No effect from double-blind trial of physostigmine and lecithin in Alzheimer disease.

Eight patients with clinically diagnosed mild to severe Alzheimer disease were treated during two six-week periods by a double-blind, single-crossover protocol with placebo or a combination of 18 gm of lecithin and 3 to 10 mg of oral physostigmine daily. No improvement in behavior, recent memory, or other neuropsychological functions occurred.

Familial chromosome translocation t(3;18)(p21;p11).

A familial translocation t(3;18)(p21;p11) was observed in a newborn male. He had multiple malformations resulting from partial trisomy 3 and partial monosomy 18. The mother, maternal uncle, and maternal grandmother were found to be balanced translocation carriers. A daughter of the maternal uncle with similar malformations probably had the same unbalanced karyotype as the proband.

A family with whistling-face-syndrome.

A family with 7 persons affected with whistling-face-syndrome in 3 successive generations is described. The clinical variability of the syndrome within one family is demonstrated by means of examining particular characteristics of four daughters in the family. Six affected persons are female; the only male carrier died in early infancy because of the severity of symptoms. The known autosomal dominant inheritance combined with clinical variability and bias towards female manifestation is present in this family.

The origin of fasciculations in motoneuron disease.

The collision technique was used to determine the origin of distal fasciculations in 25 motor units from 9 patients with amyotrophic lateral sclerosis and 6 with other diseases involving motoneurons. Fasciculations originated in the nerve proximal to the knee or elbow in 15 and distally in 2; multiple sites of origin were found in 8 motor units. The origin was not related to the presence or absence of electromyographic evidence for collateral sprouting or lesions in descending suprasegmental pathways.

[Working time of bone cements].

The period of applicability during polymerisation of four well introduced bone cements (Sulfix-6, CMW, Simplex, Palacos) was measured by a subjective method in function of room respectively conditioning temperature. Simultaneously temperature in the cement and mechanical admittance (transfer of oscillation of 750 Hz) were registered. The duration of working-up period and plymerisation period depends considerably from the room temperature. An increase of 10 degree C accelerates polymerisation by a factor of 1.5 to 2.0. The working time of the products differs widely. Increased room temperature means also increasing maximum polymerisation temperature. Mechanical admittance is an interesting measure for the objectivation of the working period. Discussing new ISO standards a proposal is made for more accurate determination of the end of the working-up period (set time).

Experiences with functional insulin substitution: a follow-up study on control and patient compliance.

Functional insulin substitution, an insulin regimen made up of two daily injections of intermediate-acting insulin and prandial boluses of regular insulin, was to be introduced in 49 type 1 diabetic patients, as previous regimens consisting of two or three daily injections proved to be inefficient due to the patients lifestyle or inherent metabolic lability. Forty-five patients were treated with human insulin injected by NovoPen. In 38 cases therapy was changed in the frame of a one-week, small-group, inpatient, structured educational course. After a mean 14 months of follow-up metabolic status improved in 33 cases while there was further derangement in 16. Eighteen patients were practising true functional therapy, i.e. doing blood glucose tests before each injection. Further 22 diabetics were trying to achieve better metabolic control through 2-3 daily blood glucose tests and insulin dose corrections. The metabolic status was not affected by the frequency of blood glucose testing, rather by raising of the daily dose of short acting insulin in conjunction with the switch in therapy acting beneficially. All patients insisted on using NovoPen further on.
