The Authors analyzed the premature deliveries performed between 1982 and 1988 at the IV Clinica Ostetrica e Ginecologica of the University of Milan at the S. Gerardo Hospital, Monza, 1267 newborns out of 12507 were preterm. The rate was between 8.7 and 11.0% (average 9.8%). The early neonatal mortality is 10.6% (excluding malformed newborns, 8.3%), the late neonatal mortality is 12.7% (excluding malformed newborns, 9.8%). The early mortality is significantly lower starting from the 32nd week of pregnancy (35.4% vs 3.2%) and with a newborn's weight greater than 1500 grams (36.9 vs 3.2%). The early mortality is lower for females (8.4%) than for males (11.1%). In spite of other Authors, the early mortality is significantly higher than for AGA newborns (19.4 vs 7.9%). The causes of the early mortality are analyzed in detail.
Fifty-three patients, 28 females and 25 males, aged from 21 to 68 years, affected with mono- and bilateral intermittent hydrarthrosis of the knee and of a non-inflammatory nature, were included in an open study. All of the knees involved were treated with intra-articular infiltrations with Rifamycin SV. The weekly dosage of Rifamycin SV was 500 mg; the average duration of treatment was 6 weeks. Follow-up 1 year after the end of treatment showed a decrease or regression in joint effusion in all of the knees treated.
106 term pregnant patients with unfavorable local condition (B.S. less than or equal to 5) and indication to induction of labour because of mother or fetus problems, were randomized for a controlled clinical trial. 52 patients (group A) received 0.5 mg. of PGE2 in 2 ml of tylose gel intracervically. 48 patients (group B) received 3.0 mg. of PGE2 in 5 ml of tylose gel intravaginally. 6 patients were excluded because of violation of protocol. The aim of our study was to evaluate the best method of cervical ripening before a classical induction with amniotomy and oxytocin. Our results show that intracervical PGE2 gel seems to have a better effect on the ripening of the cervix than the intravaginal one if we only consider the proposition of softening success (group A 14/52; group B 6/48). If the evaluation of the effect on the cervical ripening is made according to modification of Bishop's score after gel application, the situation seems inverted because the medians values of the modifications have obtained respectively for group A and B a variation of 2.0 and 3.0 points. Moreover the intravaginal way showed a significantly higher incidence of collateral effects.
