Influence of ultraviolet irradiation on plasma vitamin D and calcitonin levels in humans.

In 13 patients with essential hypertension (EH) and in 10 normotensive controls, the influence of ultraviolet irradiation on plasma calcium, phosphorus, 25-OH-D, 1,25(OH)2D and calcitonin (CT) was studied. The basal levels of total calcium (2.47 +/- 0.02 mmol/liter) and phosphorus (1.10 +/- 0.04 mmol/liter) in patients with essential hypertension were not different from the control subjects (2.49 +/- 0.05 mmol/liter and 1.22 +/- 0.06 mmol/liter, respectively). However, patients with essential hypertension had elevated levels of 25-OH-D (68.09 +/- 7.48 vs. 26.51 +/- 3.3 mg/ml), 1,25(OH)2D (175.15 +/- 32.5 pmol/liter vs. 118.0 +/- 13.23 pmol/liter) and CT (131.7 +/- 32.36 pg/ml vs. 49.0 +/- 18.62 pg/ml) than in control subjects. In contrast to normotensive subjects, the majority of hypertensive patients showed no rise of plasma 25-OH-D and 1,25(OH)2D in response to ultraviolet irradiation. The results of this study suggest involvement of abnormal vitamin D metabolism in the pathogenesis of hypertension, at least in some patients.

[Beta 2 microglobulin in the blood pf patients with chronic renal failure treated by hemodialysis and kidney transplantation].

Beta 2-microglobulin serum concentration was measured in 25 patients on chronic haemodialysis with end stage renal failure, in 52 patients with renal transplant and in 26 healthy control subjects. In hemodialysed patients with end stage renal failure the concentration of beta 2-microglobulin was 15-20 times, and in patients with renal transplant 3-4 times that of healthy controls. Elevated serum beta 2-microglobulin concentration in patients with renal transplant seems to depend not only on the kind of immunosuppression applied but also on other factors, such as, for instance, the functional efficiency of the transplanted kidney.

Urinary alanine-aminopeptidase (AAP) excretion in patients with urinary tract infection treated with ceftazidime (CAZ) or cefotaxime (CTX) plus tobramycin (TOB).

It is well known that some antibiotics are particularly nephrotoxic. In the present study we investigated kidney function and AAP excretion (as an index of nephrotoxicity) in patients with urinary tract infection treated with CAZ or CTX plus TOB. The main cause of infection was E. coli in both treated groups. After 10 days of treatment similar beneficial effects were stated in both groups. At the end of treatment urinary AAP excretion in both groups was higher than before treatment. Despite a significant increase in urinary AAP in only one patient of each group, the serum creatinine level rose by more than 45 mumol/l. Monitoring of urinary AAP may be useful in early detection of nephrotoxicity caused by antibiotics.

[Effect of water immersion on plasma renin activity, vasopressin and aldosterone level in diabetics].

In 83 diabetic patients (23 of them were insulin-dependent) and 34 healthy subjects the influence of water immersion (WI) for 2 hrs on plasma renin activity (PRA), plasma aldosterone and vasopressin (AVP) level was examined. In both examined groups WI exerted a suppressive effect on PRA, plasma aldosterone and AVP level. In this respect only quantitative but not qualitative differences between diabetics and normals were observed. Presence of moderately advanced diabetic nephropathy and autonomic neuropathy influenced only slightly WI induced alterations of the renin-aldosterone system and AVP secretion. In all diabetic patients a defective volumetric mechanism of both the renin-aldosterone system and AVP secretion was stated. In addition in diabetic patients with late diabetic complications a defective osmotic mechanism of AVP secretion was observed. These findings suggest participation also of factors other than hypervolemia and decrease of the plasma osmolality in the mechanism of the observed WI induced suppression of the renin-angiotensin system and AVP secretion in diabetic patients.
