Superior cytotoxic potency of mitoxantrone in interaction with DNA: comparison with that of daunorubicin.

The mechanism of action of the anthraquinone antileukemic drug mitoxantrone (MIT) was investigated and compared with that of daunorubicin (DNR) with emphasis on the interaction with DNA to clarify the more potent cytotoxic activity of MIT than DNR in human leukemia cells. MIT showed similar characteristics to those of the anthracyclines such as DNR in that binding of MIT to DNA was associated with reduced template activity and cleavage of DNA in HL-60 human leukemia cells. However, as compared with DNR, MIT showed an increased number of binding sites on DNA, a lower inhibition constant for DNA polymerization, and a greater DNA cleavage activity. These properties may contribute to its greater potency than that of DNR, with which MIT inhibited the proliferation of HL-60 human leukemia cells.

[A post-thymomectomy case of myasthenia gravis which developed nephrotic syndrome with membranous nephropathy during azathioprine administration].

We report a 46-year-old woman who has been suffered from myasthenia gravis and underwent thymomectomy in December 1988. Her myasthenic symptoms improved by treatment with corticosteroid and azathioprine; the latter drug was administrated for more than one year. She noticed weight gain of 10 kg and edema in both legs and feet, which developed acutely in August 1994. Laboratory data showed that she suffered from nephrotic syndrome with a large amount of proteinuria (15 g/day). Renal biopsy revealed that biopsied glomeruli showed early stage of membranous nephropathy associated with acute tubular necrosis. Although therapeutic trials of steroid pulses could not eliminate proteinuria, substitution of cyclophosphamide for azathioprine brought marked improvement of the nephrotic syndrome with disappearance of the urinary protein excretion within 10 days. From reports of similar cases with myasthenia gravis in Japan and in Europe, therapeutic usage of azathioprine in patients with myasthenia gravis associated with thymoma should be cautious for appearance of nephrotic syndrome when azathioprine is continued for more than one year.

[Two cases of primary mediastinal germ cell tumor].

We have experienced two patients with primary mediastinal germ cell tumor. The 1st case was a 26-year-old male with high AFP levels. The tumor was completely removed with en-bloc resection of the left upper lobe and postoperative combination chemotherapy was effective. He is free of disease for ten years after the surgery. The 2nd case was a 25-year-old male with normal AFP and HCG level. After 2 courses of combined chemotherapy, the residual tumor was resected. Viable tumor cells were not presented in the resected tumor. He is free of disease for 3 months after surgery.

The human aldehyde dehydrogenase 3 gene (ALDH3): identification of a new exon and diverse mRNA isoforms, and functional analysis of the promoter.

Expression of the human ALDH3 gene is regulated in a tissue-dependent constitutive as well as drug-inducible manner. We identified a new 5'-noncoding exon (exon 1) existing at about 3 kilobase pairs (kb) upstream from the first coding exon (exon 2) of the human ALDH3 gene. Analysis of ALDH3 mRNA revealed the existence of several isoforms with different 5' regions resulting from i) usage of multiple transcriptional initiation sites of the new exon 1, ii) usage of alternative splice acceptor sites at the 3' end of the new intron 1, and iii) alternative splicing out of exon 2. Usage of alternative splice acceptor sites was only found in tissues expressing ALDH3 constitutively, but not in Hep G2 induced by 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD). Nucleotide sequence analysis and chloramphenicol acetyltransferase (CAT) expression studies showed that a strong promoter region exists at nucleotide (nt) positions -216 to +54 of the gene. Repression activities were found upstream of the -216/+54 region. Several putative drug-inducible elements exist in the regulatory region. A possible regulatory mechanism for tissue-specific constitutive and inducible expression of the human ALDH3 gene is discussed.

Fluorophotometry with a small animal adapter.

