Comparison of antimicrobial susceptibility patterns of fifty-seven strains of Haemophilus ducreyi isolated in Amsterdam from 1978 to 1985.

The in-vitro activities of 16 antimicrobial agents were tested against 57 strains of Haemophilus ducreyi isolated in Amsterdam during an eight year period. The susceptibility patterns of the isolates from different years were compared. In the first four years more than 30% were beta-lactamase negative and showed MICs for tetracycline of 2 mg/l or less. From 1982 to 1985 all strains, except one, produced beta-lactamase and were tetracycline resistant. Furthermore, MICs for cefotaxime, sulphamethoxazole and trimethoprim increased. No changes were seen with cephradine, erythromycin, kanamycin, gentamicin, chloramphenicol, nalidixic acid, pipemidic acid, norfloxacin, pefloxacin or ciprofloxacin.

Clinical and microbiological evaluation of 46 episodes of genital ulceration.

In 44 consecutive patients 46 episodes of genital ulceration were studied. The presumptive clinical diagnosis was evaluated by extensive microbiological investigations. In 15 (33%) episodes the clinical diagnosis did not accord with the microbiological one. Chancroidal lesions were most commonly found to have other microbiological aetiology. Secondary invasion of treponemal or viral lesions by other bacterial species (mostly anaerobes or pyogenic cocci) or genital pyodermia were the cause of confusion in six of nine cases of chancroid. In the other three a ducreyilike bacterium was found. Direct Gram staining of ulcer material did not help the diagnosis of chancroid. The implications of the results of this study for clinical practice are discussed.

Isolation of Haemophilus influenzae and Haemophilus parainfluenzae from genital-tract specimens with a selective culture medium.

A series of 2027 genital tract specimens was cultured for Haemophilus species on non-selective chocolate agar and on a selective medium (Choc-VBCA). The latter gave a significantly higher isolation rate. H. influenzae was isolated from 27 specimens and H. parainfluenzae from 81 specimens by use of the selective medium. The biotype distribution of both species was compared with that of an equal number of isolates from respiratory-tract secretions. H. influenzae biotypes II and IV were found to predominate in genital strains and biotypes II and III in respiratory strains. With H. parainfluenzae, biotype II was most frequent in both sites. Two new biotypes of H. parainfluenzae (VI and VII) are described. The significance of the use of selective media and of biotype distribution are discussed.

Antibiotic advice based on an intra-operative Gram-stain during cholecystectomy.

Intra-operative microscopy of the bile was performed during cholecystectomy in 335 patients. Based on the results, individual antibiotic advice was given. Bacteria were seen in the Gram-stained smear from 96 patients of whom 86 had a positive culture. In five patients a negative Gram-stain correlated with a positive culture (sensitivity 94.5%, specificity 95%, positive predictive value 89.5% and negative predictive value 97.5%). In only four patients (1.2%) did the antibiotic advice turn out to be wrong. Eight patients (2.4%) had signs of postoperative infection: one after wrong antibiotic advice, one who did not have prophylaxis because of false negative microscopy, four who did not have prophylaxis for clinical reasons but who had positive Gram-stains, and two despite apparently appropriate antimicrobial prophylaxis. An intra-operative Gram-stain as a guide to individual antibiotic advice given by a medical microbiologist during cholecystectomy is recommended.

Enzymic activity of Haemophilus ducreyi.

The enzymic activity of 29 Haemophilus ducreyi strains on 28 substrates is described. The results are compared with those of seven other authors. There is agreement only about the presence of alkaline phosphatase and arginine aminopeptidase and the lack of glycosidases. Possible reasons for the contradictions in the eight reports are discussed.

Characteristics of Haemophilus ducreyi in culture.

Growth on different media and the influence of culture conditions were studied on 19 recently isolated strains of Haemophilus ducreyi, none of which had more than four passages on artificial media. The results were compared with 10 laboratory strains, which had an unknown number of passages in vitro. For all strains, growth was best on 30% rabbit blood agar and on Bieling agar. The laboratory strains showed a tendency to grow better on chocolate agar than did the fresh isolates. Of 19 fresh clinical isolates, 12 were CO2 dependent, and 2 needed extra moisture for growth. From the 10 laboratory strains, only one needed CO2 and none needed extra moisture. All 29 strains grew under anaerobic conditions. Of the 19 fresh clinical isolates, 12 grew at 22 degrees C, but only 2 of the 10 laboratory strains grew at this temperature. The laboratory strains grew better than the fresh isolates at 37 degrees C, and the optimal pH for all strains was pH 6.5 to 7.0. All strains showed starch aggregation.
