing AMI.

[Detection of interleukin-8 and nitric oxide in chronic prostatitis].

We detected levels of interleukin-8(IL-8) and nitric oxide(NO) in 62 prostatic fluids. The results were that microbiologic exam, IL-8 content, and NO level in control group were all negative; there was not significant difference on the microbiologic exam and NO level between the chronic prostatitis group and non-prostatitis group; there was significant difference between two groups of IL-8 positive ratio. These results suggest that the IL-8 detected in the prostatic fluid of patients with prostatitis may be a diagnostic indicator of prostatic infection.

[Antisense N-ras1 gene inhibits the proliferation of VSMC induced by bFGF after arterial injury].

OBJECTIVE: To develop a new way to prevent the process of restenosis after arterial injury, we transfer the recombinant antisense N-ras1 gene to the cultured SMCs and studied the influence of gene therapy with antisense N-ras1 on the SMCs proliferation induced by bFGF. METHODS: With the recombinant antisense N-ras1 plasmids (fpGv1-MT-N-ras1) transfered to the cultured SMC, we studied the effect on the SMCs proliferation induced by bFGF. RESULTS: The results of the experiment indicated that the count of cells transfered antisense N-ras1 was 16.8 +/- 1.3 x 10(4), the control cells transfered fpGv1-MT retrovirus vector were 30.1 +/- 1.2 x 10(4) (cell/ml) (P < 0.001). cell [3H] thymedine incorporation was 7,643 +/- 672 cpm in gene therapy cells, and 15,131 +/- 138 cpm in vector control cells (P < 0.001). Gene therapy cells ras mRNA and p21 were significantly decreased by Northern blotting and Western blotting technique. CONCLUSION: These results suggest that antisense N-ras1 gene may inhibit the proliferation of cultured VSMC induced by bFGF.

[Vascular endothelial growth gene therapy for narrowing of rabbit iliac artery].

OBJECTIVE: To study the mechanism of renarrowing and vascular endothelial growth factor (VEGF) gene therapy to prevent the restenosis caused by balloon angioplasty. METHODS: According to the special function of VEGF that promepts the proliferation of endothelial cell (EC), we constituted an eukaryotic expression vector of pcDNA3/VEGF, and added the pcDNA3/VEGF to the surface of polylysine-balloon. We examined the results of the test by in situ blot, eletromagnetic blood flow mechine, scanning eletromicroscope, and light microscope. RESULTS: There was higher expression of VEGF mRNA and iliac artery intima in the VEGF gene group. The extent of endothelialization was larger, the ratio of intima to media (I/M) was smaller, and the blood flow was more sufficient than in the vector control group. CONCLUSION: VEGF gene local delivery could stimulate the proliferation of EC, accelerate the endothelialization of injury-artery, and inhibite the proliferation of smooth muscle cell (SMC).

[Clinical study on treatment of aplastic anemia mainly with Yishen Shengxue tablet].

OBJECTIVE: To assess the clinical effects of Yishen Shengxue tablet (YSSXT) on aplastic anemia. METHODS: All patients were divided into two groups by simple randomized methods, 106 patients of the treatment group were treated with YSSXT, 36 patients of the control groups were treated with Renshen Guipi Pill (RSGPP). Peripheral blood and bone marrow of the two groups of patients were measured before and after treatment. The toxicity and side effects of YSSXT were also observed. RESULTS: The basical cure rate, remission rate, total effective rate of the treatment and control groups were 23.6%, 36.8%, 82.1% and 11.1%, 22.2%, 58.3% respectively. There was significant difference between the two groups (P < 0.05 and P < 0.001). The effect of treatment group was obviously better than that of control group. Follow up study showed that from onset of the disease to follow up time in the treatment group, the longest one was 18 years 4 months, so the long-term curative effect is promising. During the period of clinical observation, no obvious side effect and toxicity of YSSXT were found. CONCLUSION: YSSXT is an effective and safe drug in treating aplastic anemia. It has a good prospect and application.

[Comparison studies on pharmacodynamics of three kind of Carapax trionycis].

Three kinds of Carapax Trionycis were used in rat to prevent and treat syndrom-complex of dificiency of Yin. Compareing the improvement of symptoms of dificiency of Yin, it was found that all of them actted effectively in various degrees. The effect of ZhongHua Carapax Trionycis (ZHCT) was similar to those of Shan Rui Carapax Trionycis (SRCT), but the efficacy of Green Carapax Trionycis (GCT) was inferior to those of ZHCT and SRCT. All kinds of C. Trionycis could increase the function of responding to iriation in mice with deficiency of Yin. The intensity of ZHCT was similar to that of GCT, and superior to that of SRCT. They all could enhance phagocytic function of eticuloedothelial system obviously, promot the development of thymus in different degrees with no efect on spleen in young mice. The efficacy of increasing the immune function of ZHCT and GCT were more potent than that of SRCT. ZHCT and GCT significantly decreased the activity of SGPT in mice poisoned by CC14. ZHCT and GCT protected liver better than SRCT did.