The usefulness of the Fluorotron Master fluorophotometer fitted with a small animal adapter was evaluated with tree shrews. We also determined that the optimal measurement parameters for these animals, which are regarded as promising for experimental use in ophthalmic research, are: fluorescein-Na concentration, 0.5 x 10(-9) to 1.0 x 10(-6) g/mL; fluorescein-Na dosage, 2 mg/kg; measurement time, 30 minutes after injection of fluorescein-Na into the ocular compartments. Results indicated that ultrafiltration for the measurement of protein-unbound fluorescein can be done with a hematocrit tube and a minimal blood sample, thereby reducing the impact of the sampling on the animal's general condition. The Fluorotron Master with the small animal adapter can offer advantages in estimating the blood-ocular barrier permeability using disease models in tree shrews.

[The new laser Doppler velocimetry for the measurement of retinal circulation and its clinical application].

We developed a new laser Doppler instrument to measure retinal blood flow. This instrument is equipped with an eye tracking system, which maintains the laser beam on the retinal vessel, thus enabling the measurement of patients with poor visual acuity and poor fixation. In diabetic patients even those without retinopathy, the retinal blood flow decreased compared to the control subjects. This was mainly the result of decreased retinal blood speed and no significant change in the arterial diameter. In diabetic patients requiring panretinal photocoagulation, we studied the regional changes in retinal blood flow following superior or inferior half fundus argon laser treatment, which produced a significant retinal blood flow decrease only in the treated area. We also studied the retinal circulatory changes in retinal branch vessel occlusion. In retinal branch view occlusion, the blood flow in arteries supplying the affected lesion showed 80-90% decrease compared to the arteries supplying the unaffected region. In retinal branch artery occlusion, the retinal blood flow in occluded arteries showed 40-50% decrease compared to the non-occluded arteries. In one patient with retinal branch vein occlusion in whom we measured the retinal blood flow over 18 months, the blood flow increased with the development of collateral vessels. The results showed that retinal circulatory abnormalities can be also evaluated quantitatively during the course of retinal vascular occlusive disease by our newly developed laser Doppler instrument, which proved that this instrument is a clinically very useful tool.

Clinical characteristics of traumatic macular holes.

The clinical characteristics of 20 eyes of 20 consecutive patients with traumatic macular hole were studied retrospectively. The macular holes were elliptical with irregular edges in 19 eyes (95%) and ranged in size from 0.2 to 0.5 DD (disc diameter). Posterior vitreous detachment (PVD) was found in only 3 eyes (15%); the vitreous was detached from the macula in only one of these 3. Other conditions found in the 20 patients included commotio retinae, vitreous hemorrhage, hyphema, and choroidal rupture. These findings strongly suggest that most traumatic macular holes develop in the absence of PVD and that the pathogenesis is independent of the occurrence of PVD. We believe that the macular rupture (hole) is caused mechanically by the force of the impact on the posterior pole and the ocular deformity which result from blunt trauma.

Cloning of a cDNA encoding rat aldehyde dehydrogenase with high activity for retinal oxidation.

Retinoic acid (RA), an important regulator of cell differentiation, is biosynthesized from retinol via retinal by a two-step oxidation process. We previously reported the purification and partial amino acid (aa) sequence of a rat kidney aldehyde dehydrogenase (ALDH) isozyme that catalyzed the oxidation of 9-cis and all-trans retinal to corresponding RA with high efficiency [Labrecque et al. Biochem. J. 305 (1995) 681-684]. A rat kidney cDNA library was screened using a 291-bp PCR product generated from total kidney RNA using a pair of oligodeoxyribonucleotide primers matched with the aa sequence. The full-length rat kidney ALDH cDNA contains a 2315-bp (501 aa) open reading frame (ORF). The aa sequence of rat kidney ALDH is 89, 96 and 87% identical to that of the rat cytosolic ALDH, the mouse cytosolic ALDH and human cytosolic ALDH, respectively. Northern blot and RT-PCR-mediated analysis demonstrated that rat kidney ALDH is strongly expressed in kidney, lung, testis, intestine, stomach and trachea, but weakly in the liver.

A novel calcium-dependent activator of retinal rod outer segment membrane guanylate cyclase.

