[Serum erythropoietin concentration in women with uterine or ovarian tumors].

Patients with uterine or ovarian tumors frequently develop anaemia. Causes of anaemia in these patients are still not fully understood. We assessed serum erythropoietin (EPO) concentration in 70 women with benign or malignant uterine or ovarian tumors and in 43 control women. Thirteen women out of 50 with benign and 7 out of 20 with malignant tumors (26% and 35% respectively) were anaemic. In patients with benign tumors serum EPO concentrations did not differ from that in control subjects. In patients with malignant tumors plasma EPO was inappropriately low with respect to the haemoglobin concentration. From results obtained in this study it seems, that uterine or ovarian malignancy exerts a suppressive effect on EPO secretion. Inappropriately low EPO plasma concentration may account for the anaemia frequently occurring in these women.

[Levels of 1,25-dihydroxyvitamin D3 concentration in renal venous blood serum of patients with renovascular hypertension caused by unilateral renal artery stenosis].

UNLABELLED: Functionally significant, unilateral renal artery stenosis is characterized by reduced renal perfusion and lateralization of both excretory and endocrine function. The present study aimed to assess the influence of unilateral renal artery stenosis on plasma 1,25-dihydroxyvitamin D3 level. In sixteen patients (10 males and 6 females) with unilateral renovascular hypertension blood samples for estimation of plasma renin activity (PRA) and 1,25-dihydroxyvitamin D3 concentration were withdrawn from the renal vein of the ischaemic and normal kidney, from the femoral artery and from the vena cava inferior distally to the orifices of renal veins. Significantly elevated PRA (20.1 +/- 4.52 ng/ml/h vs 6.0 +/- 1.76 ng/ml/h, p < 0.005) but significantly reduced level of 1,25-dihydroxyvitamin D3 (99.25 +/- 4.8 pmol/l vs 115.75 +/- 6.7 pmol/l, p < 0.05) were found in renal vein blood of the ischaemic kidney as compared with the respective values of the normal kidney. No significant correlation was found between PRA and 1.25-dihydroxyvitamin D3 irrespective of the site of blood withdrawal. CONCLUSION: Renal ischaemia seems to exert a suppressive effect on 1,25-dihydroxyvitamin D3 synthesis by the kidney.

Concentrations of leptin and neuropeptide Y in maternal plasma, umbilical cord blood and in amniotic fluid in pregnant women with EPH-gestosis.

Leptin is presumed to be related to body mass index (BMI) and body fat stores and is involved together with neuropeptid Y (NPY) in the regulation of appetite. As pregnancy is accompanied both by changes of BMI and appetite, performance of studies presented in this paper were fully justified. In 43 healthy pregnant women and in 18 pregnant women with mild or moderate EPH-gestosis, concentrations of leptin and NPY were estimated in maternal venous blood, umbilical cord blood and in amniotic fluid. The control group consisted of 26 healthy nonpregnant women. Healthy nonpregnant women showed a BMI of 23.08+/-0.65 kg/m2 which was significantly lower than in healthy pregnant women (26.9+/-0.4 kg/m2, p < 0.001) and in women with EPH-gestosis (29.7+/-0.9, p < 0.0001). Also in healthy pregnant women the BMI was significantly lower than in EPH-gestosis subjects (p < 0.001). In healthy nonpregnant women plasma leptin levels were significantly lower than in healthy pregnant and EPH-gestosis women (10.9+/-1.68 vs 14.99+/-1.28 vs 21.89+/-2.58 ng/ml, respectively). In umbilical cord blood plasma leptin levels were significantly lower than in maternal blood only in healthy pregnant women (7.37+/-0.69 vs 14.99+/-1.28 ng/ml) but not in EPH-gestosis subjects (18.06+/-3.38 vs 21.89+/-2.58 ng/ml). Leptin levels in amniotic fluid were significantly lower than in umbilical cord blood both in healthy pregnant women (2.25+/-0.20 vs 7.37+/-0.69 ng/ml) and EPH-gestosis women (6.58+/-2.62 vs 18.06+/-3.38 ng/ml). In EPH-gestosis women leptin levels were significantly higher than in healthy pregnant women in maternal blood (21.89+/-2.58 vs 14.99+/-1.28 ng/ml), umbilical cord blood (18.06+/-3.38 vs 7.37+/-0.69 ng/ml) and amniotic fluid (6.58+/-2.62 vs 2.25+/-0.2 ng/ml). In both examined pregnant groups plasma NPY levels were nonsignificantly lower in healthy pregnant and EPH-gestosis women (42.28+/-4.09 and 43.68+/-8.45 pg/ml, respectively) than in nonpregnant women 50.65+/-6.13 pg/ml). In normal pregnancy a significantly higher NPY level was found in umbilical cord blood as compared with respective values in EPH-gestosis women (116.28+/-17.0 vs 49.65+/-7.01 pg/ml). Finally in both examined groups of pregnant women the amniotic fluid NPY level was of similar magnitude (13.85+/-1.52 and 13.89+/-2.46 pg/ml in healthy pregnant and EPH-gestosis women respectively). No significant correlation was found between fetal birth weight and cord-serum leptin and NPY levels respectively.

