Localization of the mRNA for bone matrix proteins during fracture healing as determined by in situ hybridization.

The expression of the mRNAs for osteonectin (ON), osteopontin (OPN), osteocalcin (OC), and matrix Gla protein (MGP) was studied by in situ hybridization during the healing process of an experimental fracture in adult rat femora. At day 1 postoperatively, ON mRNA was detected in the proliferating periosteum. At day 3, ON, OPN, and OC mRNAs were detected in woven bone. From day 5, MGP and ON mRNAs were detected in the immature chondrocytes. From day 7, ON, OPN, and OC mRNAs were detected in the osteoblastic cells in newly formed endosteal trabecular bone. OPN mRNA was also detected in some of the osteocytes in trabecular bone. From day 14, OPN and MGP mRNAs were detected in newly formed periosteal hypertrophic chondrocytes, and the ON, OPN, and OC mRNAs were detected in osteoblastic cells in newly formed periosteal trabecular bone. Although the cell types that expressed each mRNA in fractured bones were similar to those in embryonic bones, the time course of these mRNA expression in fractured bones was different from that in embryonic bones. We considered that this system is useful to investigate the phenotypic change in osteogenic and chondrogenic lineage cells that appears during fracture healing at the molecular level.

Increased capillary density due to atrophy of ischaemic soleus muscle of the rat.

This investigation was undertaken to determine whether a severe limitation of blood supply by external iliac artery ligation can change the number and density of capillaries in rat soleus muscle. The external iliac artery in one hindlimb was ligated for 2, 7 or 28 days, and the other, sham-operated, hindlimb was used as a control. Muscle blood flow in the ischaemic soleus muscle at 2, 7 and 28 days after external iliac artery ligation was significantly decreased compared with the control. The muscle fibre area and the ratio of the fibre area to body mass in the ischaemic soleus muscle at 28 days after the external iliac artery ligation were significantly reduced in the ischaemic soleus muscle, but no change in the number of capillaries per fibre was observed. Capillary density per millimetres squared at 28 days and the ratios of the number of capillaries around type I fibres to the fibre area at 7 and 28 days were significantly increased in the ischaemic soleus muscle (P < 0.05). These results suggested that long-term severe limitation of blood flow in the soleus muscle by ligation of the external iliac artery could have induced the increase in capillary density, as a result of atrophy of muscle fibres rather than capillary growth.

cDNA cloning and functional characterization of a meiosis-specific protein (MNS1) with apparent nuclear association.

It is well known that cytoskeleton and karyoskeleton proteins are associated with changes in cell shape and with the rearrangement of the dynamic structures involved in cell division and motility. In higher vertebrates, there are three major skeletal protein groups: microfilaments, microtubules and intermediate filaments, each representing a multigene family. Some of these skeletal proteins are expressed in a temporally- and spatially-specific fashion, and they establish cell-specific cytoplasmic and nucleoplasmic organization during development. Here we report the cDNA cloning of a novel 60 kDa skeletal protein from mouse spermatocytes, termed MNS 1 (meiosis-specific nuclear structural protein), whose computer-predicted protein configuration indicates long alpha-helical coiled-coil domains flanked by non-helical terminal domains. Functional characterization of MNS1 by ectopic expression in culture cells indicated that it is a detergent- and high salt-resistant skeletal protein which is involved in organization of the nuclear or perinuclear architecture. The MNS1 protein is specifically expressed at the pachytene stage during spermatogenesis, so that its function may involve the determination and maintenance of the appropriate nuclear morphology during meiotic prophase.

Limitation of panoramic radiography in diagnosing adenomatoid odontogenic tumors.

Adenomatoid odontogenic tumor is frequently not diagnosed preoperatively even when radiographs are available. This study evaluates radiographic findings with special emphasis on the characteristic radiopacities of adenomatoid odontogenic tumor. In correlating radiologic findings with pathologic findings, differences in diagnostic accuracy between intraoral periapical and panoramic radiographs were found. The intraoral periapical radiograph allows perception of the radiopacities in adenomatoid odontogenic tumor as discrete foci having a flocculent pattern within radiolucency even with minimal calcified deposits. Panoramic radiography was often unable to demonstrate radiopacities in adenomatoid odontogenic tumor when the calcification was minimal. The result indicates that intraoral radiographs may be essential for correct radiographic interpretation of an adenomatoid odontogenic tumor in the presence of minimal quantities of calcified deposits.

