[Accuracy of magnetic resonance (0.5 T) in traumatic lesions of the knee. Controlled 87 cases].

In this study the authors investigated the diagnostic accuracy of Magnetic Resonance (MR) imaging as a method to recognise traumatic disorders of the knee; in particular its capability to identify meniscal and cruciate ligaments tears was examined. The results have been compared with those from arthroscopy and/or surgery, which were considered the gold standard of the study. Eighty-seven consecutive patients with suspected traumatic injury of the knee were prospectively studied. All patients underwent MR examination (0.5 Tesla magnet, gradient echo sequences, slices thickness = 5 mm) and, within 15 days/1 month, arthroscopy or surgery. The high diagnostic accuracy (internal meniscus = 87.5%; external meniscus = 88.5%; anterior cruciate ligament = 93.1%; posterior cruciate ligament = 96.5%) has confirmed the good diagnostic capabilities of MR in assessing traumatic lesions of the knee.

Pancreatic cancer versus chronic pancreatitis: diagnosis with CA 19-9 assessment, US, CT, and CT-guided fine-needle biopsy.

The authors prospectively performed serum CA 19-9 assessment, ultrasound (US), computed tomography (CT), and CT-guided fine-needle aspiration biopsy (FNAB) of the pancreas in 81 consecutive patients with suspected chronic pancreatitis or pancreatic neoplasm. The final diagnosis was pancreatic cancer in 54 patients and chronic pancreatitis in 27 patients. CA 19-9 assessment, US, CT, and FNAB were considered nondiagnostic, respectively, in 0%, 25%, 19%, and 6% of cases. When a definite diagnosis was rendered, the positive predictive value was 90% for CA 19-9 assessment, 95% for US, 98% for CT, and 100% for FNAB; the negative predictive value was, respectively, 69%, 95%, 86%, and 100%. The accuracy of all diagnostic and nondiagnostic studies was 81% for CA 19-9 assessment, 72% for US, 77% for CT, and 94% for FNAB. It is concluded that CT-guided pancreatic FNAB is the most reliable examination for enabling differential diagnosis of pancreatic cancer and chronic pancreatitis. When the pancreas is well visualized at US, the negative predictive value for pancreatic cancer is more accurate than that of CA 19-9 assessment and CT.

[Invasive carcinoma of the cervix uteri (Stage IB-IIB). Comparison of CT and MR for the assessment of the parametrium].

In the patients with invasive cervical carcinoma, the accurate assessment of parametrial invasion greatly affects the therapeutic choice between surgery and radiation therapy. As a matter of fact, surgery is usually performed only in the patients with carcinoma confined to the cervix, whereas those with parametrial involvement, or more advanced stages, are treated with radiation therapy. This prospective study was aimed at investigating the comparative adequacy of CT and MR imaging in assessing parametrial status in the patients with invasive cervical cancer. Twenty-one consecutive patients, with histologic diagnosis of cervical carcinoma, were investigated. All of them were clinically considered as having invasive cervical cancer (FIGO stages IB-IIB), and subsequently underwent surgery. In all cases, detailed histology of the parametrium was obtained. Pathologic data were compared with CT and MR findings in all cases. As for assessing parametrial involvement by cancer, CT had 62% accuracy, 63% sensitivity, and 60% specificity, versus MR imaging 81% accuracy, 69% sensitivity, and 80% specificity. Therefore, MR imaging appears to be superior to CT in assessing the parametrial status of patients with invasive cervical carcinoma; the method yields valuable information for treatment planning.

[Treatment of obliterating arteriopathies of the lower limbs using angioplasty. Comparison of 2 techniques of recanalization: Nd-YAG laser and radiofrequency thermal probe].

The Authors report their experience about 20 cases of femoro-popliteal revascularization: in 15 cases a Nd-YAG Laser was used; in 5 cases a unity which employs radiofrequency energy to heat the tip (Thermoprobe) was utilized. We obtained an immediate recanalization in 19 out of 20 superficial femoral arteries with two early thrombosis. In 12 patients of the first group a significant increase of the ankle-brachial index (45%) was seen; it remained unchanged during the follow-up. We had successful revascularization in all the 5 patients of the second group (Thermoplaster). In selected cases, when the lesion is shorter than 10 cm, not completely calcified, and there is a valid run-off, Laser angioplasty could be a valid alternative to surgery or medical therapy.

