Endothelial cells in dominant cornea guttata.

A family with autosomal dominant cornea guttata in three generations is described. The investigated pedigree consisting of 25 healthy patients and 6 patients with cornea guttata. A few guttata were already present at an early age and very pronounced corneal endothelial guttata appeared in the middle-aged patients. No other ocular pathology was associated with this condition and the endothelial cell counts were not affected by guttata excrescences. The corneal thickness was normal in the affected and non-affected members of the family while bullous keratopathy did not seem to be associated with dominant cornea guttata in this family.

Corneal endothelial cells in essential iris atrophy. A specular microscipic study.

The specular microscope was used to study the corneal endothelium in three patients with essential iris atrophy and three patients with anterior mesodermal disorders with uncertain diagnose. Pleomorphic and enlarged endothelial cells were found to be typical for essential iris atrophy. The changes were also present in one eye with normal pressure. In the patients with other types of iridal disorder the endothelial cells were normal in form. In one of these patients high intraocular pressure and surgery had seemingly led to enlargement of the endothelial cells. According to our findings, the specular microscope is a useful aid differentiating essential iris atrophy from other disorders of the iris.

HLA-B 12 and HLA-B 27 antigens and susceptibility to the corneal allograft reaction.

Our series comprised 100 penetrating corneal transplantations: 36 cases with HLA-B 12 and/or B 27 antigen in the recipient and, as controls, 64 cases without these antigens. The cornea was vascular in 44% of the HLA-B 12/B 27 group and in 40% of the controls. In the HLA-B 12/B 27 group AR developed in 13 of 36 cases, 36%; but in the control group only in one case (2%). In vascular and avascular cases the incidence of AR was equal. Among the high-risk cases of the HLA-B 12/B 27 group with definite mismatching of B 12 or B 27 antigens, 9 of the 14 recipients had developed AR. Six further recipients of this group had untyped grafts, and AR occurred in 4 of the 6, but in none of the 16 cases that were matched for B 12 or B 27 antigens. The value of a compatible graft, at least to the carriers of HLA-B 12 and/or B 27 antigens, is thus confirmed.

Endothelial cells in the glaucomato-cyclitic crisis.

In 21 patients with unilateral glaucomatocyclitic crisis (Posner-Schlossman syndrome) the central corneal endothelium was photographed with a clinical specular microscope. In each patient the endothelial cell density was determined in both the affected and the contralateral healthy eye. In patients who had had several attacks of P-S the endothelial cell density was lower in the affected eye than in the healthy eye (16 cases). 4 patients seen after the first attack and 1 of 3 patients after the second attack had the same cell density in both eyes. Thus, loss of endothelial cells seems to be due mainly to the recurrent attacks of elevated pressure. The role of the inflammatory changes occurring in eyes with the P-S syndrome, especially keratic precipitates, is discussed.

Glaucomatocyclitic crisis.

Ten patients with glaucomatocyclitic crisis were examined for glaucoma during the interval. The follow-up period was over ten years. One patient had primary open angle glaucoma and two had suspicion of glaucoma. These findings confirmed our previous concept of an association between glaucomatocyclitic crises and open angle glaucoma. Six patients were examined during acute attacks. Focal segmental ischaemia of the iris of different degree was typical during the attack, later in the immediate postattack phase congestion, leakage and even extreme hypotony were found. Iris angiography during the interval showed generally a rich vascular pattern, and in some cases, tortuous vascular tufts. Stromal atrophy and atrophy of the sphincter were considered secondary to pressure attacks. The role of the abnormal reactivity of the vessels of the ciliary vascular system on the development of acute attacks is discussed.

Endothelial cells in capsular glaucoma.

The clinical specular microscope has made it possible to study corneal endothelial cells in vivo. In this study we report for the first time cell densities of unilateral glaucoma patients compared to the normotensive control eyes of the same patient. The corneal endothelium of 27 unilateral capsular glaucoma patients was photographed with a clinical specular microscope. The endothelial cell density was lower in the affected eye than in the normotensive fellow eye in 15 cases. In 10 patients the cell density was the same in both eyes. In two cases the glaucoma eye had a higher density than the fellow eye. Comparison of the above groups showed a statistical difference in the number of glaucoma eyes with a lower cell density (15 lower against 2 higher). The endothelial cell density could not be correlated with the duration of treatment of severity of the glaucoma.

Photography of the nerve fiber layer in retinal disturbances.

