Human papillomavirus in breast cancer.

Histological sections from paraffin-embedded breast carcinoma and axillary lymph nodes were examined for the presence of human papillomaviruses by two different techniques: the polymerase chain reaction (PCR) and the in situ hybridization with biotin-labelled probes. By PCR we detected HPV 16 DNA sequences in 29.4% of breast tumours and in some metastatic lymph nodes, though we were unable to identify any HPV DNA sequences by in situ hybridization. These results suggest that HPV's could play a role in the genesis of breast neoplasia.

Food-dependent exercise-induced anaphylaxis: report of two cases.

Exercise-induced anaphylaxis (EIAn) is a rare condition characterized by giant urticaria, angioedema and acute gastrointestinal symptoms that develops on exertion. In the most severe forms it may be associated with acute cardiorespiratory symptoms (laringeal stridor, wheezing), profound hypotension or syncope. In some individuals, EIAn characteristically occurs after a meal suggesting that the anaphylactic reaction is provoked by both exercise and ingestion of a foodstuff to which the patient has become sensitized. Two representative cases of severe food-dependent EIAn are described, which emphasize the need of performing a careful allergological evaluation in sportsmen with unexplained cardiovascular and/or respiratory symptoms during effort, especially when associated with other allergic manifestations and/or occurring in the post-prandial period.

Delayed hypersensitivity to 6-methyl-prednisolone in Henoch Schöenlein syndrome.

We report a case of allergic reaction to oral 6-methyl-prednisolone in a patient with Henoch Schöenlein syndrome. When this syndrome was first diagnosed the patient was started on 6-methyl-prednisolone orally and after 4 days he developed a pruriginous generalized maculo-papular eruption. The rash disappeared 1 week after withdrawal of 6-methyl-prednisolone. The skin tests performed 1 month after with main food allergens were negative. Patch test with 0.10 ml of solution containing 6-methyl-prednisolone 40 mg/ml was positive after 48 and 72 h. The cutaneous biopsy on the patch tested skin revealed a perivascular infiltrate of lymphocytes, histiocytes and eosinophils.

Methisoprinol-effect on the replication cycle of human hepatitis A virus.

The antiviral activity of methisoprinol was investigated under different conditions using a strain of hepatitis A virus (HAV), that shows a strong cytopathic effect on the Frp/3 cell line 7-9 days post-infection. Treatment of Frp/3 at a dose range of 125-1200 micrograms/ml had no toxic effect and showed a dose dependent inhibition of the HAV replication cycle. At the methisoprinol dose of 500 micrograms/ml the cytopathic effect was completely abolished and HAV antigen production reduced by 50% as measured by indirect immunofluorescence (IIF) and commercial enzyme-linked assay (ELISA). The virus yield was virtually abolished at the highest dose employed (1000 micrograms/ml).

Human papillomavirus DNA as a possible index of invasiveness in female genital tract carcinomas.

Paraffin-embedded tumour sections were used for the polymerase chain reaction (PCR) with three primer sets that amplify specific regions of human papillomavirus (HPV) types 11, 16 and 18. The positive samples were confirmed by hybridisation of the amplified sequences with the specific HPV probes. In all screened metastases the same viral sequences were found as in the primary tumour. HPV 16 was the most frequently detected virus in genital tract tumours. In a metastatic ovary carcinoma with unknown primary site HPV 16 DNA was observed. Moreover, pelvic lymph nodes with no microscopic evidence of metastases contained HPV DNA of the same subtype as the primary tumour. Thus, the HPV DNA detected by PCR is a useful indicator of neoplastic cells in the earlier stages of invasiveness. The finding of specific HPVs in the metastatic lesions could also provide information about the location of the primary neoplasia.

Presence of human papillomavirus type 18 DNA in vulvar carcinomas and its integration into the cell genome.