Bone morphogenetic protein-2 converts the differentiation pathway of C2C12 myoblasts into the osteoblast lineage.

The implantation of bone morphogenetic protein (BMP) into muscular tissues induces ectopic bone formation at the site of implantation. To investigate the mechanism underlying this process, we examined whether recombinant bone morphogenetic protein-2 (BMP-2) converts the differentiation pathway of the clonal myoblastic cell line, C2C12, into that of osteoblast lineage. Incubating the cells with 300 ng/ml of BMP-2 for 6 d almost completely inhibited the formation of the multinucleated myotubes expressing troponin T and myosin heavy chain, and induced the appearance of numerous alkaline phosphatase (ALP)-positive cells. BMP-2 dose dependently induced ALP activity, parathyroid hormone (PTH)-dependent 3',5'-cAMP production, and osteocalcin production at concentrations above 100 ng/ml. The concentration of BMP-2 required to induce these osteoblastic phenotypes was the same as that required to almost completely inhibit myotube formation. Incubating primary muscle cells with 300 ng/ml of BMP-2 for 6 d also inhibited myotube formation, whereas induced ALP activity and osteocalcin production. Incubation with 300 ng/ml of BMP-2 suppressed the expression of mRNA for muscle creatine kinase within 6 h, whereas it induced mRNA expression for ALP, PTH/PTH-related protein (PTHrP) receptors, and osteocalcin within 24-48 h. BMP-2 completely inhibited the expression of myogenin mRNA by day 3. By day 3, BMP-2 also inhibited the expression of MyoD mRNA, but it was transiently stimulated 12 h after exposure to BMP-2. Expression of Id-1 mRNA was greatly stimulated by BMP-2. When C2C12 cells pretreated with BMP-2 for 6 d were transferred to a colony assay system in the absence of BMP-2, more than 84% of the colonies generated became troponin T-positive and ALP activity disappeared. TGF-beta 1 also inhibited myotube formation in C2C12 cells, and suppressed the expression of myogenin and MyoD mRNAs without inducing that of Id-1 mRNA. However, no osteoblastic phenotype was induced by TGF-beta 1 in C2C12 cells. TGF-beta 1 potentiated the inhibitory effect of BMP-2 on myotube formation, whereas TGF-beta 1 reduced ALP activity and osteocalcin production induced by BMP-2 in C2C12 cells. These results indicate that BMP-2 specifically converts the differentiation pathway of C2C12 myoblasts into that of osteoblast lineage cells, but that the conversion is not heritable.

Hot spots for sulfhydryl inactivation of Cys mutants in the widely conserved sequence motifs of the metal-tetracycline/H+ antiporter of Escherichia coli.

Unique hot spots for sulfhydryl inactivation of Cys mutants of the metal-tetracycline/H+ antiporter (TetA) were found at the fourth positions of the dual conserved sequence motifs, GKXXDRXGRR and GRXXXKXGEK [Yamaguchi, A., Kimura, T., Someya, Y., & Sawai, T. (1993) J. Biol. Chem. 268, 6496-6504]. The fourth positions of these motifs are occupied by Ser65 and Ala269, respectively. N-Ethylmaleimide (NEM) rapidly bound to and inactivated the S65C and A269C mutants. M64C and I268C showed low reactivity to NEM, probably due to the partial cripticity of these residues. In contrast, NEM rapidly bound to T270C but did not inactivate it. NEM completely inactivated S65C, whereas a small sulfhydryl reagent, methyl methanethiosulfonate (MMTS), caused only 40% inactivation. [14C]NEM binding to S65C was inhibited by tetracycline. These observations indicated that position 65 is located close to the substrate-protein interaction site and that the inactivation by sulfhydryl reagents comprises volume-dependent steric hindrance. The S65M mutant, having a side chain analogous to one of the thiomethyl cysteines of MMTS-modified S65C, showed about 30% of the Vmax value for the wild-type tetracycline transport, while the S65F mutant had completely lost the activity, confirming the idea of volume-dependent steric hindrance at position 65. On the other hand, the A269C mutant was greatly inactivated by both NEM and MMTS. The degree of the inactivation by MMTS (90%) was higher than that of NEM (80%). The synergetic inactivation observed for the S65C/A269C double mutant suggested different inactivation mechanisms for A269C and S65C.(ABSTRACT TRUNCATED AT 250 WORDS)

Assessment of aortic disease using three-dimensional CT angiography.

