Social and personality factors in dapsone resistant patients.

A preliminary study in which a sample of 25 patients registered with SLR and TC, Karigiri, with proved secondary DDS-resistant leprosy were compared, with 25 patients not suspected to be DDS-resistant, for social and personality factors, which could have lead to non-compliance in treatment. A structured interview schedule enquiring into factors leading to irregular treatment and the psycho-social background of the patient was used. The Eysank's Personality Inventory (EPI) and Personality traits assessment on a linear analogue was also administered to them. Statistical analysis showed that DDS-resistant leprosy patients were as a group more irregular for treatment, showed personality traits of being casual and uncontrolled and scored high on the neuroticism scale of the EPI.

Group b Streptococcal pneumonia in the elderly.

Group B streptococcal infections, although well studied in neonates, have only recently been appreciated as important infectious agents in adults. Seven cases of Group B streptococcal pneumonia were verified by transtracheal aspiration, blood and sputum cultures, or multiple stab cultures at autopsy. The infections were largely nosocomial and, ultimately, fatal in all seven patients. Our patients were older (average age, 73 years) and much more debilitated than the 13 cases reported in the literature. Diabetes was less common than previously reported. Previous antibiotic therapy was common. Concomitant isolation of another organism (especially Staphylococcus aureus) occurred in five patients. The morphologic findings at autopsy, in one patient, were characterized by a severely necrotizing destructive process. In our experience, Group B streptococcal pneumonia is more common, more devastating, and occurs in an older population than previously reported.

Thrombosis and pancreatic carcinoma revisited.

A case of recurrent pulmonary embolism from thrombophlebitis associated with pancreatic carcinoma is reported. There is an increased incidence of thrombophlebitis with all tumors, but carcinoma of the pancreas is statistically more frequently responsible. The higher incidence of thrombophlebitis with tumors of the body and tail of the pancreas is probably due to the low trypsin levels associated with these tumors. Trypsin levels are directly related to plasma antithrombin levels and mucinous adenocarcinomas are more commonly associated with thrombus formation.

Psychiatric disturbances among leprosy patients. An epidemiological study.

An epidemiological study was done to determine the prevalence of psychiatric disturbances among leprosy patients. Using a stratified random sampling technique, 540 subjects with leprosy were selected from a total population of 5745 leprosy patients. A mental health evaluation was conducted on 494 of these subjects. Forty-nine of these patients were found to have a psychiatric disturbance, giving a prevalence rate of about 99 per 1000. Depressive reaction was the most common disturbance seen. Those patients who were sick for a longer time and those who developed physical deformities were found to be more prone to get psychiatric disturbances. It is emphasized that supportive psychotherapy is important in the management and rehabilitation of leprosy patients.

Usefulness of saliva lithium estimation.

Estimation of lithium in 24 samples of serum and saliva show that there is a very high correlation between the lithium levels in serum and saliva (r= + 0-88). The ratio found in our patients is very similar to that reported from North America. Estimation of lithium in saliva at different periods of time show there is a good degree of stability for saliva lithium levels. Thus saliva can be used as a substitute for serum for lithium monitoring. The practical advantages of this are discussed.

Observer variation and depressive phenomenology.

An investigation into observer variation and depressive phenomenology is described. A group of 6 clinicians independently completed a 43-item sheet for 20 depressed patients at the same clinical interview. The coefficient of agreements on the items are given. Patient variables and observer variables did not have any influence on the degree of agreement. There was also an agreement in the subtyping of depression.
