The new Israeli health care reform: an analysis of a national need.

This paper describes the current situation of health care services in Israel. Major problems are discussed and analyzed in terms of the dualism of the main health organizations (the Ministry of Health and the General Sickness Fund), the multiplicity and discontinuity of health care delivery, quality-of-care problems, and the uneven geographical distribution of facilities. A proposal for a reform of the health care system is outlined, and its principles enumerated. This reform, suggested by the Ministry of Health, reflects a new approach of separating the direct provision of care from the executive functions of planning and control of services. The Ministry's proposal is analyzed, and its implications are discussed in relation to the American health care system.

Hospital admissions in Israel--1957-1980: a retrospective study.

Standardized admission ratios (SAR) among the total and the aged populations in Israel from 1957 to 1980 were studied in relation to a variety of explanatory variables that were classified into factors of structure, utilization, and patient socioeconomic indices. Because of a strong time correlation, residuals of the correlation between each variable and time were analyzed. The analysis revealed that the technological factor, the rate of physician-patient contacts, and the percentage of urban population proved to be the strongest variables accounting for hospitalization among the total population. Residence in a single-person household emerged as the major explanatory variable for hospitalization of the aged population.

Combining health services in a development town in Israel.

A workshop on the evaluation of health and welfare services in an Israeli development town was conducted by the Hebrew University-Hadassah School of Public Health and Community Medicine in the town of Beit Shemesh. The extensive field study by the second-year M.P.H. students led to a better understanding of community problems and needs and to the formulation of a model for the integration of health services. The workshop's outcomes, as evaluated by the participants, revealed close cooperation between the participants and the local health and welfare personnel and a positive appraisal of the workshop's goals.

Israel's National Center for Public Health--a novel conceptual approach.

Adoption of the new perspective of public health as a comprehensive and multidisciplinary mixture of objectives and activities requires a novel approach to the planning and the evaluation of health programs and to the training of health personnel. The implication of this process for the Israeli health arena suggests the establishment of a national center for public health. The cornerstones of the Israel National Center for Public Health (INCPH) consist of the classic triad: health care providers, leaders of regulatory agencies, and representatives of the academic institutions. The INCPH basic units would be structured by a top executive board of directors, by steering or professional committees whose main objectives are to develop criteria and guidelines and the evaluation of projects, and by a permanent staff group to maintain primary responsibility for the implementation of the center's programs.A number of practical steps have been taken in respect to the establishment of the center. The suggested mode of operation encompasses a variety of mechanisms to promote research in and planning or evaluation of health services and a nationwide effort to coordinate health manpower education by combining expertise and knowledge of all the nation's teaching institutions. International activity consisting of the education of health professionals from developing countries is also envisaged.