The recently developed technique of three-dimensional CT angiography (3D-CTA) was applied to 68 patients with aortic disease. These patients were examined using a unique method of data collection: a helical scanner with continuous tube rotation and continuous table feed. The scanner was used in conjunction with administration of dynamic intravenous contrast material to enhance the vascular image. The group of patients included 35 with aortic dissection, 19 with true thoracic aneurysms, and 14 with abdominal aortic aneurysms. Three-dimensional evaluation was achieved in all patients with no complications. Surgical intervention was used in 45 patients with aortic dissections and aortic aneurysms, and 44 of these patients (98%) survived and were discharged. Preoperative 3D-CTA findings were quite similar to intraoperative findings, and were useful in determining operative procedures. Rapid and accurate assessment of aortic disease was achieved by 3D-CTA. Three-dimensional CT angiography can play an important role in the preoperative assessment of aortic dissections and aortic aneurysms leading to successful surgical treatment.

Extracorporeal shock wave lithotripsy monotherapy for staghorn calculi.

Forty-eight patients with large renal staghorn calculi of more than 35 mm in maximum length on plain X-rays were treated by extracorporeal shock wave lithotripsy (ESWL) monotherapy for a period of 2 years. Thirty patients, with a follow-up of more than 6 months after the last treatment, were included in this study. The surface area of the calculi was 1,290 mm2 on average. Based on plain X-rays, 9 cases (30%) became free of calculi while 12 patients (40%) had a small amount of residual fragments (less than 100 mm2 in area). Adding these cases to those in which all the residual calculi were eliminated, the total turned out to be 21 cases (70%) and the treatment of staghorn calculi with ESWL was thus considered to have been fairly effective in the present series. The remaining 9 cases (30%) contained a considerable amount of residuals (more than 100 mm2). The present study in the ESWL of large staghorn calculi revealed no clear relationship between the surface area of the stones and evacuation of the fragments. The amount of the residual fragments was significantly small when either the renal collecting system was less than 2,000 mm2 in area (p < 0.05), or when the ureteropelvic junction (UPJ) was equal or greater than 5 mm in diameter (p < 0.02) according to intravenous pyelography (IVP) before treatment. The complications associated with this treatment were minimal, with a high fever in only 3 patients that were treated easily by antibiotic therapy.(ABSTRACT TRUNCATED AT 250 WORDS)

[The efficacy of both antegrade and retrograde cardioplegia in coronary artery bypass surgery].

We evaluated the efficacy of the cardioplegic myocardial protection by combining antegrade cardioplegic induction via the aortic root with retrograde cardioplegia via the coronary sinus in patients who had LAD stenosis or occlusion with collateral circulation. In study I, forty-six patients were divided into two groups: antegrade group (group A; 32 cases), which were perfused by only antegrade fashion via the aortic root, and bi-directional group (group B; 14 cases), which were perfused by combining antegrade with retrograde fashion. In study II, patients were divided into four groups: 32 cases in group A were divide into three subgroups by the direction of the collateral circulation to LAD in preoperative coronary angiograms: group I (antegrade collateral circulation; 18 cases), group II (retrograde collateral circulation; 7 cases), group III (99% stenosis with delayed filling; 7 cases) and group B was as group IV. Myocardial injury was assessed with the use of enzyme indexes (total CPK, CPK-MB, %MB, peak CPK, peak CPK-MB and peak % MB) in study I and study II. There was no significant difference between the two groups in study I and the four groups in study II in terms of patients' profile, graft materials, number of grafts, aortic cross-clamping time, total cardiopulmonary bypass time and surgical results. Total CPK level in group B was significantly lower (p < 0.05) compared with group A at the point of 12 hr and 1 day after surgery and peak CPK level was also significantly lower (p < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Reoperation for recurrent chondrosarcoma of the pulmonary trunk].

