Myocardial uptake of thallium-201 diethyldithiocarbamate (201T1-DDC) in cultured heart cells and in humans.

We assessed the feasibility of SPECT imaging with 201T1-diethyldithiocarbamate (201T1-DDC), a new cerebral blood flow tracer with little distribution, expecting to observe less extensive redistribution than with 201T1-chloride. Myocardial sections were obtained in three patients presenting with documented coronary artery disease and injected at peak exercise with 100 MBq 201T1-DDC. In two patients there was a clear redistribution phenomenon at four h after injection. In cultured myocardial cells of newborn rats, the uptake and washout of 201T1-chloride and 201T1-DDC were compared. The 201T1-DDC uptake was lower than 201T1-chloride (transmembrane gradients were respectively 89 +/- 10 and 4.1 +/- 0.2, mean +/- sem, n = 14, P less than 0.001). After 2 h washout in a T1 free medium, the retention of 201T1-chloride in the cells was 4% vs 19% for 201T1-DDC. It is concluded that although myocardial imaging is feasible with 201T1-DDC, this agent redistributes significantly with time.

[Tomoscintigraphy of the myocardium labelled with thallium-201. Value in the quantification and prognosis of myocardial infarction].

Myocardial thallium 201 tomography was performed in 112 patients admitted to hospital in the acute stage of a first myocardial infarction. Our quantification method for the myocardial lacuna, validated by animal studies, is described. The necrotic mass evaluated is compared with enzymatic (CPK) (r = 0.89) and angiographic (r = 0.78) quantitative assessments. Thallium 201 tomographic quantification has an unquestionable prognostic value. Thus, among patients whose infarcted mass represented more than 25% of the total myocardial mass, 71% died or developed severe heart failure. Conversely, 93% of patients with a small infarct (less than 20% of total myocardial mass) were asymptomatic on a 24-month follow-up. Myocardial thallium 201 tomography provides a reasonable quantification of the infarct. Its prognostic value seems to be satisfactory.

Dissolution rate and transit times of technetium-99m DTPA-labeled tablets.

In this study we demonstrate that the dissolution rate and gastroduodeno-cecal transit time of radiolabeled tablets of theophylline can be determined in vivo using technetium-99m (99mTc). Six healthy male volunteers ingested a tablet containing 300 mg of theophylline mixed with 3.7 MBq of [99mTc]DTPA. Anterior and posterior scintigraphic views of the abdomen were collected serially over 8 hr, after which a 200-ml solution containing 37 MBq of [99mTc] pertechnetate was ingested in order to visualize the contours of the stomach. The in vivo activity contained in the tablet was calculated from the scintigraphic views after correction of background activity, radioactive decay, and depth attenuation. The dissolution rate of [99mTc] DTPA was also measured in vitro and compared with the dissolution rate of theophylline. The results showed close dissolution rates between [99mTc]DTPA and theophylline in vivo (T1/2 184 min and 176 min, respectively), and a faster early dissolution rate of [99mTc]DTPA in vitro (T1/2 92 min versus 156 min for theophylline). The mean gastroduodenal and duodenocecal times were 72 +/- 25 min (m +/- s.d.) and 245 +/- 15 min, respectively. Scintigraphic imaging of labeled formulations with 99mTc present useful applications in pharmaceutics and pharmacology.

[Muscular fixation of thallium-201 in lower limb arteriopathies before and after injection of Royat thermal gas].

Thirty patients with peripheral vascular disease (grade II) were studied by way of thallium-201 (Tl-201) muscular scan in order to assess a possible muscular effect of the Royat spa natural gas. Immediately after a 1.1 MBq/kg Tl-201 IV injection, a dynamic scintigraphic study was performed during 15 minutes in order to obtain a time-activity curve (characterized by Tmax) over the thighs, at rest. Then static anterior and posterior views were performed over thighs and calves during 4 minutes each. Perfusion ratios were calculated as thigh/knee and calf/ankle. The GE maxi 400 T gamma-camera was connected to an Informatek treatment system, zones of interest being chosen to obtain anterior and posterior thigh/knee, leg/ankle uptake ratios. Two studies were realized: the first one in basal conditions, the second one 7 days later, immediately after a s/c injection of natural gas. The later study was preceded by a static counting to subtract the residual muscular activity. After gas injection, one observed that Tmax was shorter, on the average, and that the difference between time-activity curves observed during the first study disappeared for patients with unilateral disease. A significant Tl-201 uptake increase was observed for the anterior image of the calf, while no difference was noted on posterior images, natural gas being injected on the front face of legs. This was particularly clear for patients with unilateral disease and receiving only unilateral gas injection.

[Acute myocarditis in Lyme's syndrome. Value of myocardial scintigraphy with gallium 67].

