Pituitary response in the doubled GnRH-TRH test on the 5th day post partum or post abortum after ablactation by oral means of ethinylestradiol sulfonate.

The function and reactivity of the hypothalamo - anterior pituitary axis had been tested after primary ablactation with ethinylestradiol sulfonate (EES) by means of the doubled GnRH-TRH-test. The reaction of the hypothalamo-pituitary axis is, in early puerperium, drastically reduced by GnRH stimulation. The physiological hyperprolactinemia of puerperium leads to a non-responsiveness by means of exogenous GnRH. Simultaneously there exists a greater sensitivity of the hypothalamo-pituitary system for the negative and a relatively lesser sensitivity for the positive feedback of estrogens in puerperium.

The impact of Haemophilus influenzae type b meningitis on nutritional status.

We investigated the immediate impact and long-term effects of Haemophilus influenzae type b meningitis on nutritional status and growth in 111 children. Mean weight change during 10 d of hospitalization was a loss of less than 1%. Follow-up median weight-for-height percentiles increased after admission (p less than 0.01). Percentile values were as follows: admission, 45th; 1 mo, 60th; 3 mo, 60th; and 6 mo, 68th. Forty-three percent of the cases were greater than 75th percentile of weight-for-height at 6 mo after disease. An additional follow-up assessment of weight-for-height indicated that 43% of a representative sample subset of 49 were still obese 1.17-5.5 y after disease. Significant differences in median concentrations of serum prealbumin were found between days 1 (128 mg/L) and 5 (199 mg/L, p less than 0.0001) and days 5 and 10 (214 mg/L, p less than 0.02). Median erythrocyte glutathione reductase activity coefficients increased between days 1 (1.16) and 5 (1.20, p less than 0.01). The mean free erythrocyte protoporphyrin-heme ratio increased between days 5 (10.78 X 10(-6)) and 10 (14.22 X 10(-6), p less than 0.01). We conclude that there were transient adverse changes in nutritional status. Obesity appears to occur after disease.

Effect of coinfusion of cholic acid and sulfated cholic acid on bile formation in rats.

The effect of combined infusion of sulfated cholic acid and nonsulfated cholic acid on bile formation was investigated. The data show that the secretion of sulfated cholic acid is slower and does not share the same transport mechanism(s) as nonsulfated cholic acid. Sulfated cholic acid significantly increased bile flow and reduced the secretion of biliary phospholipids, cholesterol, and protein associated with the secretion of exogenous nonsulfated cholic acid, only when they were infused in a ratio higher than 2:1 (sulfated:nonsulfated). Thus it is concluded that sulfated bile acids may protect the liver against the toxic effect of high concentration of bile acids. The relevance of these findings to human cholestasis remains to be determined because sulfated bile acids do not predominate in serum during cholestasis.

[The occurrence of Clostridium difficile in fecal samples of dogs and cats].

Fecal samples of 150 dogs and 175 cats originating from different veterinary practices were investigated for assessing the occurrence of Clostridium (Cl.) difficile by using a selective medium for cultural isolation. From dogs without enteric symptoms 7 (9.3%) of 75 samples were positive for Cl. difficile, with 2 strains being cytotoxic for bovine embryonic lung fibroblast cells, which could be neutralized by Cl. difficile-antitoxin. In samples of 75 dogs with enteric symptoms Cl. difficile could be isolated in 2 cases (2.7%). In cats 9 (9%) of 100 fecal samples deriving from animals without enteric symptoms contained Cl. difficile, while in 75 cats with enteric symptoms, the isolation rate was 6.7% (5 strains). Of either group only 1 Cl. difficile-strain showed cytotoxicity for tissue culture. The results of this study allow to conclude, that in contrast to the significance for man Cl. difficile is neither for dogs nor for cats an important enteric agent. However these pets can harbour and shed strains of Cl. difficile, even cytotoxigenic ones, in faeces. In view of these findings the possibility of occasional human infections by household dogs or cats needs attention and further investigation.