A 71-year-old woman who had progressive dyspnea and edema two years after resection of a pulmonary trunk chondrosarcoma was diagnosed as severe obstruction to the pulmonary trunk due to the recurrent tumor. Magnetic resonance examination following the intravenous administration of Gd-diethylenetriamine pentaacetic acid (DTPA) revealed striking contrast enhancement within the lesion of the pulmonary trunk sarcoma. At reoperation, the recurrent mass was identified within the same site of the pulmonary trunk and total resection of the tumor and replacement of the pulmonary trunk with prosthetic graft was performed utilizing a cardiopulmonary bypass. There have been very few reports on replacements of pulmonary trunks for pulmonary artery sarcomas. It is reported that the prognosis of the pulmonary trunk sarcoma is extremely poor, however, this patient has undergone thoracotomies twice within two years and continued to be symptom-free without distant metastatic lesions.

[Free arterial graft for coronary bypass grafting].

Free arterial grafts were aggressively placed in 39 patients (1991 to 1993). There were 34 males and 5 females, and mean age was 59.9 year old. Of 85 arterial grafts, 41 were free grafts, and their materials included left and right internal thoracic artery (LITA, RITA) and right gastroepiploic artery (GEA). There were one free LITA-left anterior descending coronary artery (LAD), seven free RITA-LAD, three free RITA-diagonal branch (Dx), 14 free RITA-left circumflex coronary artery (LCX), 10 free RITA-right coronary artery (RCA), two sequential RITA-Dx-LCX, one free GEA-Dx, two free GEA-LCX, and one free GEA-RCA bypass. Of 41 free arterial grafts, 38 were in the aorta-coronary position, and the proximal anastomosis was constructed first under single aortic cross-clamping to get the larger anastomotic sites for both at the proximal and distal ends of the arterial graft. The proximal sites of the remaining 3 arterial grafts were placed to concomitantly utilized saphenous vein grafts in two patients and RCA in one patient because of their shortness. Perioperative complications included no exploration for bleeding, myocardial infarction in one (2.6%), intra-aortic balloon pumping in three (7.7%), and wound complications in two (5.1%). 28 cases (72%) were completely revascularized with only arterial grafts. Of 41 free arterial grafts studied within one month after operation, all grafts were patent. All patients were free from angina after a 27 months mean follow-up. We believe that the proximal anastomosis technique for free arterial graft we used could be acceptable alternative for many surgeons. These excellent results justify wider use of free arterial grafts.

[Preoperative assessment of aortic dissection by three-dimensional CT angiography].

In 21 patients with aortic dissection including 12 patients with acute aortic dissection, aortic lesions were evaluated preoperatively by newly developed three-dimensional CT angiography (3 D-CTA) using helical scan CT. Rapid and safe three-dimensional evaluation of aortic dissection was achieved with no complications. Three-dimensional structure of aortic dissection was well reflected in the image of 3D-CTA. Preoperative three-dimensional evaluation was useful to determine and proceed the operative procedure for the repair of aortic dissection. All patients were alive and results of surgical treatments were excellent. Intimal flaps and entries were visualized by multiple threshold display (MTD) method of 3D-CTA. Multiple threshold display method is an important three-dimensional reconstructive method and 3D-CTA may be a new useful and powerful diagnostic modality for aortic dissection.

[Quadruple coronary artery bypass grafting with arterial grafts--application of internal thoracic artery and right gastroepiploic artery].

Arterial graft has been widely applied for coronary artery bypass because of its excellent long-term patency. However, when more than four coronary bypass graftings should be carried out using internal thoracic arteries (ITAs) and right gastroepiploic artery (GEA), some surgical techniques must be devised. We performed quadruple coronary artery bypass grafting using three arterial grafts; bilateral ITAs and GEA, and obtained good results. We used free RITA graft for sequential grafting in one case. In another case, GEA was divided into two segments and used as two free grafts. From our experience, we believe it is possible to perform multiple coronary revascularization using only arterial grafts with one median skin incision.