The authors report the case of a 35 year old man with no known previous cardiac disease. One month after a tic bite causing diffuse abdominal erythema, he was admitted to hospital with fever, breathlessness and bradycardia. The electrocardiogramme showed first degree atrioventricular block with a sinoatrial block (SA = 200 ms, AH = 240 ms). Echocardiography eliminated the diagnosis of pericardial effusion. Thallium 201 myocardial scintigraphy was pathological and showed irregular global uptake suggesting cardiomyopathy. Gallium 67 scintigraphy showed increased uptake in the left ventricle. The evolution was uncomplicated with normalisation of clinical, ECG and radiological changes. Cardiac catheterisation and angiography eliminated ischaemic and primary cardiomyopathy. Control radionuclide investigations were normal at one month: there was no persistent abnormal Gallium uptake. The diagnosis of Lyme's syndrome was confirmed by positive serology with successive titres of 1/1024 and 1/2048 (significant at titres over 1/256). This unusual case illustrates: the risk of myocardial disease in Lyme's syndrome; the diagnostic value of Gallium 67 scintigraphy in acute myocarditis: Gallium seems to fix specifically on inflamed tissues, so distinguishing myocarditis from primary cardiomyopathies.

[Evaluation of the effect of Royat thermal gas in arteriopathy of the legs using thallium-201 muscular scintigraphy].

12 patients (group II) suffering from an arteriopathy of the lower limbs, stage II, were examined by thallium 201 muscular scanning. The aim of this study was to look for a possible muscular effect of Royat thermal gas injected subcutaneously. A preliminary study was conducted at rest, and consisted of recording the initial decay curve of thallium in the thighs and the study of the muscular fixation by different segments of the limbs, including the determination of the ratios of amounts fixed by the muscular and non-muscular zones. The results were compared with those obtained in 11 healthy subjects (group I) and no significant difference was noted between the two groups at rest. On the other hand, a considerable improvement of the fixation in the legs was observed after subcutaneous injection of thermal gas. The results are discussed in relation to the clinical and paraclinical data.

[Diagnosis of the Gougerot-Sjögren syndrome in rheumatology. II--Test strategy].

The diagnostic evaluation of the sicca complex in rheumatology depends, on the one hand, upon performing tests and, on the other hand, upon the specific clinical presentations which lead to these tests. The frequency with which the clinical problem is encountered should be taken into account as well as the probability of whether or not the patient does indeed have the sicca complex, and the necessity of making a precise diagnosis. A labial biopsy should be performed when it is necessary to confirm the diagnosis. Schirmer's test is useful to evaluate the ocular component of the sicca complex, which is the only component requiring therapy. In other cases, one examination can follow another in sequence such as salivary nucleotide scanning followed by biopsy and histological examination.

[Early diagnosis of postinfarction ventricular aneurysm. Value of contrast scintigraphy].

Early diagnosis of a ventricular aneurysm after myocardial infarction may provide a valuable means of preventing certain complications. We studied 55 patients with acute myocardial infarction on the 8th day by Technetium 99 angioscintigraphy with study of wall motion and ejection fraction at equilibrium. The results were compared with left ventriculography performed three months after infarction. Angioscintigraphy demonstrated 17 cases of dyskinesia, 16 cases of hypokinesia, and 22 cases of akinesia, divided into two subgroups depending on whether the ejection fraction was lower (n = 11) or higher than 40% (n = 11). Contrast angiography at 3 months confirmed the presence of 27 aneurysms, 17 akinetic plaques and 11 cases of hypokinesia. Comparison between the two series of investigations showed excellent correlations: in the group of dyskinesia (17 cases) an aneurysm was confirmed in 15 cases; in the group of hypokinesia (16 cases) only one aneurysm was demonstrated by the reference angiography. The results were less uniform in the group with akinesia. In the subgroup with a low ejection fraction, 9 out of 11 cases progressed to aneurysm and, in retrospect, should be considered as likely aneurysms. There were only 2 aneurysms (2/11) in the subgroup with localised akinesia and a high ejection fraction. The overall results suggest that angioscintigraphy at the 8th day of myocardial infarction is a reliable means of detecting left ventricular aneurysm; in cases of dyskinesia or widespread akinesia with an ejection fraction of less than 40% the presence of an aneurysm was confirmed in 24 out of 28 patients (86%).(ABSTRACT TRUNCATED AT 250 WORDS)

[The diagnosis of Gougerot-Sjorgen syndrome in rheumatology. I. Evaul ation of the principal complementary examinations].

The examination of a patient with Sjögren's syndrome includes evaluation of the eye, the buccal cavity, and a search for certain factors in the blood. Schirmer's blotting-paper test is a good test but is not specific. In addition, a decreased amount of tearing is difficult to interpret after the age of 45. Slit-lamp examination (rose bengal and fluorescein) yields lesions which confirm keratoconjunctivitis due to decreased tearing. The buccal component is difficult to evaluate. A biopsy of the buccal mucosa gives the best results with minimum risk and expense. Nucleotide scanning is sensitive, but less specific. Salivary flow decreases with age. After 60 years of age this decrease can not be interpreted. The chemical composition of tears or of saliva is promising, but it is not yet a part of the usual diagnostic work-up. Of the available laboratory tests, anti-SS-A antibodies and/ or anti-SS-B antibodies are of value, but they are not found consistently.

