[Early reoperations after resection of stomach in gastroduodenal ulcers].

The material is based on the 20 years' experience in urgent surgical treatment of 2146 patients with gastroduodenal ulcers (GDU). 120 patients of them underwent relaparotomy. The main reasons for the repeated operations were insufficiency of the sutures of the duodenal stump, peritonitis, bleeding, abscesses of the abdominal cavity. Owing to new tactics of surgical treatment for GDU the authors managed to reduce the number of relaparotomies from 9.3 to 3.7% and total mortality rate from 5 to 0.7%.

[Combined surgical procedures in liver and biliary tract diseases].

The indications and contraindications for simultaneous surgical correction of the diseases of hepato-biliary system remain unclear and debatable. 308 patients at the age of 20-78 years have been operated on liver, biliary tract in combination with surgery on other organs. More than 600 operations have been performed. The rate of morbidity was 1.6% lethality-2.3% (7 patients). The risk of death was increased for patients over 60 years of age, who had urgent combined operations. The optimal operative access and drainage were of a special importance. Intramuscular Heparin injections in the dose of 5000 units 4 times a day were used in patients over 60 years of age and in case of 3d or 4th degree of obesity. Liver and biliary tract surgery was combined with resection of the stomach, hernioplasty, appendectomy, echinococcectomy, hemicolectomy, nephrectomy, thyroid gland's resection, amputation of the uterus, sectoral resection of the buast and others. Endotracheal anesthesia was used in all the cases.

[Isolated echinococcosis of the left hepatic lobe].

The authors analyse 47 patients with isolated echinococcosis of the left hepatic lobe. There were 36 males and 11 females. One cyst was found in 39 patients, two cysts in 8 patients. The volume and character of the operative interventions were determined by the peculiarities of the case and its complications. Cholectomy together with the parasite but without opening the cavity was conducted in 8 patients with cirrhosis and deficiency of the left lobe. In rupture of the cysts into the bile ducts echinococcectomy was combined with cleansing and drainage of the ducts: retrograde drainage after Abu proved to be the best method. Wedge resection was conducted in 7 patients and thermal treatment in 8 patients. In the remaining cases various methods of echinococcectomy were applied.

[Therapeutic tactics in the association of acute cholecystitis and obesity].

From experience in the treatment of 326 patients with acute cholecystitis and concomitant obesity the authors discuss the causes of unfavourable outcomes and plan the means for improving the results. Postoperative suppuration of the wound occurred in 31.5% of cases, and was encountered most frequently in patients who underwent emergency and urgent operations. It was established that concomitant obesity in patients with acute cholecystitis is a factor raising the risk of postoperative pyo-inflammatory complications. The immune status was found to be significantly depressed. Preoperative immunoprophylaxis should be applied to prevent pyo-inflammatory complications after the operation.

[Importance of the cytochemical study of blood leukocyte enzyme activity in acute pneumonias].

The authors describe the results of cytochemical study of the activity of alkaline and acid phosphatase, myeloperoxidase and NADP-HH oxidase of neutrophils, acid phosphatase, succinate dehydrogenase and mitochondrial alpha-glycerophosphate dehydrogenases of peripheral blood lymphocytes in 90 patients with acute pneumonia on days 1, 3, 5, 7, 14 and 21 of the disease. It has been shown that changes in the activity of leukocytic enzymes are early and sensitive signs of acute inflammation in the lungs, reflecting the gravity and character of the disease. Some features of the enzymatic alterations depending on the degree of respiratory failure and allergic complications have been disclosed.

[Vagotomy in the treatment of gastroduodenal ulcers in middle-aged and elderly patients].

Experience with the treatment of 190 patients with a complicated ulcer disease of the stomach and duodenum in elderly and senile patients is presented. In 115 of the patients gastric resection was performed. Different kinds of vagotomy were used in 75 patients. Postoperative complications after vagotomy were noted in 9,3% of the cases, after gastric resection in 40%. No lethal outcomes followed vagotomy. Lethality rate after gastric resection was 9,6%.
