Prognosis of patients with gastric cancer and pyloric stenosis: histological differentiation.

The clinicopathology of gastric cancer with pyloric stenosis was examined with special reference to histological differentiation. One hundred sixteen patients treated in Department of Surgery II, Kyushu University Hospital, were classified into differentiated (DT: n = 47) and undifferentiated types (UT: n = 69). The UT group was significantly younger than the DT group (P < 0.05). There were no differences in the incidence of serosal invasion, liver metastasis, lymph node metastasis, and the clinical stage between the groups. The incidence of peritoneal dissemination of the UT group was 41% (28/69), a value significantly higher than the 23% (11/47) for the DT group (P < 0.05). The 5-year survival rates of the DT and UT groups were 27% and 16%, respectively, with survival time for the UT group being significantly less than that of the DT group (P < 0.05). In patients with pyloric stenosis and an undifferentiated adenocarcinoma with the risk of peritoneal dissemination, intensive adjuvant chemotherapy seems essential to improve the prognosis.

Helix pomatia agglutinin binding activity and lymph node metastasis in patients with gastric cancer.

We made use of Helix pomatia agglutinin (HPA) staining to examine 218 gastric cancer tissues (163 primary tumors and 55 metastatic lymph nodes), using the avidin-biotin-peroxidase complex method. The positive HPA staining rate was 59% (96/163) for the primary tumors, and this positive staining correlated well (with statistical significance) with lymphatic invasion and metastasis (P < 0.01). In the metastatic lymph nodes, the majority of cancer cells (49/55:89%) stained positively for HPA. Tumors with positive HPA staining often metastasized to lymph nodes. Patients with gastric cancer and positive HPA staining of their tumor tissues lived for a shorter time than did those with a negative staining, even in the absence of metastasis to the lymph nodes (P < 0.01). Careful follow-up and aggressive therapy are required postoperatively for patients with an HPA-positive gastric cancer.

Serum lipids, lipoprotein lipids and coronary heart disease in patients with xanthelasma palpebrarum.

Serum lipids and lipoprotein lipids were studied in 53 patients (21 males and 32 females) with xanthelasma palpebrarum and 40 age-matched normal controls (20 males and 20 females). Patients were subdivided into patients with normolipidemia, hyperlipidemia or familial hypercholesterolemia (FH). In both male and female patients with hyperlipidemia or FH, the serum cholesterol (Chol) levels were significantly higher than in normal controls. In both male and female patients with normolipidemia or hyperlipidemia, the VLDL-Chol levels were significantly higher than in normal controls. Male patients with FH showed significantly higher levels of VLDL-Chol than normal controls. Both male and female patients with normolipidemia, hyperlipidemia or FH showed significantly higher levels of LDL-Chol, lower HDL-Chol levels and lower HDL-Chol/LDL-Chol ratios than normal controls. In both male and female patients with hyperlipidemia and in male patients with FH, the serum triglyceride (TG) levels were significantly higher than in normal controls. Both male and female hyperlipidemic patients showed significantly higher levels of VLDL-TG than normal controls. In male patients with FH, the VLDL-TG levels were significantly above the control levels. In male patients with normolipidermia, the LDL-TG levels were significantly higher than in normal controls. In both male and female patients with hyperlipidemia or FH, the LDL-TG levels were significantly higher than in normal controls. The HDL-TG levels in patients with normolipidemia (males) or FH (females) were significantly lower than in normal controls. The prevalence of coronary heart disease in patients with normolipidemia, hyperlipidemia or FH was 29.4%, 24.0% and 45.4%, respectively.

Successful treatment of a 93-year-old patient with hypoplastic acute monocytic leukemia using macrophage colony-stimulating factor.

A 93-year-old patient with hypoplastic acute monocytic leukemia (AMoL) achieved a complete remission after macrophage colony-stimulating factor (M-CSF) therapy. Initially, the patient was treated with a low dose of cytarabine, but this treatment proved ineffective. M-CSF was administered for 14 days by drip intravenous infusion, 800 x 10(4) units per day. After a gradual decrease in the number of leukemic cells, a rapid increase in neutrophils was observed in the peripheral blood, and a bone marrow examination 22 days after discontinuation of M-CSF medication revealed a complete remission. These findings suggest that M-CSF may be useful in treating some elderly patients who have hypoplastic AMoL.

Hyperphosphorylation of tau in PHF.

Tau in PHF is known to be highly phosphorylated and immunochemical study has indicated the similarity of the phosphorylation between PHF-tau and fetal tau. We have determined the exact phosphorylation sites in both PHF-tau and fetal rat tau by ion-spray mass spectrometry and sequencing of ethanethiol-modified peptides. In PHF-tau, 19 sites have been identified; all the phosphorylation sites except for Ser-262 are localized to the amino- and carboxyl-terminal flanking regions of the microtubule-binding domain. Half of them are shared by fetal tau. Thus, PHF-tau is much more phosphorylated. Whereas most of the sites in fetal tau are proline-directed, half of them in PHF-tau are nonproline-directed. Overall, the hyperphosphorylation of PHF-tau can be considered to consist of fetal-type phosphorylation and additional proline-directed and nonproline-directed phosphorylation. This extraphosphorylation may provide PHF-tau with the unusual characteristics including assembly incompetence.

Measuring short-wavelength-sensitive cone discrimination thresholds using pseudoisochromatic figures displayed on a color monitor.

PURPOSE: To simplify the testing of short-wavelength-sensitive (SWS) cone function in the clinic. METHODS: SWS-cone discrimination thresholds were measured along the tritan axis using pseudoisochromatic figures displayed on a color monitor. A circular 6 degrees field, containing spatially discrete patches of varying sizes and luminances, was presented on a background. A subset of patches formed the target patch in the shape of a C. Eight subjects with normal color vision reported the direction of the gap in the C using a cursor controlled by a joystick. DATA: were expressed in units of SWS-cone trolands. RESULTS: SWS-cone discrimination threshold increased slowly as the SWS-cone trolands of the starting chromaticity increased. The dependence of the threshold on the SWS-cone activation level was similar to literature reports of chromatic discrimination measured with conventional paradigms. CONCLUSIONS: The advantages of this method: (a) It is a simple intuitive task for patients. (b) The paradigm can be implemented with an 8-bit/gun color monitor. (c) The test avoids the need to define equiluminance for the individual patient before the color test is administered. This method can provide a useful technique for measuring SWS-cone function in a clinical population.