Diagnosis of oral precancer and cancer.

The early diagnosis of oral cancer and precancer is important because oral cancer is usually well advanced at the time of diagnosis, having spread to regional lymph nodes in a substantial percentage of cases. The earlier the diagnosis is made, the better the prognosis. Several diagnostic methods that are used, such as stomatoscopy, toluidine blue staining, oral exfoliative cytology, histopathological examination and other more advanced diagnostic procedures, are discussed.

[Fluoride content of tea and amount ingested by children].

The fluoride concentration on their respective infusions was determined in the twelve most popular brand of tea which can be obtained in Valparaiso, Chile. Fluoride concentration was measured 5, 45 and 180 minutes after tea preparation, by using an specific fluoride electrode. The amount of ingestion of tea in children was estimated by means of a survey on 303 children from a sample of 7,690 boys and girls living in a fluoridated area. The data revealed a large variability in fluoride concentration depending on the brand of tea. Mean fluoride concentration was 2.36 mg F/l. at the five first minutes. Sixty eight per cent of children drinking tea as usual beverage, therefore it was estimated that about 22.1% of this sample have risk of dental fluorosis.

Meconium ileus and its equivalent as a risk factor for the development of cirrhosis: an autopsy study in cystic fibrosis.

Although dehydrated obstructing mucus is thought to account for the obstructive pathology involving the lungs, the pancreas, the reproductive system, and the intestinal tract, its relationship with CF-associated liver disease remains largely hypothetical and little is known about possible risk factors. Complete clinical and autopsy records were available in 38 of 73 deaths occurring over a 10-year period. The liver was normal in only five cases, and they were all infants. Steatosis was the only lesion present in 9, hypoxic liver disease was documented in 8, and biliary cirrhosis in 16 (focal in 10 and multilobular in 6). There was no relationship between the presence of cirrhosis, gallbladder abnormalities, age at death, and clinical status recorded during the year precoding their demise. Mucus plugs characterized by amorphous eosinophilic material within proliferated bile ductules were present in 75% of children with focal or multilobular biliary cirrhosis as opposed to 14% in those without (p = 0.015). A history of meconium ileus or its equivalent was recorded more frequently (p = 0.038) in those with cirrhosis. Finally, biliary cirrhosis was invariably present when there was a history of meconium ileus or its equivalent and when mucus plugs were noted. These findings suggest that patients with intestinal obstruction are at greater risk for the development of cirrhosis and that strategies should be developed to increase the detergent capacity of bile and its flow in order to decrease the viscosity of mucus in the biliary tree.

Primary signet-ring cell carcinoma of the prostate.

The occurrence of signet-ring cells within a mucinous prostatic carcinoma is an extremely rare finding. This cancer is characterized by both intracellular retention and extracellular secretion of mucinous material. We report a case of primary signet-ring cell carcinoma of the prostate that was confirmed by autopsy. The results of gross, microscopic, and immunohistochemical examinations are described.

Silica granuloma of a lymph node.

A case of silica granuloma in the submasseteric lymph node is presented. Sarcoidosis was excluded, as crystalline particles, with a double-refractile property, were seen in polarized light. The most probable cause was a delayed hypersensitivity reaction to silica from a lipstick cream introduced through a recurring angular cheilitis.

Contribution of the fluid phase endocytosis to bile flow in cholestasis and choleresis in rats.

Using a single iv injection of 1 mg Evans blue dye/100 g body wt in rats, the contribution of the fluid phase endocytosis to bile flow was estimated. In control rats, this pathway contributed 2-4% of the bile flow or 10-20 microliter/hr/100 g body wt. This contribution was not affected by cholestasis induced by taurolithocholic acid or bile duct obstruction or by choleresis induced by dehydrocholic acid. It is concluded that fluid phase endocytosis does not significantly influence bile formation but, rather, may be implicated in the remodeling of liver cell plasma membranes.
