[The epizootiology of tuberculosis of cattle in the Federal Republic of Germany].

Mycobacterial strains from different outbreaks of tuberculosis of cattle in Germany from 1996 to 2001 were differentiated by two molecular biological methods (Spoligotyping, RFLP IS6110). The causative agent was in one case Mycobacterium (M.) africanum, in 10 cases M. bovis and in 17 cases M. bovis ssp. caprae, respectively. The results of the molecular biological methods are discussed from the perspective of epizootiology and the particular importance of infections by M. bovis ssp. caprae emphasized. Direct contact of the animals, purchase from infected stocks, infected zoo animals and wildlife, as well as livestock handlers are discussed as possible sources of infection.

[Immune complexes and peripheral neuropathy: about two cases (author's transl)].

Two female patients aged 64 and 54 were studied, affected by a progressive, distal sensori-motor mononeuritis multiplex. A nerve biopsy specimen from the first patient, suffering from chronic active hepatitis without detectable serum hepatitis B surface antigen (HBsAg) or antibody (anti-HBs), showed segmental demyelination and widespread inflammatory and necrotizing vasculitis and perivasculitis. The second patient, affected by a localized, caeseating tuberculous infection of the right kidney, refused to undergo a nerve biopsy. Serum levels of circulating immune complexes, detected by C1q BA binding method were strongly positive in both subjects; activation of the complement system (decreased serum level of C3 and/or C4) was also present. The course of the peripheral neuropathy was progressive and worsened in the first patient whose circulating immune complexes were always detectable. On the contrary the second patient showed improvement after the nephrectomy, which coincided with a quick disappearance of the circulating immune complexes. The findings may suggest in these two patients an immune complex mediated pathogenetic mechanism like the one recently accepted for the peripheral neuropathies in the course of chronic hepatitis B.

Stereology: a new quantitative morphological method to study epididymal function.

Stereology is a method to obtain quantitative information of structural changes at the light and electron microscopic level. The basic principles of stereology are outlined below. This paper shows how stereologic techniques can be used to obtain structural data from histologic and electron micrographs of intact tissue and cells. A stereologic model, which provides information on the structure of the epididymis has been developed for the rat epididymal head. The model consists of morphologically defined space and membrane compartments of the rat epididymal head and the principal cells. The alterations, induced in the principal cell of the epididymal head after long term hypophysectomy were studied by electron microscopy. The results presented are relative to a cubic centimeter of epididymal tissue, a cubic centimeter of principal cells and principal cell cytoplasm. An attempt to relate stereologic and biochemical data is shown.

Toxicological detection of ibuprofen and its metabolites in urine using gas chromatography-mass spectrometry (GC-MS).

Ibuprofen, (R,S)-2-(4-isobutylphenyl)propionic acid, is a non-opioid analgesic which is prescription-free available in pharmacies since 1989. Consequently, use and misuse of this analgesic strongly increased. For diagnosis of an abuse or an intoxication a GC-MS procedure was developed for the toxicological detection of ibuprofen in urine after extraction at pH 5 and methylation. For screening, mass chromatography with the masses m/z 220, 161 (methylated ibuprofen), 178, 119 (methylated hydroxy ibuprofen) and 264, 145 (methylated carboxy ibuprofen) was used to indicate the presence of these compounds in urine. The identity of the peaks underlying full mass spectra was confirmed by comparison with the reference spectra using computer library search. After ingestion of one single oral dose of 400 mg ibuprofen, the parent compound could be detected for 27 to 34 h, hydroxy ibuprofen for 34 to 40 h and carboxy ibuprofen for 5 to 6 d. Using this procedure ibuprofen which was taken even several days ago could be detected.

[Promotion of wool growth and body weight development of sheep by alimentary supplementation with thiocyanate].

Adult sheep were administered 12 mg thiocyanate (SCN-)/kg body weight orally twice a year in altogether 16 respectively 41 single doses under conventional keeping during the period of stabling (winter) in two consecutive years. Fattening lambs got an analogous SCN- dosage with altogether 4 respectively 34 single doses under intensive keeping. Aided by this temporarily obtained balance of an alimentary SCN- deficiency the following effects were reached during conventional keeping: Significant increase of the wool mass in the greasy state and a tendency to better breading performance. During intensive fattening an increase of the wool mass and of the staple cross-section was proven statistically in the mean for all the test animals, but for the increase of the staple length it could be shown only by comparison of twins.

Treatment of active and postactive ileal and colonic Crohn's disease with oral pH-modified-release budesonide. German Budesonide Study Group.

BACKGROUND/AIMS: Budesonide is a glucocorticoid with a high topical anti-inflammatory but low systemic activity due to its rapid hepatic inactivation. The aim of this open, multicenter study was to investigate efficacy and safety of oral pH-modified-release budesonide in patients with active Crohn's disease of the ileum and colon and in maintaining budesonide-induced remission in postactive Crohn's disease. MATERIALS AND METHODS: 81 patients (intention-to-treat) received 3 x 3 mg budesonide/day for 6 weeks, followed by 3 x 2 mg budesonide for another 6 weeks in case of response to initial treatment. Clinical and laboratory parameters were assessed at study entry as well as after 2, 4, 6 and 12 weeks of treatment. RESULTS: On an intention-to-treat basis remission was induced in 54.3% of 81 patients with active Crohn's disease, 71.4% of 35 patients stayed in remission after the acute-phase treatment until the end of the trial. Typical steroid-related side effects were observed during the acute-phase treatment in only 18% of the patients. Duration, severity and extent of disease at study entry played no significant role in the outcome of the trial, but there was a tendency towards better results during the acute-phase treatment in patients with moderate disease activity and affection of the terminal ileum and proximal colon. CONCLUSIONS: Budesonide could be an alternative to conventional steroid treatment in patients with active Crohn's disease.

Bowel obstruction in the postoperative period of laparoscopic inguinal hernia repair (TAPP): review of the literature.

A case is presented of a male patient that presented with intestinal obstruction in the early postoperative period of a transabdominal preperitoneal inguinal repair (TAPP) that was diagnosed and repaired successfully using laparoscopy. Whenever a total extraperitoneal procedure (TEP) cannot be performed, the peritoneal closure of the TAPP should be done water-tight using interrupted stitches of absorbable monofilament sutures.
