Development of an instrument to measure patient perception of the quality of nursing care.

Although patient satisfaction has been given considerable attention in health care, analysis of the conceptual and measurement limitations of existing measures indicates that a more elemental approach to obtaining patients' perspectives is warranted. In this investigation we developed and evaluated the psychometric properties of an instrument designed to measure patients' perceptions of the degree to which their needs were met while hospitalized. This 15-item instrument, Patient Perception of Hospital Experience with Nursing (PPHEN), based on Swanson-Kauffman's framework of caring, is internally consistent and represents a single construct best described as feeling cared for. The relationship of PPHEN to other measures demonstrates concurrent validity; moreover, the scale is responsive to differences in care provided, as shown by differences in means for different hospitals. PPHEN offers a brief, theoretically oriented, internally consistent, and valid patient self-report measure of nursing care. It does not require patients to compare their expectations of care with the care received but only to evaluate whether their needs were met. It promises to be useful to clinical and health services researchers.

The effect of a professional practice model on staff nurse perception of work groups and nurse leaders.

OBJECTIVE: The objective of the study was to measure the effect of an enhanced professional practice model on perception of work groups and nurse leaders. BACKGROUND: Previous studies of staff nurse perception of work environment and nurse leaders were used to guide development of the professional practice model and the selection of variables. Implementation of the model was expected to result in more favorable perceptions of the work group and a desire for facilitative leadership style. METHODS: A quasi-experimental design was used to compare change over time in five-hospitals--an urban medical center, two community hospitals, and two rural hospitals. FINDINGS: Introduction of the model resulted in more favorable perceptions about the work group and a desire for a more facilitative nurse leader. CONCLUSION: Change in perception of work group and leader rather than job satisfaction may be an early indication of favorable outcome of planned change. Changes in nurse satisfaction may be a late outcome, which may be the reason findings from previous studies are mixed.

Implementation of a professional practice model for nursing in a rural hospital.

Growing concern about rural hospitals and the health of rural citizens highlight the need for high quality, progressive nursing care. To address this concern, several program changes to improve patient and organizational outcomes were made in one rural hospital; implementation success varied. Hospital culture and changes occurring within the health care environment affected the change process.

Evaluation of a clinical advancement system.

Evaluation of a nursing clinical advancement system was conducted to determine if it had achieved its goals: higher staff nurse job satisfaction and improved patient care. Findings included a trend toward higher job satisfaction and markedly lower turnover rates among clinically advanced staff. The subjective opinions of respondents supported a positive impact on patient care. The cost-benefit ratio for this clinical advancement system, however, was negative. Recommendations for improving the credibility and effectiveness of such systems are presented.

Helping adolescents who have disabilities negotiate transitions to adulthood.

Adolescents who have disabilities face unique challenges as they progress through the transitions necessary to achieve optimum functioning in adulthood. These youths often need professional assistance to successfully negotiate these important transitions. Our article describes processes for collaborating with these adolescents, their families, and other professionals to facilitate successful transitions to a more healthy, productive, and satisfying adulthood.

Improving child and family health through primary prevention of neural tube defects.

The neural tube defect, myelomeningocele, is the most complex congenital anomaly that is compatible with normal life, if appropriately treated. Knowledge of evidence-based primary prevention strategies for neural tube defects prevents the greatest number of these conditions from occurring. Primary prevention is key to family health. Only through primary prevention can the following be avoided: (a) the potential emotional trauma brought about by prenatal diagnosis of an affected fetus; (b) the associated decision-making about secondary prevention via selective termination; (c) the physical and emotional stress of the termination procedure, if it is done; and (d) the complex and often devastating problems faced by the child who is born with myelomeningocele and his or her family.

Health services research in nursing: current status and future directions.

To assess the current status of health services research in nursing, 113 studies reported between 1980 and 1989 were reviewed. Identified gaps in this research included comprehensive cost-benefit and cost-effectiveness analyses, effects of technology on patient care, evaluations of information systems, evaluations of innovative care delivery approaches, and studies of ethical issues related to the use of scarce resources.

A cost estimation model for measuring professional practice.

A cost estimation model was developed to monitor the effect of an enhanced professional practice model on cost to the institution. The model detected differences across units and changes in unit activities. One unit's cost savings were greater than the others and this difference was believed to be related to stable leadership over the course of the study.