CT-guided pancreatic percutaneous fine-needle biopsy in differential diagnosis between pancreatic cancer and chronic pancreatitis.

Differential diagnosis between pancreatic cancer and chronic pancreatitis is still difficult to establish. In 63 patients with suspected pancreatic neoplasm we performed: serum CA 19-9 assessment, abdominal ultrasound. CT scan and CT-guided pancreatic percutaneous fine-needle biopsy. The conclusive diagnosis was pancreatic cancer in 40 patients and chronic pancreatitis in 23 patients. With regard to the differential diagnosis, sensitivity and specificity were respectively 80% and 78% for serum CA 19-9, 75% and 65% for abdominal US. 85% and 70% for CT scan. 00% and 87% for percutaneous fine-needle biopsy. We conclude that CT-guided percutaneous fine-needle biopsy is the most reliable method for differential diagnosis between pancreatic cancer and chronic pancreatitis.

[Assessment of the myometrial infiltration of endometrial carcinoma (FIGO stage I-II). The accuracy of magnetic resonance (1.5 T)].

Thirty-three patients with histological diagnosis of endometrial carcinoma were studied with MR imaging at 1.5 T. All patients were clinically diagnosed as having stage I or II disease (FIGO) and underwent abdominal hysterectomy. MR tissue characteristics and morphological features were compared with morphological features were compared with microscopic pathological findings, in all patients. The following initials were adopted for statistical comparison: M0 = tumor confined to endometrium; M1 = infiltration of the inner third of myometrium; M2 = invasion of the central third of myometrium; M3 = infiltration of the outer third of myometrium. T1-weighted (SE 600/30) and T2-weighted (SE 2000/35.90) (SE 1500/28.60) images were obtained in sagittal plane (section thickness = mm 4). Overall MR accuracy in determining the extent of myometrial invasion was 78%. Correct evaluation of myometrial invasion is essential in patients with stage I or II endometrial carcinoma who are to undergo transvaginal hysterectomy without lymphadenectomy (M0, M1).

[Conventional radiography and digital radiography of the thorax: evaluation of anatomo-radiological parameters].

We report the results of a comparative study of digital and conventional chest radiographs to detect normal anatomical structures of the thorax. A digital Toshiba unit (TCR 201) was used to examine 100 selected patients who were diagnosed with no chest pathologic conditions. The images in both modes were submitted for interpretation to five radiologists. The depiction of nine normal anatomical structures was more accurate on digital than on conventional radiographs. The mean confidence levels achieved in viewing digital images were higher than those obtained with conventional radiographs. This difference was statistically significant (p = 0.002).

[Glucose-6-phosphate dehydrogenase and succinate dehydrogenase in metastatic cells of Lewis lung carcinoma].

By the experimental model of the Lewis lung carcinoma we studied the activity of glucose-6-phosphate-dehydrogenesis (G6PDH) and succinic-dehydrogenesis (SDH) in the carcinoma itself, the lung metastases, the peripheral blood. A set of carcinoma, lung sections, peripheral blood smears were studied by histochemical techniques day by day since the 3rd day to the 15th one after the carcinoma grafting. The reported results allow us to conclude that G6PDH looks like a determinant of metastatic cell survival and growth and can be regarded as a marker of metastatic cells in the present model.

Internal hernia with volvulus and intussusception: case report.

An 82-year-old male presented to the hospital because of acute exacerbation of abdominal pain and biliary vomiting. Contrast-enhanced computed tomography of the abdomen was performed. A left paraduodenal hernia associated with volvulus, intussusception, and bowel wall ischemia were radiologically diagnosed. Surgery confirmed the diagnostic imaging findings. We present the first case of an association of these acute abdominal conditions.