The membrane guanylate cyclase in retinal rod outer segments (ROS-GC) is known to be negatively regulated by calcium; when the calcium concentration is reduced below the dark-adapted level of about 500 nM, the enzyme is activated by a soluble protein. We now report that the enzyme is also positively regulated by calcium; a novel soluble protein is identified and purified from bovine retina which activates ROS-GC, with half-maximal activation occurring at 2-5 microM calcium. The activation is dose-dependent, and at its maximum, cyclase is stimulated up to 25-fold. The activator has a molecular mass of about 40 kDa and is a multimer of a 6-7 kDa peptide.

Dissection of strain difference in acquired protective immunity against Mycobacterium bovis Calmette-Guérin bacillus (BCG). Macrophages regulate the susceptibility through cytokine network and the induction of nitric oxide synthase.

Protection against infection with intracellular pathogens operates in two stages, early innate resistance and late acquired protective immunity (API), in inbred mouse strains. Although both C57BL/10 (B10) and BALB/c mice bear the susceptible phenotype of innate resistance, Calmette-Guérin bacillus (BCG) vaccination generated efficient API in B10 but not in BALB/c mice. Employing a specific nitric oxide (NO) synthase inhibitor, we revealed that NO production plays a pivotal role in the API of B10 mice. Consistent with this, expressions of the inducible isoform of NO synthase (iNOS) protein and mRNA were significantly higher in the spleen of B10 mice than in that of BALB/c mice. Furthermore, IFN-gamma, a potent inducer of iNOS, and mRNAs for IL-12 (p40); an inducer of IFN-gamma and IL-2 were also vigorously expressed in the spleen of B10 mice compared with that of BALB/c mice. In an attempt to clarify the mechanism by which the different capacities for API are generated, we analyzed the cytokine network between T cells and macrophages in both B10 and BALB/c mice. We found that multiple functions of macrophages, which include capacities to express IL-12 (p40) mRNA in response to BCG and to express mRNAs for iNOS and IL-12 (p40) in response to IFN-gamma, were impaired in BALB/c mice as compared with B10 mice. However, T cells appeared to express comparable level of IFN-gamma mRNA in both strains when stimulated with IL-12. Taken together, these results indicate that the macrophage functions play a pivotal role in both the induction and effector phases of API to determine the susceptibility of mice to BCG infection.

An atypical variant of Fabry's disease in men with left ventricular hypertrophy.

BACKGROUND: Fabry's disease is considered very rare. Left ventricular hypertrophy is one of the common manifestations in adults with classic hemizygous disease. Recently, several cases of an atypical variant of hemizygous Fabry's disease, with manifestations limited to the heart, have been reported. Therefore, we assessed the incidence of hemizygosity for Fabry's disease among male patients with left ventricular hypertrophy. METHODS: We measured plasma alpha-galactosidase activity in 230 consecutive male patients with left ventricular hypertrophy. Clinical manifestations were assessed, endomyocardial biopsies were performed, and the patients were screened for mutations in the alpha-galactosidase gene. RESULTS: Seven of the 230 patients with left ventricular hypertrophy (3 percent) had low plasma alpha-galactosidase activity (0.4 to 1.2 nmol per hour per milliliter; 4 to 14 percent of the mean value in normal controls). These seven unrelated patients, ranging in age from 55 to 72 years, did not have angiokeratoma, acroparesthesias, hypohidrosis, or corneal opacities, which are typical manifestations of Fabry's disease. Endomyocardial biopsy was performed in five patients and revealed marked sarcoplasmic vacuolization in all five. Samples from four patients were examined by electron microscopy and revealed typical lysosomal inclusions with a concentric lamellar configuration in all four. Two patients had novel missense mutations in exon 1 (Ala20Pro) and exon 6 (Met296lle). The remaining five had no mutations in the coding region of the alpha-galactosidase gene, but the amounts of the alpha-galactosidase messenger RNA were markedly lower than normal. CONCLUSIONS: Seven unrelated patients with atypical variants of hemizygous Fabry's disease were found among 230 men with left ventricular hypertrophy (3 percent). Fabry's disease should be considered as a cause of unexplained left ventricular hypertrophy.

The transcriptional regulation of human aldehyde dehydrogenase I gene. The structural and functional analysis of the promoter.

