[Cerebral toxoplasmosis. Neuroradiologic diagnosis and prognostic monitoring].

Cerebral toxoplasmosis remains the most frequent etiology of cerebral masses in AIDS. In most cases, the disclosure of multiple enhanced masses is suggestive of diagnosis of toxoplasmosis and leads to undertake presumptive therapy. Sometimes, the pattern is less suggestive, and the possibility of primary cerebral lymphoma (PCL) is a diagnostic dilemma, because this is a short term life-threatening lesion in the absence of an undelayed fitting therapy. However, apart from the periventricular form of PCL, where directly proceeding to biopsy should be suggested, there is no distinctive pattern of PCL. The most reliable features in distinguishing between PCL and toxoplasmosis are: solitary lesion, homogeneous enhancement of a lesion whose diameter is larger than 2 cm, situation in deep periventricular white matter and limited edema and mass effect. The presence of haemorrhagic foci in the lesion, if there is no steroid therapy is rather indicative of toxoplasmosis. The lack of enhancement on enhanced CT-scan does not allow to eliminate cerebral toxoplasmosis, and indicate MR examination, or empiric antitoxoplasmic treatment. The radiologic follow-up depends on clinical response and on initial pattern. The criteria of response to antitoxoplasmic treatment are: the decrease in volume or number of lesions, the decrease of edematous reaction and mass effect (if no steroid was administered). If the lesion was initially equivocal, the lack of growth in a 8-day delay is a criterion for toxoplasmosis, just as the occurring of haemorrhage in the lesions (if there is no steroid-therapy).

Finite element analysis of the temperature field around two adjacent cryo-probes.

A finite element code was developed for the analysis of the temperature field around two adjacent cylindrical cryo-probes. The two-phase, two-dimensional Stefan problem is solved using a moving boundary approach and space-time finite elements. Solution of one-cryo-probe problem compared well with an existing analytic solution. The two-cryo-probes problem yielded reasonable results. The program simulated the nonsymmetric activation of two probes and the merging of the two freezing fronts in the case of symmetric activation.

Percutaneous aspiration biopsy in cervical spine lytic lesions. Indications and technique.

We describe the technique and the results of the percutaneous aspiration biopsy (PAB) in a series of 9 patients presenting with neck pain and different degrees of myelopathy, in whom the cervical spine X-ray demonstrated lytic lesions of unknown origin. PAB is a useful, relatively safe technique, and leads to histological diagnosis between metastatic and inflammatory processes. Furthermore, in inflammatory lesions with negative hemoculture. PAB may help in detecting the micro-organism responsible and therefore allow a better antibiotic treatment.

[Angiolipoma of the central nervous system].

Angiolipomas of the central nervous system are rare tumors (37 cases in the literature) and are mostly located in the spine (95%), rarely in the skull. When they are intraspinal, these masses are extradural in more than 90% of the cases. Angiolipomas are benign tumors containing vascular and mature adipose elements. The clinical symptomatology is non specific, but computed tomography and mostly MRI provide a precise diagnosis. With 2 additional cases and a review of the literature, we define the main characteristics of these tumors.

[Sub-callus epidermoid cyst. A case report].

The cranio-encephalic epidermoid cysts are rare tumors. These lesions are extra-axial and can occur at the infratentorial or supratentorial level. The epidermoid cysts are classically in contact with the base of the skull, and less often intraventricular; the other locations are exceptional. Because of their CT aspect (hypodense lesion, rarely isodense, well defined, without contrast enhancement, and without surrounding edema) and MR aspect (in more than 70% of the cases: more or less homogeneous lesion, hypointense in T1 and hyperintense in T2), the authors believe that it is possible to suggest such a diagnosis even with an atypically located case. We report the case of an epidermoid cyst located between the corpus callosum and the falx cerebri.

Reactivation of the secretion from the prostatic gland in cases of reduced fertility. Biological study of seminal fluid modifications.

The efficiency of Pygenum africanum treatment was tested on a population where the acid phosphatase activity was shown to be reduced at two successive examinations. The treatment caused an increase in acid phosphatase activity and total protein secretion. However, the fact that the men who had IgA present in their ejaculate reacted more favorably to the treatment shows that Pygenum africanum is more efficient in cases where there is no prostate gland inflammation.

[Leydig cell tumours of the testis. Two cases presenting opposing sexual features : gynaecomastia, precocious virilisation, and revealed by study in vitro of aromatisation activity].