Value of abdominal sonography and MR imaging at 0.5 T in preoperative detection of pancreatic insulinoma: a comparison with dynamic CT and angiography.

BACKGROUND: Abdominal sonography, computed tomography (CT), angiography, and magnetic resonance (MR) imaging are the most widely used modalities for preoperative localization of insulinomas. CT and angiography are generally considered the techniques of reference, and the role of sonography and MR imaging in these patients is controversial. The purpose of this study was to compare these four modalities in a group of patients with pancreatic insulinoma and determine an effective radiological approach to this disease. METHODS: Twenty-eight patients with clinical and biochemical signs of pancreatic insulinoma underwent abdominal sonography, MR imaging at 0.5 T (spin echo technique), bolus dynamic CT, and digital subtraction angiography. Examinations were evaluated independently for the presence, size, and location of the lesions; preoperative diagnoses were compared with surgical findings based on palpation and intraoperative sonography. Tumoral vascularity was histologically graded. Sensitivities of the four imaging techniques were calculated and compared with the size, location, and vascularity of the tumors. Detection rates of combined techniques were finally determined. RESULTS: At surgery, 29 lesions in the 28 patients were found (range = 0.8-4.3 cm, average = 1.65 cm). Sensitivities of abdominal sonography. MR imaging, CT, and angiography were 79.3%, 65.5%, 44.8%, and 69% respectively. Correct localization of tumor was achieved in 96.6% of cases by a combination of sonography and MR imaging and in 72.4% of cases by using CT with angiography. CONCLUSION: In our experience, sonography and MR imaging performed well in the preoperative detection of pancreatic insulinoma. Therefore, we believe that the combination of abdominal sonography and MR imaging may represent the first radiological approach in clinically suspected insulinomas and that CT and angiography should be reserved for negative and/or doubtful cases.

MRI assessment of coronary stents.

Several types of coronary stents are poorly visualized by fluoroscopy. Until now, the only reliable procedure to establish coronary stent patency is to repeat coronary angiography. Purpose of this study was to localize previously placed coronary artery stents and to verify their patency with MR angiography. 13 stented coronary arteries (6 RCA, 5 LAD, 2 LCx) were prospectively examined with MR angiography one day to 8 months after stent placement. 18 amagnetic stents (6 Palmatz-Schatz, 6 Multilink, 5 Crossflex, 1 Wiktor) were imaged. Imaging was performed on a 1.5 T MR-system (GE Signa Horizon Echo Speed) with a phased array multicoil. Segmented k-space fast gradient-echo sequences (TR/TE/FA = 11/4.5/25 degrees; views per segment = 8; matrix = 256 x 160; thickness = 3 mm; no gap) were acquired with and without fat suppression at several phases of the cardiac cycle within a single breath-hold. Images of consecutive slice location were shown in a cine format. Correlation with coronary angiography (standard of reference) was performed in all patients. No MRI-related adverse events were observed. All the stents were visualized as areas of signal loss. The length of the signal loss corresponded to the length of the stents in all 18 cases. Stents were more directly visualized on images without fat suppression. In all cases the patent blood flow distal to the stents was observed as a high-signal linear structure distal to the signal void, corresponding to stent patency at coronary angiography. It was concluded that breath-hold cine coronary MR angiography is a safe technique to visualize coronary artery stents. Stent location and patency can be noninvasively established with this technique.

The role of radiology in the diagnosis and management of Takayasu's arteritis.

In the last years new computer-based imaging techniques, like color Doppler sonography, Computed Tomography, and Magnetic Resonance, have allowed a non-invasive approach to vascular diseases, partially replacing angiography, and increasing the role of radiology in the diagnosis and management of many chronic diseases, such as Takayasu's arteritis. Simultaneous evaluation of luminal and vascular wall changes may now allow a simpler diagnosis of this condition also in its early phase and the effective therapy monitoring. Application of new procedures of interventional radiolgy provides a safer and more conservative correction of late steno-occlusive complications. Familiarity with the different imaging features of Takayasu arteritis will permit a more accurate clinical diagnosis and management of this insidious disorder.