Human cytosolic aldehyde dehydrogenase 1 (ALDH1) plays a role in the biosynthesis of retinoic acid that is a modulator for gene expression and cell differentiation. Northern blot analysis showed that liver tissue, pancreas tissue, hepatoma cells, and genital skin fibroblast cells expressed high levels of ALDH1. Sequence analysis showed that the 5'-flanking region contains a number of putative regulatory elements, such as NF-IL6, HNF-5, GATA binding sites, and putative response elements for interleukin-6, phenobarbital and androgen, in addition to a noncanonical TATA box (ATAAA) and a CCAAT box. Functional characterization of the 5'-regulatory region of the human ALDH1 gene was carried out by a fusion to the chloramphenicol acetyltransferase gene. A construct containing 2.6 kilobase pairs of the 5'-flanking region was efficiently expressed in hepatoma Hep3B cells, but not in erythroleukemic K562 cells or in fibroblast LTK- cells, which do not express ALDH1. Within this region, we define a minimal promoter (-91 to +53) that contains positive regulatory elements. The study using site-directed mutagenesis demonstrated that the CCAAT box region is the major cis-acting element involved in basal ALDH1 promoter activity in Hep3B cells. Gel mobility shift assays showed that NF-Y and other octamer factors bound CCAAT box and an octamer motif sequence, but not GATA site existing in the minimal promoter region. Two additional DNA binding activities associated with the minimal promoter were found in the nuclear extract from Hep3B cells, but not from K562 cells. These results offer the possible molecular mechanism of the cell type-specific expression of ALDH1 gene.

CGRP in peripherally axotomized mesencephalic trigeminal neurones of the rat.

Immunoreactivities for substance P (SP-ir) and calcitonin gene-related peptide (CGRP-ir) were examined in neurones in the mesencephalic trigeminal tract nucleus of nerve-injured rats. One week following unilateral transection of the trigeminal root, the masseteric nerve (MN) or the inferior alveolar nerve (IAN), CGRP-ir but not SP-ir was detected in these neurones bilaterally with ipsilateral dominance. Most of the CGRP-ir (immunoreactive) neurones were retrogradely labelled when fluorogold (FG) had been applied to the proximal stump of the MN or the IAN. Sham surgery including insult to the masseter muscle induced CGRP-ir while skin incision did not.

Molecular abnormality of a phosphoglycerate kinase variant (PGK-Alabama).

The molecular abnormality of a phosphoglycerate kinase variant associated with severe red cell enzyme deficiency ( about 4% of normal) and episodes of hemolysis with jaundice was examined. The Michaelis constants for the substrates and co-enzymes (1.3-diphosphoglycerate, 3-phosphoglycerate, ATP and ADP) were not grossly different from that of normal. However that variant enzyme was very labile in vitro. Nucleotide sequence analysis of the variant cDNA revealed a deletion of codon AAG in exon 7. The codon deletion should result in the election of one of the tandem lysine residues existing at amino acid 190-191 of the enzyme protein. Based on the three dimensional structure of the protein, molecular instability could could be induced by the deletion of a lysine residue.

Prevalence of dry eye in Japanese eye centers.

BACKGROUND: The purpose of the investigation was to ascertain the prevalence of dry eye in new outpatients. METHODS: A total of 2127 consecutive new outpatients seen in eight Japanese centers from April 1992 to January 1993 underwent comprehensive examinations, including double vital staining and measurement of tear film break-up time, basal tear secretion, and tear clearance. Dry eye was diagnosed if patients had abnormalities of both the tear film and the ocular surface. RESULTS: Three hundred fifty-nine patients (17%) had dry eye. There was no seasonal pattern for dry eye. The condition was significantly more common in Tokyo than in suburban areas (P < 0.01). The prevalence of dry eye in visual display terminal (VDT) users and contact lens (CL) wearers was significantly higher than in non-VDT users and non-CL wearers (P < 0.05 and P < 0.02, respectively). CONCLUSION: Our findings suggest that dry eye is one of the most common ocular disorders encountered by physicians. Furthermore, if patients use VDTs or wear CLs, the likelihood of dry eye occurring is higher.
