Personal construct systems of psoriatic patients.

Repertory grid techniques were employed to assess the personal construct systems of psoriatic patients (n = 35), controls with other skin conditions (n = 28) and controls with no skin condition (n = 29). A grid was designed with six supplied constructs representing previously reported subjective experiences of psoriatics, and 12 elements representing typical social conditions. Social adjustment was defined as the relationship between constructs. The results suggest that psoriatics are less well socially adjusted in highly visible situations. There was no difference in social adjustment between males and females specific to the psoriatic group, but body awareness in female psoriatics correlated positively with the severity of the condition.

Increased collagen-linked fluorescence in skin of young patients with type I diabetes mellitus.

Our objective was to determine whether the fluorescence of skin collagen, which may reflect the accumulation of advanced glycosylation end products, is increased in young patients with type I (insulin-dependent) diabetes. Our study design was a cross-sectional case-control study in a referral-based diabetic clinic in an academic hospital. Study subjects comprised a convenience sample of 18 type I diabetic patients aged 17-30 yr and 8 age-matched healthy control subjects. The fluorescence of collagen was measured in skin biopsy material. Collagen-linked fluorescence (CLF) was increased in diabetic patients (mean 10.5 [range 5.8-15.8] U/mg) compared with control subjects (7.6 [5.6-10.1] U/mg, P less than 0.02). In diabetic patients, CLF was related to age (r = 0.581) and duration of diabetes (r = 0.697) but not concentration of glycosylated hemoglobin (r = 0.082). Partial correlation analysis demonstrated that duration of diabetes is the main factor determining the fluorescence of collagen in these patients. There was a relationship between CLF and presence of diabetic retinopathy after the data were adjusted for patient age and duration of diabetes (P = 0.023). Increased fluorescence of skin collagen can be detected in young type I diabetic patients and is primarily related to duration of diabetes.

Integrated in-patient and out-patient dermatology management.

The physical integration of out-patient clinics, out-patient treatment and in-patient management areas allows full cohesion and continuity of medical and nursing care of dermatology patients. The conversion of a dermatology ward to an out-patient treatment centre has resulted in major benefits in the daily management of dermatology patients, allowing increased flexibility of care despite a reduction in bed numbers and financial savings. Broad changes in concepts of dermatology management are described and arguments given which may be used in the negotiation for improved facilities.

How much disability is caused by acne?

One-hundred patients completed a questionnaire designed to assess the disability resulting from acne; patient acne severity was also graded clinically. Ten questions which correlated strongly with clinical acne severity were used to form an Acne Disability Index (ADI). This ADI correlated with the severity of facial acne (r = 0.246, P less than 0.01), chest acne (r = 0.347, P less than 0.001) and back acne (r = 0.436, P less than 0.001). Measures were made of the financial value to patients of acne treatment: when hypothetically offered either a cure for their acne or 500 pounds, 87% of patients preferred the treatment rather than the money. All 13 patients who stated a preference for the 500 pounds had minimal acne. There is no correlation between the clinical grading of acne and the amount patients would be prepared to pay for a hypothetical cure but there is a correlation between the acne disability score and the amount patients would pay (r = 0.229, P less than 0.05).

A patient with Tay's syndrome.

A 4-year-old girl with Tay's syndrome had the typical features of congenital ichthyosis and the peculiar anomaly of hair growth, trichothiodystrophy. Her hair shafts were brittle and showed alternating light and dark banding when examined microscopically between polarizing filters. Her hair cystine content was 4.6% (control 8.4%).

Assessment of disease progress in psoriasis.

Effective methods of recording and measuring progress of the signs of psoriasis are essential and central to the accurate assessment of therapy. Currently there is wide variation in assessment methods used, and even well-designed clinical assessment techniques have a wide observer error: objective assessment methods may have advantages. The clinical assessment techniques used in 30 clinical trials of treatment in chronic plaque psoriasis in 1985 and 1986 were reviewed and a detailed analysis is presented. In only 18 trials were individual regions assessed, and widely differing methods of scoring signs were used. Reproducibility of the assessment by clinicians of area of skin involvement by psoriasis was studied. Ten patients with chronic plaque psoriasis had standardized photography, and four clinicians assessed area of involvement from the photographs. The clinicians significantly differed in their rank ordering of the area of psoriatic skin involvement. Area of involvement by psoriasis in the same photographs was assessed objectively using computer-assisted planimetry. Although the clinicians' assessment did not differ randomly from the objective measurement, the accuracy of subjective area assessment is not the same for all clinicians. As even experienced clinical observers show wide variation when attempting to record clinical severity of psoriasis, there is clearly a need for better objective measures of psoriasis severity.

The effect of the vehicle formulation on the stratum corneum penetration characteristics of clobetasol 17-propionate in vivo.

The multiple skin surface biopsy technique to measure stratum corneum (SC) penetration characteristics in vivo has been more accurately quantified and used to provide information on the kinetics of transfer of a topical steroid through the SC. Radiolabelled clobetasol 17-propionate formulated in two different vehicles, consisting primarily of dibutyl adipate or of propylene glycol, was applied and the penetration characteristics compared. Using exponential expressions for radioactivity in skin surface biopsies as a function of SC depth, and total SC radioactivity as a function of time, a mathematical model was developed to calculate the rate of loss of steroid through superficial shedding and the rate of transfer of steroid from the SC into the epidermis. When a topical steroid was applied for 8 h under gauze occlusion, over 80% was recovered from the gauze.

The lateral spread of clobetasol 17-propionate in the stratum corneum in vivo.

Radiolabelled clobetasol 17-propionate in two different bases, propylene glycol (PG) and di-n-butyl adipate (BA), was applied to the backs of 10 normal male volunteers, and the application sites protected. After 8 h samples of stratum corneum (SC) were taken from the application site and an adjacent site, and the level of steroid measured. At the application site there were higher concentrations of the PG formulation at the deeper levels of the SC. At the adjacent sites there were higher concentrations of the BA formulation at the superficial levels. The mean radioactivity at the adjacent sites expressed as a percentage of that at the corresponding application sites (+/- SEM) was 7.8 +/- 5.3% for the BA formulation and 3.0 +/- 2.9% for the PG, (P = 0.0002). Total percentage of the applied dose in the skin adjacent to the application site was 11.7 +/- 6.3% for the BA formulation and 5.3 +/- 5.7% for the PG (P less than 0.0002). Clobetasol 17-propionate, therefore, moves laterally to a greater extent in BA than in PG.

Lymphomatoid granulomatosis presenting with polyarthritis.

Polyarthritis in lymphomatoid granulomatosis is rare. In the present report we describe a patient with lymphomatoid granulomatosis who presented with an acute inflammatory polyarthritis three years before the typical skin and pulmonary manifestations of the disease became evident.

Ultrasound transmission time: an in vivo guide to nail thickness.

Human fingernail thickness was measured distally in 39 subjects using an adapted electronic micrometer and ultrasound transmission time was measured both distally and proximally. Distal ultrasound nail transmission time correlated well with micrometer measurements. There was a clear rank order of distal and proximal nail transmission times: thumb greater than index greater than middle greater than ring greater than little fingers. Male nails had greater transmission times than female nails on the ring and little fingers. Distal nail transmission time was 8.8% less than proximal transmission time.