Markers of bone turnover in patients with nephrolithiasis.

A total of 19 patients with active nephrolithiasis, 14 patients with non-active nephrolithiasis and 17 healthy subjects were examined under standardized intake of calcium, phosphorus, purine and protein. In patients with both active and non-active renal stone disease the following abnormalities were found: elevated plasma levels of PTH and osteocalcin, increased activity of the bone isozyme of alkaline phosphatase, low plasma levels of phosphate and increased urinary excretion of calcium and oxalic acid. These abnormalities were more marked in patients with active than non-active nephrolithiasis. No correlation was found between plasma PTH levels and parameters of bone turnover as well as calciuria and oxaluria. Results presented in this paper suggest that (a) Smith's criteria of active renal stone disease are of minor pathogenetic and therapeutic value and (b) patients with active nephrolithiasis differ from non-active renal stone formers by more elevated oxaluria and markers of bone turnover and more marked abnormalities in calcium-phosphate metabolism related parameters.

Influence of water immersion on plasma erythropoietin concentration in patients with essential hypertension.

UNLABELLED: Water immersion (WI) is followed by hypoxemia and hemodilution, and induces alterations in renal hemodynamics (increase in tubular sodium load). These facts justified the performance of studies which aimed to assess the influence of WI on erythropoietin (EPO) secretion. Serum EPO and atrial natriuretic peptide (ANP) concentrations as well as plasma renin activity (PRA) were estimated in 18 patients with essential hypertension (EH) and in 9 healthy subjects (HS): before, after 2 h of WI and 2 h after discontinued WI. WI was followed by a significant increase in plasma volume and decrease in PRA, which were of similar magnitude in both examined groups. Patients with EH showed significantly higher basal levels of serum EPO (66.7+/-11.4 mU/ml in EH vs. 20.0+/-3.4 mU/ml in HS) and ANP (110.6+/-15.4 pg/ml in EH vs. 75.6+/-8.2 pg/ml in HS). WI was followed by a significant increase in both EPO (by 34.0+/-8.9 mU/ml in EH and 17.0+/-5.4 mU/ml in HS) and ANP (by 106.9+/-19.2 pg/ml in EH and 149.4+/-16.9 pg/ml in HS). Only in EH, a significant correlation was found between serum EPO level and mean arterial pressure post WI and natriuresis during WI, respectively. CONCLUSIONS: (1) water immersion induced increase in plasma EPO both in healthy subjects and patients with essential hypertension. (2) Patients with EH are characterized by elevated basal plasma levels of ANP and EPO. (3) Participation of the renin-angiotensin system and ANP in the regulation of EPO secretion could not be proven both in normotensive subjects and hypertensive patients.

Influence of acetate and bicarbonate hemodialysis on the plasma erythropoietin concentration in patients with chronic renal failure.

The present study aimed to assess the influence of acetate (AC) and bicarbonate (BI) hemodialysis with a cuprophane membrane on plasma erythropoietin (EPO) levels in 30 patients with chronic renal failure (CRF). Fifteen patients were severely anemic with an Hct below 30%, while 13 were moderately anemic with an Hct equal to or more than 30%. None had received rHuEPO or ACE inhibitors before the study. Blood samples for EPO and pO2 estimation were withdrawn before and after 1, 2 and 5 h of AC dialysis. In 11 of the patients with an Hct < 30% the same protocol was repeated during BI dialysis. In spite of a significant decrease in pO2 during the first hour of AC dialysis, the plasma EPO concentration decreased significantly in both severely and moderately anemic CRF patients from 35.4 +/- 2.6 to 23.6 +/- 3.5 mU/ml after 5 h of hemodialysis (p = 0.0001 by paired t-test) and from 39.5 +/- 8.6 to 27.5 +/- 8.9 mU/ml (p < 0.01) respectively. In contrast to AC dialysis, Bi dialysis failed to change the plasma EPO concentration (from 32.3 +/- 4.1 to 30.3 +/- 4.7 mU/ml after 5 h of hemodialysis). Conclusions AC but not BI hemodialysis shows a significant suppressive effect on plasma EPO levels in anemic CRF patients in spite of a significant decline in pO2. Factors other than pO2 seem to be involved in the regulation of EPO secretion in CRF patients during AC dialysis.

[Level of vasopressin in renal venous blood of patients with renovascular hypertension due to unilateral stenosis of renal arteries].

