Purification and characterization of two kinds of porins from the Enterobacter cloacae outer membrane.

Two major outer membrane proteins of Enterobacter cloacae 206 were purified and identified as porins by using reconstituted vesicles. The 37-kilodalton porin forms a channel with a radius of 0.6 nm, which prefers positively charged substances to negatively charged ones, whereas the 39- to 40-kilodalton porin forms a larger channel with a radius of 0.8 nm, which has weaker selectivity for electric charges.

The effects of diethyldithiocarbamate and carbon disulfide on acute nephrotoxicity induced by furan, bromobenzene and cephaloridine in mice.

In previous studies (Masuda and Nakayama, 1983), diethyldithiocarbamate (DTC) and carbon disulfide (CS2) have been found to be protective against acute nephrotoxicity induced by CHCl3 and 1,1-dichloroethylene in normal and CCl4-poisoned mice, and it has been suggested that the protective action of DTC and CS2 might be mediated through inhibition of bioactivation of these nephrotoxicants in the kidney. As an extension of these studies, similar experiments were undertaken with furan, bromobenzene and cephaloridine, other nephrotoxic agents that are also thought to require metabolic activation. DTC or CS2 prevented mice from suffering renal injury induced by furan and bromobenzene, as evidenced by suppression of elevations in plasma urea nitrogen concentration and kidney calcium content and of morphologic alterations. Cephaloridine nephrotoxicity, however, was not prevented. In CCl4-poisoned mice, furan nephrotoxicity was augmented, whereas bromobenzene and cephaloridine nephrotoxicity was suppressed. The augumented furan nephrotoxicity was also prevented by DTC or CS2. These observations suggest that furan, like CHCl3 and 1,1-dichloroethylene, may exert nephrotoxicity through active metabolites formed in the kidney. For bromobenzene and cephaloridine nephrotoxicity, a renal bioactivation mechanism is suspected. DTC and CS2 also protected against hepatotoxicity induced by furan and thiophene.

Serum erythropoietin (ESF) levels and erythroid progenitors (CFU-Es) of patients with chronic myeloproliferative disorders.

Serum erythropoietin (ESF) levels and the numbers of marrow and blood erythroid progenitors (CFU-Es) of patients with chronic myeloproliferative disorders (CMPD) were studied simultaneously. The numbers of marrow and blood CFU-Es per 1 x 10(5) cells were normal or greatly elevated. There was an inverse correlation between the hemoglobin concentration and the serum ESF level in patients with chronic myelogenous leukemia when the hemoglobin concentration ranged from 9.0 to 13.0 g/100 ml. The serum ESF level was closely related to the hemoglobin concentration in CMPD and it was suggested that the negative feedback mechanism might operate in anemic patients with CMPD.

[The appraisal of biliary drainage for intrahepatic gall stones, with special reference to an end-to-side hepaticojejunostomy].

Between 1961 and 1983, eighty-six patients with intrahepatic gall stones were identified from a group of 1,140 patients admitted for cholelithiasis. Surgical procedures performed in this series were 37 choledochotomies with external biliary drainages, 33 transduodenal papilloplasties, 37 bilioenteric anastomosis and 15 hepatectomies. In the long term follow up studies after surgical treatment by each procedures, the favorite results were obtained in 41.2% of cases with choledochotomies with external drainages, in 34.4% of cases with transduodenal papilloplasties, in 82.4% of cases with bilioenteric anastomosis and in 88.7% of cases with hepatectomies. Hepatectomy seemed to be a most effective treatment for the prevention of recurrence of stones. However, if the calculi were in the right or both of hepatic lobes, hepatectomy might be a high risk operation and technical proficiency were required in operation. In such cases, we performed an end to side anastomosis between the common hepatic duct and the jejunum (Roux en Y anastomosis) for the postoperative endoscopic lithotomy. In this operation the jejunal stump was made to be an enterocutaneous fistula for the later percutaneous endoscopic lithotomy. This operation has the following advantages; 1) the time of operation were shortened in the removal of stones because of this operation were performed for the postoperative endoscopic lithotomy, 2) retrograde cholangitis was less likely to develop than other operations, 3) retained stones dropped into bowel easily by making a big anastomotic stoma, 4) the cholangioscope could be inserted into either bile ducts of the right and left hepatic lobes, 5) the fistula would be able to be reused for the endoscopic treatment at later recurrence of intrahepatic gall stones.

