Oblique resection of the lateral end of the clavicle and transfer of the coracoacromial ligament into the medullary cavity of the clavicle according to Weaver and Dunn had been performed in 12 fresh and 5 old type 3 separations of the AC-joint. At follow-up 1--5 years after the operation all patients had stable and painless shoulders. Heavy work and competitive sports had been resumed. The functional and cosmetic results were excellent. Stress radiographs showed good alignment and stability. The muscle strength, recorded with an isokinetic technique, was normal in the shoulder and slightly lowered in the elbow.
During the period October 1975-June 1977, 70 Marmor knee arthroplasties were performed. The operative procedure provided a good opportunity to study the anatomical conditions in vivo. The causes of extension deficit and lateral instability of the knees were analysed in a prospective investigation. Thirty-seven out of 69 knees had an extension deficit exceeding 10 degrees preoperatively. At operation a bony impediment to extension was found in 32 of these 37 knees, and by removal of this obstruction the extension deficit was relieved in 31 of the knees. At radiography a bony impediment was demonstrated preoperatively as a cause of an extension deficit in 29 of the 32 knees. No false positive diagnosis was made radiographyically. Varus or valgus instability exceeding 5 degrees was observed preoperatively in 42 of the the 70 knees. Lateral stability was achieved in all knees at operation by compensating for the intra-articular loss of cartilage and bone with the Marmor module system. Macroscopically intact collateral ligaments were found in all knee joints even in those severely affected by rheumatoid arthritis. All knees except three showed lateral stability 1 year postoperatively.
Ender's method of intramedullary nailing of pertrochanteric fractures of the femur has continued to gain favour in Sweden. 500 cases, out of a total 1000 fractures of the neck and proximal end of the femur, treated during the five year period 1973-1977, were compared to the results obtained by treatment with McLaughlin's nail plate. Ender's method is simpler, easy to learn and requires a shorter operating time. It gives minimal blood loss and infection is very rare. Our investigation showed that with increasing experience of the technique the duration of the operation could be much shorter, and that the problems associated with difficult fractures could also be overcome.
Three patients had hematogenous infections caused by the same organism (as determined by identical antibiotic sensitivity profiles) in more than one joint. In each of the patients, the same infecting organism had also been recovered from a previous distant infection. Evidence that sepsis resulted from late hematogenous contamination is overwhelming. All three cases clearly had at least some late total joint infection which was not from organisms introduced at the time of total hip arthroplasty but from hematogenous contamination from a distant site occurring at a late date unknown to the surgeon. Patients with a total joint arthroplasty should probably have systemic antibiotic coverage to protect the prosthetic joint against hematogenous contamination from distant sites of infection and from potential bacteremia induced during medical or dental procedures.
A young patient with tetraplegia resulting from an unstable and irreducible fracture dislocation of the lower cervical spine unexpectedly died from an intercurrent gastrointestinal hemorrhage. Following routine autopsy the whole, intact cervical spine was frozen in situ, tomographed in the lateral projection and sectioned through on a heavy duty cryomicrotome with use of a newly modified technique. Detailed analysis of 50 sections revealed, among other things, that a fragment from the lacerated intervertebral disc had become impinged between the body of a vertebra and the spinal cord constituting the hindrance to reduction. No obstacle to reduction was seen at the level of the facet joints.

In 15 patients, mean age 60 years, undergoing knee arthroplasty under lumbar epidural analgesia, changes in ECG, blood pressure, blood gases, serum potassium, acid-base status and serum creatine-phosphokinase were studied following release of a tourniquet applied for 70-135 min. During and after the ischaemia, the skin temperature of the leg operated on was monitored. Although dextran-70 was rapidly infused following tourniquet release, the blood pressure decreased significantly, due to a reduction in peripheral resistance. There were no signs of myocardial or pulmonary disturbances. Serum potassium in arterial blood increased significantly and reached a peak 3 min after tourniquet release. There was a tendency to metabolic acidosis, most pronounced after 3 min and caused by local lactate production. The skin temperature of the ischaemic leg fell progressively, which, combined with muscular relaxation, indicates a low metabolic rate. There was no evidence of local ischaemic muscular injury. It is concluded that the systemic changes after interruption of up to 2 h of tourniquet ischaemia are moderate and reversible, even in the elderly.
This paper presents four patients with polycentric artificial knee joints in whom reoperation revealed new menisci interposed between the artificial joint surfaces. One of the menisci had a bucket handle tear and was excised. Histological examination showed a fibrocartilage structure with the appearance of a newly formed meniscus. The artificial knees functioned well and the reoperations were performed because of trauma in one patient, because of femoro-patellar problems in one knee, and for conversion of a hemi-arthroplasty to an arthroplasty including all the compartments of the knee joint in two patients.

Low back pain is a common cause of inability to work. At the Rehabilitation Centre of the Uppsala County Council an annual number of more than 100 patients with this condition undergo an investigation aimed at evaluating their capacity for work and their need for rehabilitation. By compiling and analysing the information recorded in the case journals of the patients with low back disorders during the years 1969 to 1971, it was hoped that more knowledge would be gained about the factors constituting a hindrance to rehabilitation. The present paper is a preliminary report on an analysis of 47 case journals unsystematically selected from the entire material. The study verifies a number of previously reported observations, e.g. the low frequency of roentgenological changes. It is also noted that in a remarkably large number of patients no or only slight abnormalities were found on physical examination. Compared with the results of other investigations, there was a very low frequency of concurrent psychiatric disorders and abuse of alcohol. It was considered from this analysis that the information in the case journal material in question is adequate for elucidating the relevant problems in the planned larger investigation; further, it provides motivation for a future follow-up study.
High-molecular hyaluronic acid was injected repeatedly into normal knee joints of adult rabbits. Histologically the articular cartilage was not affected by this treatment. The content of chondroitin sulphate, assessed by a histochemical method, was not altered and neither was the water content of the hyaline articular cartilage. On the other hand, in the synovial membrane of joints treated with hyaluronic acid a diffuse infiltration of plasma cells and lymphocytes was observed, indicating that the hyaluronic acid administered exerts its action via the synovial membrane.
A case report is given on a woman with rheumatoid arthritis and deformed knee joints. She got a knee arthroplasty with a metal-to-metal Vitallium endoprosthesis ad modum Walldius. About one year after the arthroplasty she showed both a sterile fistulation from the knee joint and an eczema of the skin of the operated knee. An epicutaneous test for cobalt was positive. It is suggested that the metal-to-metal contact in the prosthesis and the proximity of the prosthesis to the skin is a possible explanation to the allergic reaction.
