IUPAC collaborative trial study of a method to detect genetically modified soy beans and maize in dried powder.

This paper presents results of a collaborative trial study (IUPAC project No. 650/93/97) involving 29 laboratories in 13 countries applying a method for detecting genetically modified organisms (GMOs) in food. The method is based on using the polymerase chain reaction to determine the 35S promotor and the NOS terminator for detection of GMOs. reference materials were produced that were derived from genetically modified soy beans and maize. Correct identification of samples containing 2% GMOs is achievable for both soy beans and maize. For samples containing 0.5% genetically modified soy beans, analysis of the 35S promotor resulted also in a 100% correct classification. However, 3 false-negative results (out of 105 samples analyzed) were reported for analysis of the NOS terminator, which is due to the lower sensitivity of this method. Because of the bigger genomic DNA of maize, the probability of encountering false-negative results for samples containing 0.5% GMOs is greater for maize than for soy beans. For blank samples (0% GMO), only 2 false-positive results for soy beans and one for maize were reported. These results appeared as very weak signals and were most probably due to contamination of laboratory equipment.

[Surgical treatment of autonomous chronic pancreatitis (apropos of 147 cases)].

Surgery in chronic pancreatitis gives only poor results: 40 p. 100 of cures or long-term improvements. Apart from certain mistaken indications, three factors are associated with a poor prognosis: calcification, late operation, persistence of alcoholism. However, mechanical complications, such as jaundice, pyloric stenosis, digestive hemorrhage and pseudo-cyst, are the main undoubted indications for operation. The choice of operation depends mainly on the state of Wirsung's duct. Usually, either part of the gland is removed or an internal digestive fistula is performed. Removal of the left lobe of the pancreas gives good results. Cephalic duodeno-pancreatectomy which is satisfactory in the short term, does not give, in the long run, better results than those of drainage operations. Pseudo-cysts are treated by removal when situated in the body or the tail and a drainage operation in the cephalic form. Simple operations should be preferred.

[Respective value of direct vision or optical lumboscopy].

The lumbar retroperitoneal space can be explored with 2 endoscopic procedures: direct lumboscopy with the Hald-type retroperitoneoscope and optical lumboscopy with a laparoscope inserted into the retroperitoneal space after CO2 insufflation. The latter procedure was tried without success due to the insufficient opening of the lumbar retroperitoneal space by the retropneumascos. On the contrary, direct lumboscopy was performed 44 times and yielded encouraging results as histological interpretation was possible for 40 samples (sensitivity 90%). This preliminary experiment seems to demonstrate the superiority of direct lumboscopy over optical lumboscopy, although the latter procedure theoretically has the advantage of a greater field of view.

[Malpighian carcinomas of the thoracic esophagus. Pretherapeutic computed tomographic evaluation].

Fifty cases of epidermoid carcinoma of the thoracic oesophagus were examined by computed tomography (CT). The tumor was located in the lower third of the oesophagus in 15 cases, in the middle third in 24 cases and in the upper third in 11 cases. More than two-thirds of these patients underwent complete surgical exploration. CT provided valuable information on the size of the tumor, its extension to the mediastinum and the presence of distal metastases, but lymph node involvement often remained undetected. Nevertheless, the resectability of the lesions could be predicted in a fairly large number of cases. A new CT classification based on the TNM system is suggested. According to the criteria used, this new system significantly improves staging, with results that are close to the pTNM classification of the American Joint Committee on cancer.

[Neonatal purulent pericarditis. Value of echocardiography and surgical drainage].

Five neonates are described who developed purulent pericarditis during a septicaemia which suggest that this is not a rare complication. The diagnosis is difficult as classical clinical, ECG and radiological signs are usually absent. Rapid enlargement of the cardiac silhouette or just cardiomegaly in the presence of a severe infection are the usual modes of presentation. Echocardiography is the easiest and safest way to confirm the pericardial effusion. Surgical drainage is essential.

[Surgical treatment of cancer of the cardia].

Adeno-carcinoma of the cardia corresponds to an accurate definition. During a period of 20 years we have operated on 114 such tumours, and resected 88 of them, 44 by esophageal and proximal gastric resection (group I), and 44 by esophageal and total gastric resection (group II). The resection rate was 77%. Hospital mortality was 20 p. cent in group I and 27 p. cent in group II. There were 1 T1, 8 T2 and 77 T3 (on 86 specimens); 14 p. cent of tumour invasions were observed on the esophageal section (18 p. cent in group I, 9 p. cent in group II); 90 p. cent had lymphatic nodes invasion. Five year's survival was nil in group I, 5 patients survived in group II. Five year's cure can be obtained only in stages I et II (NO MO or NI MO) with total gastrectomy associated with omentectomy, lymphatic curettage of the three coeliac chains, resection of 8 cm of esophagus, and mediastinal lymphatic curettage. When the condition of the patient is poor, or for more evolved stages, proximal gastric resection associated with terminal esophagectomy is the best palliative treatment.
