Reporting cerebrospinal fluid data: knowledge base and interpretation software.

The compilation of cerebrospinal fluid (CSF) patient data together with a graphic display of immunoglobulin patterns in a single CSF report has two main advantages: analytical and clinical plausibility control of a complex set of data improves quality assessment and allows improved clinical specificity and sensitivity for recognition of disease-related "typical" data patterns. The widespread use of automated on-line evaluation programs can now be combined with knowledge-based programs for interpretation by clinical chemists and neurologists. These programs are based on knowledge of neuroimmunology, blood-CSF barrier function and dysfunction, influence of CSF flow on concentrations of blood-derived and brain-derived proteins in CSF, specific intrathecal antibody synthesis and relevance of brain proteins for differential diagnosis of degenerative diseases. The relevance of hyperbolic discrimination functions in quotient diagrams for the detection of intrathecal immunoglobulin synthesis is compared with earlier, still frequently used, linear interpretation functions. Differences found in commercially available interpretation software are discussed.

Knowledge-based test result in interpretation in laboratory medicine.

The medical interpretation of laboratory test results aims for an optimization of the diagnostic decision process to improve the quality of medical care. It is of growing importance in Clinical Chemistry and Laboratory Medicine. Interpretation is an active process that depends on patient history, experience of the interpreter and local disease prevalences. For the field of Laboratory Medicine the tool Pro.M.D. was developed that enables the medical expert to formulate his knowledge and experience in a knowledge-based system. Nine Pro.M.D. systems are in routine use for knowledge-based test result interpretation at several hospital laboratories today. Our experience in providing clinicians with interpretive reports shows that a medical dialogue on individual findings can be established or improved. With the development of a new, highly differentiated and extensive knowledge base our group introduced a concept called NOTABENE as the basis for new maintenance tools.

[The Internet as a communication medium in veterinary medicine].

Medicine is based on expert experience and expert knowledge. One of the newest means to exchange up-to-date information is the Internet, the so-called information highway. Among academic institutions in Germany, transferring information over this world-wide communication network has become increasingly important, particularly concerning national and international cooperations in science and education. The acceptance has improved with new user-friendly graphical computer applications. The internet is regarded as a milestone on the way to the information society. In this report, we will present an overview of resources and services relevant to veterinary medicine available in the international data-network free of charge.

Terminological reference of a knowledge-based system: the data dictionary.

The development of open and integrated knowledge bases makes new demands on the definition of the used terminology. The definition should be realized in a data dictionary separated from the knowledge base. Within the works done at a reference model of medical knowledge, a data dictionary has been developed and used in different applications: a term definition shell, a documentation tool and a knowledge base. The data dictionary includes that part of terminology, which is largely independent of a certain knowledge model. For that reason, the data dictionary can be used as a basis for integrating knowledge bases into information systems, for knowledge sharing and reuse and for modular development of knowledge-based systems.
