Prevalence of hepatitis B surface antigen among pregnant women in a low-risk population.

OBJECTIVES: To estimate the prevalence of chronic hepatitis B carriers among our low-risk obstetric population and to determine the need for routine screening. METHODS: A group of 1224 pregnant women, admitted to Gazi University Medical Center, were screened for HBsAg using an enzyme-immunoassay technique, and the obstetrical care records were reviewed. RESULTS: Fifty-three patients were seropositive with a prevalence of 4.33% for HBV chronic carrier status in our obstetric population. CONCLUSIONS: Routine HBsAg screening is advisable in our antenatal population.

Surgical treatment of urinary stress incontinence by a suburethral sling procedure using a Mersilene mesh graft.

A suburethral sling procedure using a Mersilene mesh has been performed in 24 patients. Six patients had recurrent incontinence after an unsuccessful past operation. A postoperative follow-up was done for an average of 24 months. Twenty-three patients were free of symptoms, while only 1 patient had recurrent incontinence. The suburethral sling procedure can be applied to all patients, particularly for recurrent stress urinary incontinence, with reasonable cure rates when performed with a proper technique.

Bone mineral density in Turkish postmenopausal women.

OBJECTIVE: To determine the bone mineral density curve in Turkish postmenopausal women and to evaluate the efficacy of estrogen replacement therapy in the prevention of bone loss. METHOD: Spinal bone mineral density was measured using dual photon absorptiometry in 118 postmenopausal women, prospectively. Parity, previous and current use of estrogen preparations were correlated with BMD using Student's t-test and the chi 2-test. RESULTS: With a mean BMD value of 0.967 +/- 0.013 g/cm2, 62.5% of our postmenopausal study population had a mean BMD value of < 1.0 g/cm2 and 33.7% were already osteoporotic (< 0.9 g/cm2). Bone loss was faster and more intense in the surgically menopausal than in the naturally menopausal group (P < 0.05). Parity after the fourth delivery dropped the mean BMD value (P < 0.05). Previous and current use of estrogen treatment seemed to decrease bone mineral loss in the postmenopausal years. CONCLUSION: Our data stress the need for locally obtained normal BMD values in every country in the world. High parity, surgical menopause and duration of 5 or more years of natural menopause are risk factors for bone loss. Estrogen replacement diminishes the annual bone loss.

Bone loss in young women with primary hypogonadism and its prevention with calcitonin.

A total of 63 healthy young women with primary amenorrhea, 37 control women at the same age with normal ovarian function and 118 postmenopausal women underwent bone mineral density (BMD) measurements at the lumbar spine (L2-L4) by dual photon absorptiometry. Mean +/- SE BMD value in the primary amenorrheic patients (0.878 +/- 0.015 g/cm2) was significantly lower than in both the control ovulatory women (1.069 +/- 0.026 g/cm2) and the postmenopausal patients (0.967 +/- 0.013 g/cm2) (p < 0.05). When hormonal parameters were compared between primary amenorrheic and ovulatory control cases, only the prolactin mean levels were found to be significantly lower in the first group (8.74 +/- 0.86 vs. 16.31 +/- 1.05 ng/ml; p < 0.001). Serum total alkaline phosphatase activity and LDL-cholesterol were negatively correlated with bone mineral density. Total cholesterol and lipid levels were significantly higher in women with primary amenorrhea when compared with the eumenorrheic control group (p < 0.001).

Tc-99m-HMDP bone uptake quantification and plasma osteocalcin levels in hemodialysis patients--a preliminary study.

In this preliminary study plasma osteocalcin levels and Tc-99m-HMDP (Technetium 99m hydroxymetylene diphosphonate) bone uptake (BU) were measured in 10 chronic end-stage renal failure patients who were on maintenance hemodialysis. The aim of this study was to determine the correlation between bone uptake and osteocalcin-a sensitive and specific marker of osteoblastic activity. There was a statistically significant increase in both 20 and 180 minute uptake in the patient group (36 +/- 2.7 and 39 +/- 3.6) when compared to the normal volunteers (32 +/- 3.1 and 19 +/- 2.7). Plasma osteocalcin levels were also significantly high (24.5 +/- 5.6 ng/ml) when compared with normal values (6.5 +/- 2.3 ng/ml). The correlations between osteocalcin and 20 and 180 min BU were high (r = 0.62 and 0.72 respectively). In conclusion, our preliminary study suggests that, in hemodialysis patients, Tc-99m-HMDP bone uptake quantification is a sensitive and non-invasive method for showing increased osteoblastic activity.

The value of 99mTc-DTPA renal scintigraphy in the evaluation of post-traumatic abdominal fluid collection.

A patient with a post-traumatic retroperitoneal urinoma is presented. On admission, there was a clinical suspicion of retroperitoneal hematoma and ultrasonography (US) was performed which showed a hypoechoic fluid collection suggesting retroperitoneal hematoma. In order to determine the nature of the fluid, radionuclide angiography and renal scan were performed successively with 99mTc-DTPA. Demonstration of urinary leakage into the mass in the delayed renal scintigraphic images suggested a urinoma. At laparotomy, total transection of the left ureter in the uretero-pelvic region was found and the diagnosis of urinoma was confirmed.

Treatment of estrogen-dependent gynecological disorders with the gonadotropin releasing hormone agonist buserelin.