The diagnostic value of nerve fiber layer photography is assessed. The series comprised 39 patients with various neuro-ophthalmological diseases. Photography of the nerve fibers in the retina is time consuming and requires skill on the part of the photographer. Differences in the papillomacular nerve fiber layer are difficult to distinguish using the present methods. The wasting of nerve fibers could be documented by photography in advanced cases with bitemporal hemianopia, but not in small relative scotomas. Cases with homonymous hemianopia are interesting because lesions in the optic tract can be differentiated from affections of the second neuron on the basis of retinal nerve fiber atrophy. In demyelinating diseases inspection of the retinal nerve fiber layer is of clinical importance and diagnostic changes appear,

Corneal transplantation reaction in avascular keratoconus patients due to HLA-associated immune aberration against infection. A hypothesis.

HLA-B12 and HLA-B27 appeared to be characteristic of avascular keratoconus patients at risk of developing a corneal allograft reaction, which was mainly of late type and triggered by a nasopharyngeal infection. Equally incompatible grafts to patients with HLA-B5 had an uneventful course. This may be inference to the role of HLA-associated immune resistance factors, which necessitate the search for histocompatible corneal grafts also for avascular recipients bearing such indicators of risk.

[Mykosis of soft contact lenses (author's transl)].

A 63-year-old patient suffering from old sclerokeratitis was fitted with a therapeutic soft contact lens one and a half years after corneal transplantation and half a year after cataract surgery. The patient was on oral and local cortisone treatment. The first three months were uneventful with an aphakic high water content lens worn continuously. This lens was lost and she was given a B-L plano T. One month later the patient was admitted to the hospital with a red eye and a large corneal ulcer. Two white spots were noted on the contact lens. In a frozen section yeast-like structures were seen on the lens that partly invaded the lens material. Fungal culture from the other deposit revealed candida tropicalis and fusarium. The corneal ulcer healed with antimycotic therapy. The possible relationship between cortisone treatment, corneal ulcer and fungal contamination of the lens is discussed.

The cyclopentolate provocative test in suspected or untreated open-angle glaucoma. IV. Fluorescein angiography of the vessels of the iris in open-angle glaucoma eyes with a positive cyclopentolate response.

Fluorescein angiography of the iris (IFAG) was performed on 15 patients with a positive cyclopentolate response (IOP elevation greater than or equal to 8 mmHg) in 17 eyes to the cyclopentolate provocative test. The chamber angles were open in all the eyes. Seven of the responder eyes had capsular glaucoma undergoing treatment, six had simple glaucoma, two had pigmentary glaucoma and two suspicion of open-angle glaucoma. The object was to study with IFAG whether vascular changes can be established in the iris of the responder eyes such as could have a role in the elevation of IOP. All the eyes with capsular glaucoma displayed vascular changes, vasoproliferation and fluorescein leakage from the iris vessles. No other vascular changes were seen in the irises of the responder eyes. IFAG revealed no differences in the iris vasculature between responder and non-responder eyes. A vascular aetiology for the IOP elevation in responders is improbable.

Salzmann's nodular degeneration of the cornea.

Eleven corneal specimens from nine patients with Salzmann's nodular degeneration of the cornea, together with all available clinical information, were collected for this study. The specimens were examined by light and electron microscopy. An antecedent keratitis was diagnosed by history and microscopic findings in every case. The corneal epithelium showed degenerative changes, its thickness varied, and in nodular areas it often consisted of only a single layer of flattened epithelial cells by light microscopy. Bowman's membrane was missing over the nodules, and in this zone there was excessive secretion of a basement membrane-like material. Hyaline degeneration of collagen, cellular debris, and electron-dense hyaline deposits were seen in the collagen of the nodules. The number of fibrocytes in the nodules varied from many that were active to a few that were degenerating. External irritation because of poor epithelial protection was interpreted as a causative factor, although other tissue repair mechanisms may also have played a role.

Lens changes in a galactosemic fetus.

The first child of a Spanish-American couple died presumbably from galactosemia. Early in the mother's fourth pregnancy, the parents' red blood cell levels of galactose 1-phosphate uridyl transferase were measured and both parents were found to be heterozygous (carriers) for transferase deficiency. A prenatal diagnosis of galactosemia in the fetus was made with the aid of amniocentesis and tissue culture. Abortion was induced when the fetus was 5 months old. Biochemical studies of lung and liver tissues confirmed the diagnosis of galactosemia. Cataractous changes were found by electron microscopy in both fetal eyes. Lens epithelial cells showed degenerative changes and a tendency to proliferate into two cell layers. Edema, liquefaction, and a variation in density characterized the lens cells.