UNLABELLED: Assessment of plasma renin activity (PRA) in renal vein blood is used in the diagnosis of unilateral renovascular hypertension (URVH). Recently also other markers of renal ischaemia (atrial natriuretic peptide, adrenalin, nonadrenaline and dopamine) have been described. The present study aimed to assess renal handling of vasopressin (AVP) by the ischaemic kidney in patients with URVH. In 16 patients with URVH, PRA and AVP were estimated in renal vein blood of the ischemic (IK) and non-ischemic kidney (NK), in arterial blood (A) and in blood samples withdrawn from the inferior vena cava (VCI) below the orifices of the renal veins. In contrast to PRA no significant difference between plasma levels of AVP in renal vein blood of the ischaemic and non-ischaemic kidney was noticed (4.8 +/- 0.9 pg/ml vs 5.1 +/- 0.8 pg/ml respectively). CONCLUSION: Chronic hypoperfusion of the kidney does not influence renal handling of AVP in patients with URVH. Thus assessment of AVP in renal vein blood in these patients is deprived of diagnostic value.

[Secretion of atrial natriuretic peptide in patients with active renal stone disease].

The role of the atrial natriuretic peptide (ANP) in Na and water metabolism is well recognized. Much less known is the physiological importance of ANP in the metabolism of other electrolytes e.g. calcium and magnesium, which are presumably involved in the pathogenesis of active renal stone disease (ARSD). The present study aimed to assess the potential role of ANP in the pathogenesis of ARSD. Two groups of subjects were examined. The first one comprised 30 patients with ARSD (diagnosed according to Smith's criteria) while the second one consisted of 21 healthy subjects. Both groups were studied under bed rest (BR) and water immersion (IW) conditions. The examined groups were not different by age, sex, serum electrolyte profile (Na, Ca, Mg) and urinary excretion of Na, Ca, Mg and oxalic acid. Patients with ARSD showed significantly higher basal level of ANP and a significantly higher response of ANP secretion to IW as normals. In spite of this abnormality, patients with ARSD showed a similar increase in water, Na, Ca, Mg and oxalic acid excretion stimulated by IW as compared with normals. In contrast to healthy subjects, patients with ARSD showed no significant correlation between serum ANP levels and urinary excretion of Na, Ca and Mg. In addition, only patients with ARSD showed a significant positive correlation between serum ANP and urinary excretion of oxalic acid during WI. Results obtained in this study suggest, that ANP may be involved in the pathogenesis of ARSD.

Does industrial environment influence the prevalence of arterial hypertension, plasma cholesterol and uric acid concentration and activity of the renin-aldosterone system?

The present study aimed to assess the influence of a highly industrialized environment on the prevalence of arterial hypertension and plasma levels of cholesterol, uric acid, aldosterone and plasma renin activity. Epidemiological studies were performed in 5008 inhabitants of a highly industrialized city (Piekary Slaskie) and in 5004 inhabitants of non industrialized city (Olesno Slaskie) in the south part of Poland. Only subjects over 15 years were enrolled. The prevalence of arterial hypertension in the industrial population was 17% while in the country city 8.2%. Significantly higher serum cholesterol levels were found in hypertensive and normotensive subjects of the highly industrialized region than in the non industrialized one. Finally in hypertensive women aged < 40 years and normotensive men and women of the industrialized region significantly higher plasma renin activity and plasma aldosterone concentration were found than in corresponding population groups of the non industrialized region. From the results obtained in this study it seems, that the highly industrialized environment seems to stimulate hypertensinogenic factors.

[Life threatening hypercalcemia in a patient with chronic renal failure. Case report].

In the present paper a 67 years old female with chronic renal failure is reported, who developed at least three episodes of live-threatening hypercalcemia during long-term therapy with calcium carbonate. Results of biochemical and hormonal parameters and of dual x-ray photon absorptiometry (DEXA) were indicative for the presence of adynamic bone disease. Results obtained in this patient and published in the literature suggest, that presence of subnormal plasma concentrations of intact parathyroid hormone seems to be a valuable predictor of hypercalcemia in uremic patients treated with calcium carbonate.

[Nephrotoxicity of contrast media--etiopathogenesis and methods of prevention].

Contrast-induced nephropathy remains one of the most serious adverse event of contrast imaging. Patients with existing renal impairment, with or without diabetes, those with congestive heart failure or myeloma, and patients who receive nonsteroidal antiinflammatory drugs may be at higher risk. The pathogenesis of contrast-induced nephrotoxicity probably involves renal ischemic injury, tubular obstruction and direct tubular toxicity. Some prophylactic procedures based on these pathogenetic hypotheses have been proposed. Up-to-date knowledge on incidence, pathogenesis, risk factors, preventive procedures and therapeutic approaches of radiocontrast nephropathy is reviewed.