Two Leydig cell tumours presented with changes in sexual characteristics. The first patient was a child with precocious puberty whilst the second was that of an adult with recurrent gynaecomastia. Histochemical examination in vitro of tumour tissue from the second patient made it possible to explain the synthesis of oestrogens by a primary tissue destined to an essentially androgenic activity.

[Current considerations on male secretory sterility].

The problems of male sterility are considered in the light of fundamentals underlying the embryology and histophysiology of spermatogenesis and its hormonal control. Some forms of infertility are now understood but no treatment for them has yet been found. The mechanisms of other forms still elude firm definition: this is the case of, among others, some oligoasthenospermias in which, according to our experience, excessive estrogen production appears to play a physiopathologic role. In the prevailing uncertainty about therapeutic methods, the best short-term solutions in this field are in-vitro enhancement of the sperm's fecundating power and the improvement of artificial insemination techniques.

Endovascular treatment of a cervical paraspinal arteriovenous malformation via arterial and venous approaches.

We describe a cervical congenital paraspinal arteriovenous malformation (AVM) drained by paraspinal and epidural ectatic veins, which caused massive erosion of the C6 and C7 vertebral bodies, threatening the cervical stability and necessitating treatment. During the first session, six arterial embolizations were performed to reduce the size and the flow of the AVM. Two months later, a venous approach was used to occlude the remnant venous exit of the AVM and achieve a complete cure. All embolizations were performed using N-butylcyanoacrylate.

Delayed stroke secondary to increasing mass effect after endovascular treatment of a giant aneurysm by parent vessel occlusion.

A 47-year-old woman, who had lost vision in her left eye because of a giant left supraclinoid internal carotid artery aneurysm, was referred for endovascular treatment. Parent-vessel occlusion was performed to obtain circulatory exclusion of the aneurysm. Eight days after treatment, she became hemiparetic and dysphasic. Repeat angiography showed compression of the left middle cerebral artery by the swelling giant aneurysm. Preventive measures should be taken to avert worsening of mass effect when giant aneurysms become thrombotic.

Spinal angiolipomas: CT and MR aspects.

Spinal angiolipomas are rare benign tumors containing vascular and mature adipose elements. In greater than 90% of the cases they are located in the epidural space. The clinical symptomatology is nonspecific, but CT and particularly MR studies allow for a precise diagnosis. Computed tomography (four cases in the literature plus one explored in our department) showed a hypodense lesion in 80% of the cases. In one case the angiolipoma was isodense to the cord. Magnetic resonance (three cases in the literature plus two explored in our department) showed a more or less homogeneous mass with a signal in T1- and T2-weighted images close to that of the subcutaneous fat. The infusion of gadolinium (one case in the literature plus one of ours) is helpful as an indicator to the degree of vascularization of the angiolipoma.

Robertsonian heterozygosity and male sterility.

Meiotic and synaptonemal complex studies using electron microscopy were carried out on infertile man with a 13/14 translocation. Synaptonemal complex analysis showed a typical trivalent with incomplete pairing of the acrocentric elements. The sterilizing effect caused by the failure of synapsis is discussed. This pairing failure has a diagnostic value and a poor prognosis.

[Persistent value of intra-arterial fibrinolysis 8 hours or more following central retinal artery occlusion or of its branches].

PURPOSE: To establish that intra-arterial fibrinolysis of the ophthalmic artery can still be efficient 8 hours or more after a central retinal artery occlusion. MATERIAL AND METHODS: Nine to twenty hours (average 12.5 H) after a loss of vision due to a central retinal artery occlusion or the occlusion of its branches, 7 patients were treated by intra-arterial fibrinolysis. The vision of the affected eye was limited to light perception for four patients, to hand movement at 1 meter for two patients, and to 4/10 P2 for one patient. Under radiological guidance, 300.000 IU of urokinase was injected through a microcatheter placed in the ophthalmic artery (6 patients), or in the facial artery (1 patient); in this case the angiogram showed an occlusion of the internal carotid artery and a retrograde filling of the ophthalmic artery through the facial artery. RESULTS: We did not observed any complication. The 3 patients who had a vision better than light perception recovered a vision of 10/10 P2. For the others: two patients recovered a vision of 9/10 P2 and 5/10 P2, two did not recover. CONCLUSION: Fibrinolysis of the ophthalmic artery must ideally be performed as soon as possible, but a recovery is still possible more than 8 hours after the onset of the loss of vision. The time limit is not yet defined but after 24 hours, the chance of improvement seems to be very low.