The authors examined the effect and tolerability of buserelin in 40 women with endometriosis and ten women with uterine leiomyoma. Buserelin was given intranasally, 200 micrograms three times a day for 6 months. Laparoscopy was performed before and after, and ultrasonography during the treatment. Hormone and lipid profiles and other biochemical tests were run during the treatment. The bone mineral density was tested by dual photon absorptiometry before and after therapy in a group of patients. Although most of the patients complained of hot flushes, no women dropped out. AFS mean pelvic score decreased from 24.10 to 6.95 and the size of the fibroids decreased by 69% at the end of 6 months of treatment. In conclusion, our data suggest that the use of GnRH agonist has a place in the treatment of endometriosis and uterine leiomyoma but further studies are needed to conclude that buserelin given intranasally at a dose of 600 micrograms/day for 6 months is an alternative to other conventional medical treatment modalities in terms of pregnancy and recurrence rates.

Prevalence of Chlamydia trachomatis infection in the Turkish female population.

Chlamydia trachomatis infection is the most common of the sexually transmitted diseases. In this study of screening for C. trachomatis infection in the Turkish female population, Chlamydia IgG enzyme immunoassay (Ch IgG EIA) was used to detect the exposure to C. trachomatis. Ch IgG EIA was studied in 469 women attending the Gynecology and Obstetrics Clinic of the University of Gazi School of Medicine. Ch IgG EIA was found positive in 35 (7.46%) blood specimens. The highest ratio of seropositivity was detected among the infertile population (11.17%).

Immunoperoxidase EM localisation of cytoplasmic actin in cultured fibroblasts.

Human smooth muscle autoantibody (SMA) of defined anti-actin specificity was tested by the indirect immunoperoxidase staining method on frozen sections of tissues and on cultured rat lung fibroblasts. The serum stained tissue sections and cultured fibroblasts in a pattern identical with that obtained by indirect immunofluorescence. Ultrastructural studies carried out on the immunoperoxidase stained cells showed that the long parallel filaments spanning the long axis of cultured fibroblasts seen by light microscopy correspond with the thick bundles of microfilaments.

Distribution of actin and myosin in muscle and non-muscle cells.

Specific anti-actin and anti-myosin antibodies were shown to react in single and double immunofluorescence sandwich tests with identical sites in non-muscle cells in frozen sections of tissues and in cultured cells. In tissues, both antibodies reacted with liver cell membranes, parts of renal glomeruli, brush borders and peritubular fibrils of renal tubules, brain synaptic junctions, and membranes of lymphoid cells in thymic medulla, lymph nodes and spleen. Both antibodies reacted strongly with long parallel cytoplasmic fibrils in cultured fibroblasts, and with disrupted fibrils in cytochalasin-B treated cells. In neuroblastoma cells both antibodies gave prominent staining of growth cones and microspikes. The observation that the distribution of myosin parallels that of actin in non-muscle cells argues strongly in favour of a functional interaction between the two molecules in the generation of contractile activity in non-muscle cells.

Mechanisms of smooth muscle antibody production: a clinical study in children with infections, haemolytic syndromes, and idiopathic thrombocytopenic purpura.

Sera from 530 children suffering from various diseases and from 64 controls were tested for smooth muscle autoantibodies (SMA) by indirect immunofluorescence. A high incidence of SMA (51-86%) was found in patients with viral and bacterial infections (viral hepatitis, infectious mononucleosis, measles, mumps, chickenpox, typhoid fever, and brucellosis), independently of liver invovlvement, and in patients with acute haemolytic anaemia due to G-6-PD deficiency (48%). By contrast, the incidence of SMA from patients with beta-thalassaemia major and idiopathic thrombocytopenic purpura was no higher than in the controls. The discrepancy in incidence in haemolytic anaemias due to different causes may reflect the effect of endogenous and extrinsic agents. In the viral infections, SMA were mainly of the IgM class and gave an 'SMA-V' staining pattern. In bacterial infections (typhoid fever and brucellosis), SMA were either IgG only or IgM and IgG, and the staining pattern was also mainly 'SMA-V'. In infections which affect or may affect the liver (viral hepatitis, infectious mononucleosis, typhoid fever, and brucellosis), SMA was present at high titres (1:80-1:320), whereas in infections not affecting the liver (measles, mumps, and chickenpox) the titres were lower (less than or equal to 1:80). In most patients SMA occurred transiently and without apparent pathogenetic significance. The antigen against which infection-induced SMA is directed is not actin; its nature has yet to be identified.

Viral infections and IgM autoantibodies to cytoplasmic intermediate filaments.

Seventy-four out of 113 sera from patients with infectious hepatitis, chickenpox, measles and mumps reacted with both smooth muscle and cytoplasmic filaments in cultured fibroblasts and neuroblastoma. Five out of eighty-five control sera also reacted in this way. That the cytoplasmic structures are intermediate filaments was suggested by their rearrangement into coils of perinuclear filaments in colchicine- or vinblastine-treated fibroblasts, but not in cytochalasin B-treated cells. The idenity of these structures was confirmed by the demonstration that the same structures reacted with the post-viral sera and a rabbit and human anti-intermediate filament antibody. Immunoabsorption studies showed that twenty-seven out of thirty-two positive sera were neutralised by skeletin, the intermediate filament protein from smooth muscle. In all but one of the sera, the antibody was IgM. Antibody titres fell in the second specimen in eleven out of fourteen pairs of acute and convalescent sera. The association between viral infections and autoantibodies suggest that production of antibodies suggests that production of antibody to intermediate filaments may be initiated by viruses.

Heterotopic pregnancy: case report.

Heterotopic pregnancy in a spontaneous cycle is a rare entity with an estimated frequency below one per 30,000 pregnancies. Its incidence evidently has increased in accordance with the widespread use of in vitro fertilization and ovulation induction. We report a case of heterotopic pregnancy in a 40-year-old woman who presented with acute abdominal pain. We also present findings from transvaginal ultrasound imaging.
