[Meatomyosynangiosis (Ugo Fisch operation) in progressive perceptive deafness. Principles, results and indications].

Meatomyosynangiosis is the operation suggested by Ugo Fisch. It consists of positioning a flap of superficial temporal muscle in contact with the osteoperiosteal and vasculo-neural elements of the internal auditory meatus. In about one third of the cases this operation results in tonal and vocal improvement for patients suffering from progressive perceptive deafness. According to Ugo Fisch, these results are due to retrograde revascularisation of the internal ear starting from the implanted muscular flap. Our own surgical experience confirms these clinical findings. The greatest chance of securing such an improvement is with young patients under fifty and results from this operation in other cases of perceptive deafness are definitely less conclusive and more uncertain. The main indication then for this meatomyosynangiosis is, at present, for progressive perceptive deafness in young patients with no more than 50 Db at lower frequencies. Furthermore, the arteriolar anastomosis pictures we have obtained after this operation was carried out on animals would appear to confirm the physiopathogenetic explanation suggested by Ugo Fisch.

[Cancers of the rhinopharynx in Tunisia. Epidemiological and anatomo-clinical aspects].

Three hundred and ninety eight cancers of the nasopharynx were collected in five years (1969-1973). They occupy the first place in E.N.T. sites of cancer. The majority (390 cases) were carcinomas. Its frequency in youth is conspicuous in this study (63 cases--16%--under the age of 19). The age and year adjusted incidence per 100,000 in relation to the world population is 7.5 for men and 3.3 for women. Clinically, lymph node enlargement was apparent in 2/3 of the cases. The relative frequency of paraneoplastic syndromes in the young is noteworthy. With regard to aetiology, a study of the viral and immunogenetic hypothesis is in train.

[Scleroma of the upper respiratory tract. Apropos of a case of laryngeal localization].

A case of rhinoscleroma with laryngeal localization is reported. Geographic distribution of this granulomatous disease is discussed. Clinical and paraclinical features are explained. Its tumor-like appearance and local spread arouses suspicion of malignancy but differential diagnosis also includes fungal infections and numerous granulomatous diseases. The classic histopathology consists of large Mikulicz's cells. Treatment is essentially based upon antibiotics.

[The rupture of the pleura: a serious complication of hydatid cyst of the lung in children (author's transl)].

The authors report 5 cases of rupture of hydatid cyst into the pleural cavity observed among 336 patients operated for hydatid cyst of the lung during a 13-year period. Three cases were spontaneous ruptures and two were the result of repeated pleural tappings. One case among the spontaneous ruptures was presented with a picture of acute surgical abdomen. There were no death among the five patients.
