Haemoglobin Presbyterian [beta 108 (G 10) Asn----Lys] in a Spanish family.

Clinical, haematological and biochemical aspects of a new family with heterozygous haemoglobin Presbyterian [beta 108 (G 10) Asn----Lys] are described. It could be readily separated by isoelectric focusing, DEAE-cellulose chromatography and by cation exchange high-performance liquid chromatography (HPLC). The characterization of this mildly unstable haemoglobin was made by reverse-phase HPLC. This is the fourth family with Hb Presbyterian so far described.

The identification of five rare beta-chain abnormal hemoglobins by high performance liquid chromatographic procedures.

The detection, quantitation, and characterization of five relatively rare beta chain abnormal hemoglobins mainly by high performance liquid chromatographic procedures are described. The variants involved are Hb City of Hope (beta 69 Gly----Ser), Hb Austin (beta 40 Arg----Ser), Hb Leiden (beta 6 or 7 Glu----0), Hb Louisville (beta 42 Phe----Leu), and Hb Presbyterian (beta 108 Asn----Lys). Some clinical and hematological data are also included.

[Evaluation of stomatological care in Santiago de los Caballeros, Dominican Republic, stage 1].

In Santiago de los Caballeros, Dominican Republic, a study was done of the status of dental care with the long-run aim of reforming the system and increasing the integration of education and service. The study centered on the current status of that integration, which is taking place between the Federal Secretary of Public Health and Social Welfare and the Universidad Católica Madre y Maestra, respectively the service and education establishments concerned. The study is being done in two stages. In the first, carried out in 1983 and described in the article, the chief aim was to assemble information on the system considered. The methodology consisted in the selection of a sample of 61 patients treated at three centers of basic dental care in the area; use of the techniques of direct observation, interviews, measurement of the time spent on each operation, and the building up of files; and a study of all the variables that could have influenced an assessment of the situation at the time and of projections for the future. Some of the variables were the priority group, the age group, health status, progress of the treatment, and the reason for seeking care and choosing the center. It was found that 81,97% of the patients were under 14 years of age and 78,69% were between 5 and 14 years old. Most of the patients said they were in good health, although a high percentage reported having had certain diseases. Of all patients studied, 73,77% were discharged--a satisfactory figure. The main reasons for coming to the center were personal initiative and the recommendation of another, not medical referral, and the reason for choosing the center was its proximity to the patient's home. On the basis of the study the research team recommended a revision of the standards for dental services nationwide, adaptation of the work done to the physical premises, planning for the integration of education and service, the encouragement of innovative attitudes in service and education professionals and, lastly, extension of the manpower training program--with the integration of education and service--to the communities and instead of confining it to intramural instruction.

Steroid secretion by a lipoid cell tumor causing virilization and its diagnosis with computerized tomography.

Presented is the case of a virilized woman with a lipoid cell tumor of the ovary localized by computerized tomography. The major secretory products of the tumor were testosterone and estradiol; the production of androgens was responsible for the clinical features and hindered the effects of estrogens. Elevated levels of estradiol indicated important ovarian aromatase activity as reflected by large ovarian-peripheral venous gradients.

Fungal endocarditis caused by Arnium leporinum following cardiac surgery.

A case of fungal endocarditis caused by the coprophilous ascomycete Arnium leporinum is reported in a 19-year-old male living in Colombia. The fungus was seen in tissue and recovered in pure culture from peripheral emboli, a mitral valve, pus from an osteolytic lesion, and kidney. The patient died 21 months after cardiac surgery. The range of A. leporinum in the Americas is expanded to include Colombia.

Haemoglobin LeporeBaltimore in a Spanish family.

Description of the clinical, haematological and biochemical aspects of a new family with heterozygous haemoglobin LeporeBaltimore. Of the 6 members affected. 4 were totally asymptomatic. All of them showed morphologic features of heterozygous beta-thalassaemia. The levels of haemoglobin Lepore ranged from 12.2 to 15% (mean value: 13.7 +/- 1.1%). The analysis of the primary structure of the haemoglobin shows that crossing over takes place between amino acid 50 of the delta-chain and amino acid 86 of the beta-chain. This is the third family, to our knowledge, with haemoglobin LeporeBaltimore so far described.

Intraerythrocytic adaptation to anaemia in thalassaemia.

Red cell 2.3-diphosphoglycerate (2.3-DPG) response to anaemia is evaluated in 83 patients with different types of heterozygous thalassaemias, and 40 patients with chronic ferropenic anaemia and 120 healthy subjects are used as control groups. In heterozygous beta, beta delta, and alpha thalassaemic patients the 2.3 DPG concentration is appropriate for the degree of anaemia present.

Relationship of red-cell 2,3-diphosphoglycerate with anaemia, hypoxaemia and acid-base status in patients with cirrhosis of the liver.

The red-cell 2,3-diphosphoglycerate (DPG) concentration is determined in 60 patients with hepatic cirrhosis, in 33 with iron deficiency anaemia and in 86 healthy subjects. In all cases, the erythrocyte volume fraction and the haemoglobin concentration are simultaneously measured, while the cirrhotic patients undergo, at the same time, analyses of the arterial pH, pO2 and pCO2 and of the levels of inorganic phosphate, bicarbonate and lactate in their venous blood. In the 60 cirrhotic patients the red-cell DPG concentration (7.40 +/- 1.23 mmol/l) is significantly higher (P less than 0.001) than in the 86 control subjects (4.58 +/- 0.59 mmol/l) and the 33 patients with iron deficiency anaemia (5.86 +/- 1.06 mmol/l), although the level of anaemia in the latter is greater (P less than 0.001) than in the patients with liver cirrhosis. The DPG concentration found in the cirrhotic patients was far higher (P less than 0.001) than the theoretical value attributable to them by virtue of their grade of anaemia (5.21 +/- 0.95 mmol/l), which value is deduced mathematically from the equation of the regression line between haemoglobin and DPG normal in patients with iron deficiency anaemia. Anaemia, hypoxaemia and acid-base disturbances are disorders frequently associated with cirrhosis of the liver. In the present study we deduce that alkalosis, and therefore the plasma pH level, is the most important factor causing the increased DPG concentration in patients with liver cirrhosis for any level of haemoglobin, with respect to other subjects with anaemia.