Feasibility of gated single-photon emission transaxial tomography of the cardiac blood pool.

A procedure for gated single-photon emission transaxial tomography of the cardiac blood pool using a commercially available rotating camera and a computer is described. During image acquisition, the end-diastolic and end-systolic frames are retained after each of the 16 angular steps of the rotation of the camera around the anterior side of the patient's chest. Complete acquisition takes 25 minutes; 10 more minutes are required for the reconstruction and processing of the sections. One set of images is oriented transaxially; the other set includes vertical sections obtained by rotation around a vertical axis passing through the center of the left ventricular cavity. Results from the planar and tomographic examinations were in good agreement for most of the 18 patients who underwent both studies, although more inferior abnormalities were detected with tomography.

Study of cell changes induced by radiolabelling of human lymphocytes with 99m-technetium.

With the aim of evaluating cell changes possibly induced by labelling lymphocytes with 99m-technetium (99mTc), several rosette tests and stimulation assays with mitogens have been carried out before and after radiolabelling. The results presented here show that (i) as the rosette tests are not modified statistically, the original labelling method does not seem to induce short term membrane structure modification and (ii) as stimulation by mitogens decreases after labelling with 99mTc, medium term radiation damage, linked to autoirradiation of the cells, can be evoked. This study indicates that there is reason to limit the time taken to perform scintigraphic explorations with labelled lymphocytes in man.

Labeling of human lymphocytes with 99mTc by means of stannous pyrophosphate. Scintigraphic applications.

An original 99mTc-labeling method applied to human lymphocytes is described. This technique is based on the use of stannous pyrophosphate to reduce sodium pertechnetate. The proposed procedure has two main advantages: firstly, the labeling process takes place in a neutral and buffered medium which prevents any cellular aggregation; secondly, the labeling yield obtained is compatible with scintigraphic studies in man. The influence of each parameter on the labeling quality had been studied as well as the binding stability of the technetium fixed on cells. A systematic study of lymphocyte surface markers, before and after labeling, has led to the suggestion that the labeling process seems to affect cellular membranes, probably because of the technetium binding. Finally, scintigraphic results are presented, the study being performed on eight healthy volunteers. These results are compared with those published by other authors who used either a different radioisotope or a different labeling method.

Emission computed tomography vs perfusion scanning in lung disease.

Emission computed tomography has achieved considerable credibility in thallium-201 cardiac imaging and in brain imaging studies. To date the procedure has had only limited application in the study of other organs. In the present study traditional perfusion lung scans were compared with horizontal, sagittal and frontal tomograms in 30 patients. Preliminary results showed agreement between the two methods in most cases. Tomography, however, gave more precise information about the site and extent of areas of deficient lung perfusion. In a small number of cases tomography also revealed defect not seen on conventional scans. In patients with cardiomegaly in whom conventional scanning data were difficult to assess, tomography proved enlightening.

Histological and immunological study in patients with rheumatoid arthritis showing isolated abnormalities of salivary scintigraphy.

We report the histological and immunological findings in 20 rheumatoid arthritis (RA) patients with impaired salivary scintigraphy but without sicca complex. This condition was stable over 2-5 years and associated with mild infiltrates on lip biopsies. Increased levels of IgG, immune complexes, and antinuclear antibodies were found in seven patients (35 vs 69% in secondary Sjögren's syndrome). Two of the seven patients later developed xerostomia. The above-mentioned immunological abnormalities may increase the likelihood of developing a clinical impairment of salivary function in patients with an abnormal salivary scintiscan, the latter being a frequent, yet often isolated, finding in RA. Antisalivary duct antibodies were not related to any other parameter of salivary gland involvement.

A comparison between conventional scintigraphy and emission tomography with thallium-201 in the detection of myocardial infarction: concise communication.

We have compared emission tomography and conventional scintigraphy with thallium-201 in a series including 15 normal subjects and 64 patients showing transmural myocardial necrosis in various locations, fully documented by clinical, electrocardiographic, and enzymatic evidence. The reconstruction was derived from 32 projection images collected around the left side of the patient's chest by a rotating scintillation camera. The conventional views and the transverse, frontal, and sagittal sections were interpreted independently by two observers. The final calculated sensitivity was 89% with conventional scintigraphy and 98% with emission tomography, and the specificity was 93% in the two cases. Thus, emission tomography provides a better sensitivity and also a better interobserver agreement than conventional scintigraphy in the detection of transmural myocardial necrosis with thallium-201.
