The therapeutic action of Triazolam has been assessed against a placebo by administering the two substances on a double blind basis the evening prior to surgery. The results show that Triazolam was significantly better than the placebo as regards induction time and the duration and quality of sleep. There were few unimportant side-effects.

The Authors performed ona group a patients suffering from drug exanthema a thorough clinico-anamnestic and allergo-immunological investigation (skin reactions, patch-tests, Prausnitz-Küstner passive transport, in vitro lymphocyte blastization, total serum IgE radioimmunoassay). The results obtained do not support an immunoallergical pathogenesis of this syndrome. The possible alternative hypotheses and the future perspectives of study are discussed.
Inhibition of prostaglandin (PG) biosynthesis has been proposed as a mechanism of aspirin-induced asthma. In order to verify the existence of a generalized abnormal sensitivity of the PG cyclo-oxygenase in intolerant patients, we have compared the inhibitory effects of aspirin on platelet PGE2 and PGF2alpha spontaneous formation in vitro, in 10 healthy subjects, 10 aspirin-sensitive asthmatics, and 10 nonasthmatic aspirin-sensitive patients. No statistically significant differences were found in the pattern nor in the absolute concentrations of both PGs in the control platelet superfusates. Moreover, the synthesis and release of both PGs were inhibited by aspirin with the same potency in the three groups studied.

Skin-tests, patch-tests, Prausnitz-Küstner reactions, passive cutaneous anaphylaxis in guinea pigs according to Ovary and lymphocyte proliferation tests in vitro using thymidine 2-C14 have been carried out in 31 patients with aspirin intolerance. Acetylsalicyclic acid, aspiryl-polylysine, lysine acetylsalicylate, and sodium salicylate were used in these investigations. Collagen-induced platelet aggregation according to BORN in the presence of aspirin was also studied. An immunological mechanism was revealed in only a small percentage of the patients studied, indicating in the remaining cases the possible role of nonimmunological processes or the inadequacy of the test technique employed.

Oesophageal papillomas are known to occur in cattle infected with bovine papillomavirus type 4 (BPV-4), and BPV-4 papillomas may undergo malignant progression in cattle that feed on bracken fern. In the south of Italy, where bracken fern is common, examination of 1133 slaughterhouse cattle aged 4-12 years revealed oesophageal lesions (single or multiple peduncuolated proliferations, or mucosal thickening) in 147 (13%). These two types of lesion were consistent with exophytic and inverted papilloma, respectively. BPV-4 was detected by polymerase chain reaction (PCR) analysis in >60% of the samples in which oesophageal papilloma was diagnosed histopathologically. Nucleotide sequencing of the PCR amplicons confirmed the presence of BPV-4 in the papillomas. This is the first report of such infections in a European country other than Britain.
A case of hepatoid adenocarcinoma of the stomach is presented. The characteristic features of the tumor are summarized on the basis of the authors' experience and the literature. Ultrastructural examination revealed patchy condensations of chromatin throughout the nucleus suggestive of necrosis-like programmed cell death (PCD). These nuclear alterations were associated with the occurrence of vacuoles and lipofuscins, conferring an autophagic phenotype to this PCD. Thus, the case reported here provides an example of autophagic-related necrosis-like PCD. Alternative PCDs are reviewed and their morphologic distinction is discussed.
We studied 150 metal workers occupationally exposed to metals and metalworking fluids (MWFs) to determine the prevalence and nature of contact dermatitis. 150 office workers were used as non-exposed control group. Questionnaires were administered to evaluate occupational and non-occupational exposure. All subjects underwent a dermatological examination and patch-testing with standard allergen series and MWFs used in the plant. Twenty-eight metal workers (18.6%) presented minor skin disorders involving the hands (vs. only 2% of the controls), ten (6.6%) had major disorders (similar to the figure for the control group - 5.4%), and 112 (74.8%) had no lesions, as opposed to 92.6% of the control group. Positive patch tests were found in ten metal workers: eight had major skin disorders (six to nickel, cobalt and chromium, one to nickel and cobalt, one to nickel) and the remaining two were asymptomatic (one positive for nickel and chromium, one for nickel). Among the controls there were three cases of positivity, all among asymptomatic subjects. Patch tests with MWFs were negative. The prevalence of dermatoses among the metal workers was significantly higher than that of controls (p<0.01), and all cases of allergy in this group were provoked by metals themselves.
Although atopic dermatitis (AD) is a common disease, its etiopathogenesis is not well known. The diagnosis of AD is based solely on the clinical criteria proposed by Hanifin and Rajka. In order to understand the immunological mechanisms involved in the pathogenesis of AD, we have classified the patients affected by this disease in four groups according to the results of skin prick-tests, specific IgE and patch-tests. This classification is intended to separate and compare the patients affected by AD according to the involvement of immunological type I and/or type IV mechanisms. Our results show that, although all the patients studied are clinically affected by AD, there are four different groups of patients who present an apparently diverse immunopathological mechanism. There is a group that seems to have an IgE mediated mechanism, another group that suggests a cell mediated mechanism, another group which seems to involve both mechanisms, and yet another group that apparently does not show any of the above mentioned mechanisms. In the present article we hypothesize and argue that the imbalance of the immune system is a consequence of the still unknown etiopathogenetic mechanism of AD, but perhaps not the cause of AD.