We have screened 78 genital tract tumours from Italian female patients for the presence of human papillomavirus type 16 (HPV-16) and HPV-18 DNA. Dot and Southern blot hybridization experiments revealed that DNA sequences of HPV-16 and/or HPV-18 are present in 66% of tumour samples. HPV-18 was detected in 80% of vulvar carcinomas. In these tumours the integration of HPV-18 DNA seems to occur in the E1/E2 region of the virus genome with long deletions in almost all samples. The invariable retention of the non-coding region and the E6/E7 open reading frames suggests that the integration event and the possible expression of these sequences could play a role in some stages of cellular transformation.

Antigenic and biochemical characterization of poliovirus type 2 isolated from two cases of paralytic disease.

Three isolates of poliovirus type 2 were obtained from rectal and pharyngeal swabs of two patients with paralytic poliomyelitis from Southern Italy (1981/1983). Neither one of the patients had received oral poliovirus vaccine, although a systematic vaccination campaign is conducted in the area. Neutralization tests with strain-specific antisera and analysis of the structural proteins by SDS-PAGE indicated that the three isolates were typical Sabin-like poliovirus type 2. Moreover, neutralization assays by means of a panel of well-characterized monoclonal antibodies indicated that the three isolates carried an antigenic mosaic where both wild-type and Sabin-specific epitopes were represented. The T1 fingerprints of the RNA of the three isolates diverged slightly from the standard Sabin reference map. A comparison of the oligonucleotide fingerprints of the viruses isolated from the pharynx and the feces of one patient indicated that mutations appeared as early as during replication in the intestinal tract.

ASA disease: the clinical relationship of nasal polyposis to ASA intolerance.

We studied the complex relationship between nasal polyposis and ASA (acetylsalicylic acid) intolerance in 154 patients with nasal polyps. The clinical histories of all patients were reviewed, and diagnostic tests for immune or allergic causes and the responsiveness of patients to challenges with ASA-substitutive drugs were analyzed. A third of our patients tested were found to have positive personal histories of atopy and 35% showed ASA intolerance. Although 40% had bronchial asthma, only 16.8% of all patients had positive tests for allergy. We were unable to find a specific mechanism to explain the relationship between nasal polyposis and ASA intolerance and further investigations are still required.

Isolation and molecular cloning of a fast-growing strain of human hepatitis A virus from its double-stranded replicative form.

A fast-growing strain of human hepatitis A virus was selected and characterized. The virus has the unusual property of developing a strong cytopathic effect in tissue culture in 7 to 10 days. Sequences of the viral genome were cloned into recombinant plasmids with the double-stranded replicative form as a template for the reverse transcription of cDNA. Restriction analysis and direct sequencing indicate that this strain is different from that described by Ticehurst et al. (Proc. Natl. Acad. Sci. USA 80:5885-5889, 1983) in the region that presumptively codes for the major capsid protein VP1, but both isolates have conserved large areas of homology in the untranslated 5'-terminal sequences of the genome.

Usefulness of carcinoembryonic antigen measurement in gastric juice of patients with gastric disorders.

Due to conflicting reports on the role of CEA measurement in the gastric juice of patients with gastric malignancy, we have assessed gastric CEA levels in a variety of both nonmalignant and malignant lesions of the stomach. The average gastric CEA levels were higher in patients with gastric ulcer and gastric cancer than in healthy subjects and those with duodenal ulcer. In addition, the frequencies of abnormally high values were significantly (P less than 0.001) higher in gastric ulcer or gastric cancer than in healthy or duodenal ulcer groups, whereas there was no difference between gastric ulcer and gastric cancer groups. The evaluation of gastric CEA levels in relation to gastric inflammatory changes leads us to suggest a close relationship between gastric CEA values and the degree of gastric inflammation. Gastric lavage cytology did not provide a sensitive tool for detection of gastric disorders. This study suggests little value for gastric CEA determination in gastric cancer detection. However, we believe the association of rising gastric CEA levels with various types of gastritis justifies measuring gastric CEA to recognize "precancerous" mucosal changes of the stomach.
