Measurement of IgG antibodies to Tamm-Horsfall protein (ATHA) in 92 bacteriuric schoolgirls aged 5-12 did not show a significant rise compared with the titres found in the sera of 24 healthy controls. ATHA titres showed no correlation with the presence of vesicoureteric reflux or kidney scarring and it is concluded that measurement of serum ATHA is of no value as a screening procedure for the detection of vesicoureteric reflux.

Rats sensitized to various strains of Escherichia coli (078, O2K13H1, O6K13H1 and O6K2a2cH1) showed cutaneous delayed type hypersensitivity (DTH) reactions after intradermal challenge with the same or a different strain of Esch. coli. Cutaneous DTH was transferred to non-sensitized rats by administration of lymphocytes from sensitized animals. The common antigen(s) responsible for DTH was not identified but DTH reactions were found to be unrelated to the O or K serotype of the Esch. coli strains used. Intrarenal administration of killed Esch. coli to animals showing cutaneous DTH and non-sensitized controls did not produce evidence of DTH reactions of the kidney. Instead it was shown that intrarenal administration of formalin- or heat-killed Esch. coli leads to kidney scarring in both sensitized animals and non-sensitized controls. These scars were comparable in severity, and were similar to those obtained after infection of the kidney with live organisms. It is concluded that DTH reactions do not play a role in the pathogenesis of kidney scarring associated with Esch. coli infection of the rat kidney but that the strains of Esch. coli studied possess a common heat- and formalin-stable nephrotoxic factor which induces kidney scarring.
In Wistar rats V-U reflux with distilled water and sterile saline of varying osmolar strength produces scars in the perihilar region of the kidneys in 80% of animals. Using Indian ink as a marker, rupture of the forniceal epithelium regularly followed V-U reflux. Intrapelvic pressure rose to 120 mm Hg during the reflux procedure. A slowing of proximal tubular transit time was observed by the Lissamine Green technique in the nephrons in the perihilar region immediately after the kidney had been subjected to reflux. It is suggested that the kidney scars which develop after reflux of sterile solutions are ischaemic in origin. It was shown that kidneys damaged by reflux have an increased susceptibility to haematogenous infection.
Chronic pyelonephritis is a disease of childhood that is carried on into adult life. Children under the age of 4 in whom UTI, vesicoureteric reflux and pyelotubular backflow coexist appear to be particularly at risk of developing kidney damage. Attempts to prevent kidney damage in adults by detection and treatment of covert UTI are doomed to failure because persistent bacteriuria in the adult is a consequence of underlying abnormality of the urinary tract rather than its cause. Our main hope for the prevention of kidney damage associated with bacteriuria and reflux is to detect and treat these conditions at the earliest possible age. The controlled trials of the value of treatment of bacteriuria and vesicoureteric reflux in childhood that are in progress in various centers in the United Kingdom should be allowed to continue and medical opinion as to the value of such treatments should not be allowed to crystallize before the results of these trials are known.

During screening of 16,800 primary schoolgirls, aged 4-12 years, in Cardiff and Oxford, significant bacteriuria was found in 294 (1-7%). Intravenous urography and micturating cystography were performed in 246 of these girls. The urinary tract was abnormal in 47%. Pyelonephritis with or without vesicoureteric reflux was present in 26% and reflux without renal abnormality in a further 16%. The prevalence of pyelonephritis and reflux was independent of age. With few exeptions kidneys without pyelonephritic scars appeared to be normal in size, even when ureteric reflux was present.