Antigenic reactivities of purified preparations of carcinoembryonic antigen (CEA) and related normal antigens using four different radioimmunoassay systems for CEA.

Antigenic reactivities of 4 purified carcinoembryonic antigen (CEA) preparations including the WHO standard CEA and 4 CEA-related antigens--nonspecific cross-reacting antigen (NCA) from normal lungs, nonspecific cross-reacting antigen-2 (NCA-2) from meconium, normal fecal antigen-1 (NFA-1) and normal fecal antigen-2 (NFA-2) from normal adult feces--were investigated using 4 different radioimmunoassay (RIA) systems (Hoffman-La Roche, CEA-IRE-SORIN, Dainabot and our system). In general, all CEA preparations including the respective standard CEAs gave similar reactions with these assay systems. Differences in reaction intensity were, however, observed among the CEA preparations, and the order of reaction intensity among the preparations varied due to assay systems, e.g., a given CEA preparation which reacted most strongly with an assay system reacted very weakly with another assay system, and vice versa. Both NCA-2 and NFA-2, which possessed physiochemical properties very similar to those of CEA, revealed reactivities indistinguishable from those of the CEA preparations in all systems. NCA, which showed a partial cross-reactivity with CEA in the double immunodiffusion test, revealed a remarkably weak reactivity in all systems. NFA-1, however, which was also partially cross-reactive with CEA but antigenically unrelated to NCA, showed a fairly strong reactivity in all systems, but its dose-dependent slope was apparently different from that of the other antigen preparations.

Brachymesomelia-renal syndrome.

We have studied a male Japanese infant with severe upper limb brachymesomelia, glomerulocystic renal dysplasia, abnormalities of the cranium and face, corneal opacities, and a possible congenital heart defect. He was born at term and died on the 10th day of heart and kidney failure. Review of the literature failed to show a similar case. Glomerulocystic renal dysplasia has been reported in association with a variety of the nonskeletal malformations but has not previously been described in association with bony malformations.

The role of vitamin D in the medullary bone formation in egg-laying Japanese quail and in immature male chicks treated with sex hormones.

The effect of vitamin D3 on medullary bone formation was investigated in egg-laying Japanese quail and in immature male chicks treated with sex hormones. When laying quail were fed a vitamin D-deficient diet for 16 days, their eggshell weights and egg production rate were markedly reduced in a time-dependent manner with a significant decrease in plasma calcium and 25-hydroxyvitamin D3 levels. The calcium content of the medullary bone of femurs decreased markedly with the progress of vitamin D deficiency, whereas that of the cortical bone remained unchanged. Quantitative histological examination also showed that the area of the mineralized portion of medullary bone in quail that were fed the vitamin D-deficient diet markedly decreased compared with that in the control laying quail, whereas the total area of the mineralized and unmineralized portions of medullary bone in the bone marrow cavity increased moderately. Daily administration of vitamin D3 (0.75 microgram/day) to the vitamin D-deficient quail increased the mineralization of medullary bone as early as day 4. Daily administration of both estradiol (0.3 mg/day) and testosterone (0.9 mg/day) for 3 weeks to immature male chicks induced an apparent hypercalcemia and matrix formation of medullary bone, regardless of the vitamin D status of the chicks. Mineralization of medullary bone was observed only when vitamin D3 was administered together with the sex hormones. These results suggest that vitamin D3 is directly involved in the mineralization of medullary bone in birds.

The value of transrectal ultrasonography in preoperative assessment for transurethral prostatectomy.

Transrectal ultrasonography was done in 19 healthy men and 236 patients with benign prostatic hyperplasia to elucidate the pathomorphology of the prostate and to determine preoperative prostatic size in patients who underwent transurethral prostatectomy. We found that the ultrasonographic examination allowed for an accurate estimation of the prostatic weight and a good assessment of the completeness of resection of the prostatic adenoma.
