'I feel a fraud': men and their experiences of acute admission following chest pain.

The meaning of acute chest pain to men and how their masculinity affects the way that men respond to the care provided have received little attention in nursing research or nursing literature. The data for this paper were collected through participant observation whilst clinically working with men who had been admitted with acute chest pain. This paper will present a key theme that emerged from this study, relating to the dissonance that the men experienced when the initial pain that brought them to the hospital was quickly removed leaving them no longer feeling 'ill'. This paper will also argue that there is a need to consider the theories on masculinity to help in interpreting the reactions of the male patient to their condition and also to help direct the care that is being provided.

Training medical students in evidence-based medicine: a community campus approach.

BACKGROUND AND OBJECTIVES: Clinicians need skills in critical appraisal of medical literature to improve quality of care. This report on evidence-based medicine (EBM) curricula describes 1) the role of family medicine educators, 2) timing, 3) value of a standard format across multiple communities, and 4) outcomes in attitudes and skills. METHODS: In 1992, a nine-session curriculum delivered across six community campuses was introduced during the third year of medical school in the College of Human Medicine at Michigan State University. Evaluation compared 1) responses on the Association of American Medical Colleges graduation questionnaires from classes who received the curriculum with the 1994 class who did not (424 students), 2) responses to questions on group process performance, and 3) focused surveys of two classes. RESULTS: Trained classes reported higher levels of confidence in critical appraisal and research skills than the 1994 class and other schools. Respondents reported the small-group process as effective, greater appreciation of the training after 1 year of residency than at graduation, and no change in research activity. CONCLUSIONS: Family medicine educators can lead a new curriculum in EBM and maintain consistent standards across multiple communities. Many questions remain concerning the ideal curricular design to help clinicians apply the best research to patient care.

Strategies for reduction of noise levels in ICUs.

This paper reports on a study of noise levels in an intensive care unit. The various noise sources and their sound levels were identified. Sources of noise were classified into those generated by: people, equipment and room furniture. Strategies for the reduction of noise to levels likely to be less disturbing to patients are discussed.

Laparoscopic colorectal resection: initial experience in a specialist unit.

The adoption of the laparoscopic approach to colorectal resection has been slow amongst colorectal surgeons principally due to concerns regarding oncological safety. Recent randomized controlled trials have confirmed both the safe and some advantage of this procedure have been performing laparoscopic assisted colorectal resection since 2002 and have now performed over 100 cases on non consecutive and selected patients. We have reviewed our experience with the introduction of this technique. 61 patients were operated on for cancer and 39 for benign disease mainly Crohn's and diverticular disease. Operative time was a median of 128 minutes over the course of study. Conversion rate was 5%. Pathological analysis of the resected specimens in the cancer cases revealed adequate lymph node harvest and margins. No patient had a positive margin and no port site metastasis have been seen. Duration of ileus and length of stay were a median of 0 and 6 days. Post operative morbidity and mortality were comparable to open colorectal surgery with the exception of port site herniation which occurred in 4% of patients. This study suggests that a laparoscopic approach to colorectal resection can be successfully introduced in an Irish hospital setting. The challenge facing Irish surgery is to disseminate this technique in a controlled and safe manner for Irish patients.

The hemodynamic effects of halothane in anemic dogs.

Hemodynamic dose-response curves and MAC, apneic and lethal concentrations for halothane were determined in two groups of dogs at a normal hematocrit (Hct) (39 +/- 6), and during acute anemia (n = 4) and subacute anemia (n = 4), with a combined Hct of 18 +/- 6. The ratio of subendocardial/subepicardial blood flow was determined by the injection of radioactive microspheres. MAC, apneic, and lethal concentrations of halothane were not significantly different in anemic animals. There were also no differences in the halothane dose-response curves plotted for cardiac output, blood pressure, heart rate, left ventricular end-diastolic pressure, or arterial base excess. In 32 microsphere injections with Hct as low as 13, no dog had redistribution of myocardial blood flow suggestive of subendocardial ischemia. We conclude that halothane is a safe anesthetic during severe anemia.
