Pseudo-Kaposi sarcoma induced by minor trauma in a patient with Klippel-Trenaunay-Weber syndrome.

Arteriovenous malformations (AVM) are vascular anomalies containing a communication between an artery and a vein without an intervening capillary bed. In 1990, Klippel and Trenaunay reported a patient with limb overgrowth, cutaneous angiomata and varicose veins. In 1918, Parkes Weber mentioned the additional feature of AVM terming the condition 'hemangiectatic hypertrophy'. Most authors now consider both the Klippel-Trenaunay syndrome and the syndrome of multiple congenital arteriovenous fistulae to be two forms of Parkes Weber's hemangiectatic hypertrophy. Clinical and histological 'Kaposi sarcoma'-like lesions in the Klippel-Trenauny-Weber syndrome are well documented. We report an additional case appearing after minor trauma.

Agminated blue nevi: case report and review of the literature.

Blue nevi may rarely appear in multiple form and grouped in a circumscribed area, a pattern of arrangement that is more properly designed under the term agminated blue nevi. In this paper a new case with light and ultrastructural studies is described, and the previously reported cases are reviewed. Histologically, there was a characteristic perifollicular arrangement of dermal melanocytes, most of which showed ultrastructurally an extracellular sheath. Agminated blue nevi seem to be benign lesions, but because of their rarity, no definite prognosis can be given.

Congenital segmental dermal melanocytosis in an adult.

BACKGROUND: Dermal melanocytosis is characterized by the presence of melanocytes in the dermis. There are several morphologic forms, such as the blue nevus, the mongolian spot, the nevus of Ota, and the nevus of Ito. In this article, we report an unusual case of dermal melanocytosis. OBSERVATIONS: A 28-year-old white woman was referred to us, as she had an extensive, speckled gray-blue pigmentation on the right aspect of her trunk that was in a segmental pattern. The lesion had been present at birth. Histologic and ultrastructural examinations revealed sparse, elongated dermal melanocytes. CONCLUSIONS: We comment on the differential diagnosis with classic recognized forms of dermal melanocytosis. We also review other isolated cases of unusual dermal melanocytosis. This is a distinct case, clinically different from previously reported cases in the literature; we propose the term "congenital segmental dermal melanocytosis" to name this case.

[Bacillary necrosis in larvae of the bivalve Euvola ziczac (Linnaeus, 1758) caused by a Pseudomonas sp].

The incidence of bacteria as etiological agents of diseases in larvae of bivalve mollusks, is well documented in the literature. In this study, a series of test were performed to identify and estimate pathogenic level of marine bacteria isolated during an epizootic in a larval culture system of the tropical scallop Euvola ziczac. Such bacteria was identified as Pseudomonas sp. belonging to the group alcaligenes. The bacterium produced not only lethal effects on the scallop larvae through infection (invasive action), but also a possible pathogenic activity through endotoxins associated to bacterial cell-wall. In addition, it is possible that the isolated bacteria could act as a pathogen with members of the genus Vibrio, which was also present during the epizootic outbreak in the culture system. Susceptibility of Pseudomonas sp. to commercial antibiotics was remarkable.

Association between left-handedness and allergy: a reappraisal.

There are conflicting reports in the literature about the possible association between left-handedness and immune disorders, including autoimmune diseases as well as allergies. In this study we compared the distribution of right- and left-handers, assessed with the Edinburgh Handedness Inventory, in a group of patients consulting an allergy clinic and a control population with a similar sex and age distribution. There was no overall association between left-handedness and allergies, although we found a tendency towards left-handedness in patients whose allergic symptoms started before puberty, suggesting that left-handers may have an increased predisposition to allergic disease that manifests itself during early life.

[Ischemic cerebrovascular accidents in childhood].

Authors review 53 children, aged 0 to 14 years, affected with cerebrovascular ischemic strokes. Largest aetiological groups were: a) congenital heart disease, 16 patients; b) arteritis of unknown cause, 11; c) idiopathic arterial occlusion without arteritis images on angiography, 7; d) moyamoya disease, 6; and d) local or systemic infections, 5. The mode of onset was as completed stroke in 72% and stroke in evolution in 24%. After acute stage 17.6% of patients presented other definitive strokes, 11.7% suffered only transient ischemic strokes (TIA), and 4% reversible ischemic neurologic deficits (RIND). Mean follow-up was 4.36 years, 9.8% of patients died, 11.8% recovered completely and 52.9% improved after initial stroke. Poor global evolution was associated with heart disease (p less than 0.05) and with onset of strokes before age 2 (p less than 0.05). Most important sequelae, besides motor impairment, were epilepsy (49%) and mental retardation (50% got less than IQ 80). Late epilepsy was associated with seizures at onset (p less than 0.05). Clinical factors of adverse mental development were: a) seizures at onset, b) late epilepsy and c) stroke before age 2. 66% of cases had two or more arterial lesions in the same or in different arterial trees. Therefore, embolic and arteritic factors probably play an important role in infancy and childhood stroke.

[Triad syndrome in the neonatal period: an anatomoclinical study of 11 cases].

Eleven newborn children with clinical and anatomic characteristics of Prune-Belly syndrome were studied. This establish an approximate incidence of 1:28,000 newborns. All these children died in the first-days of life, nine of them as a result of pulmonary hypoplasia secondary olygohydiamnios. All of them except one had urinary tract dilatation and in four anatomic stenosis of urethra was found as the cause of obstruction. These findings support mechanical theory more than a primary mesodermic disorder as a pathogenic mechanism. All this, together with establishment of an early prenatal diagnosis, opens the possibility of an intrauterine treatment which could free the urinary tract and prevent oligohydramnios and its, sequelae.
